1.A Case of Aneurysmal Benign Fibrous Histiocytoma.

Aneurysmal benign fibrous histiocytoma(ABFH) is a relatively rare variant and has a distinctive histopathological features, that is collections of capillaries, foci of hemorrhage, siderophages, and foamy macrophages surrounding cleft-like and cavernous blood filled spaces in the tumor. But, in spite af the distinctive features, it is not infrequently confused with malignant melanoma, angiomatoid malignant fibrous histiocytoma, and vascular tumors such as spindle cell hemangioendothelioma, nodular Kaposi's sarcoma and angiosarcoma. We report a case of ABFH developed on the left upper extremity of a 41-year-old man.

3.Hemorrhagic Chicken Pox.

Primary varicella infection usuelly runs a benign clinical course in the helthy population. However, hemorrhagic chicken pox presents with a very extensive eruption of hemorrhagic vesicles in patients with decreaed platelets or impaired immunity and is accompanied by severe constitutional symptoms. A 7-year-old male was admi due to abdompal pain for 1 day and peneralized vesicular eruption for 5 days. The eruption first appeared on the trunk and then spread to involved face, scalp and extrsmities. The skin rash was compatable with varicella but despite the administration of acclovir intravenously, the vesicular eruption became hemorrhagic. Ten days after admission, having experienced cardisc arrest twice and with his mental state in coms, he was diseharged as his situation was hopelesa. We have experienced a giatient diagnosed as having hemorrhagie, chicken pox complicated by encephalopathy.

4.A Clinical Analysis of Pediatric Cardiopulmonary Resuscitation.

BACKGROUND: The pediatric CPR is rather uncommon practice than adult CPR in emergency room. Therefore, few interest and studies were focused in pediatric CPR. The object of this study is to make an analysis about pediatric CPR performed within hospital setting. METHOD: From January 1. 1990 to December 31. 1996, a total of 59 patients, less than 15-year-old, who received cardiopulmonary resuscitation were subject of this study. The study was done retrospective chart analysis for sex, age, weight, the place of cardiac arrest, initial EKG rhythms, endotracheal tube size, the causes of arrest, CPR time, ROSC, the amount and types of CPR drugs used during resuscitation. Wilcoxon rank sum test and chi2 test were used to compare the dose of drugs during CPR in each group. RESULTS: There were no statistical difference between ROSC and non-ROSC group in place of arrest, initial EKG rhythms, CPR time, dosage of CPR drugs. CONCLUSION: The uniform reporting guideline of pediatric advanced life support should be used for future pediatric CPR study, such as Utstein style guideline in pediatric CPR.

5.Clinical Characteristics in Korean Patients with Insulin Receptor Antibody Positive Acanthosis Nigricans.

BACKGROUND: Insulin resistance syndrome shows extreme insulin resistance and is associated with acanthosis nigricans. We can differentiate it into type A insulin resistance that has insulin receptor defect, and type B insulin resistance due to insulin receptor autoantibody. Type B insulin resistance was firstly described by Kahn in 1976. It was often found in adult female and showed autoimmune characteristics. As clinical characteristics, there are hyperglycemia, hypoglycemia, hyperinsulinemia, extreme insulin resistance and acanthosis nigricans. METHODS: We investigated 17 insulin receptor autoantibody positive cases and 8 cases of normal control who had visited Wonju Christian Hospital from October 1994 to December 1995. Among insulin receptor autoantibody positive subject, male was 4 cases and female 13 cases. Their mean age was 42.2. We compared patients who had insulin receptor antibody positive acanthosis nigricans(IRA) with normal controls. IRA patients were 6 cases(35.3%) of nomal glucose tolerance, 11 cases(64.7%) of abnormal glucose tolerance including overt diabetes mellitus. RESULTS: The 11 cases(64.7%) among IRA patrents were obese and 13 cases(76.5%) had hyperininsulinemia. In IRA patients, mean serum insulin concentration during oral glucose tolerance test was 202.1mU/mL and it was greater than 46.3 of normal controls. Insulin sensitivity in 1.79mg/L * mM * xmU * min normal controls was higher than 0.74mg/L * mM * min of IRA patients. CONCLUSION: IRA patients showed abnormal glucose tolerance including overt diabetes millitus, severe insulin resistance, hyperinsulinemia and obesity.

6.Comparative Histopathologic and Immunohistochemical Studies of Epidermal Cysts of Palm & Sole , Other Epidermal Cysts and Pilar Cysts.

BACKGROUND: Superoxide disrnutase(SOD) is an essential enzyme oi protective defense mechanism in cellular compartments against the potential cytotoxicity of superoxileinion. It seems SOD activity is increased in psoriasis because of an increase in oxygen intermediates dae to hyperoxia. However SOD activity is decreased paradoxically. OBJECTIVE: In order to understand the pathophysiology of psoriasis and to elucidate the significance of SOD aetivity in keratinocyte proliferution, we measured the SOD activities in psoriatic and in normal skin. METHODS: Authors assayed the level of SOD activities in 16 psoriatie and 16 normal skin specimens, which were obtained from face/neck (1and 4 speeimens), forearm(5 and 4 specimens), trunk(6 and 3 specimens), and lower extremiy(4 and 5 specimens). We also investiga.ed changes of SOD activity by age and regional differences of' SOD sctivities in the psoriatic and normarl skins. RESULT : First, activities of tot,al SOE Mn-SOD and Cu, Zn-SOD were L2.75+8.15, 4.28+ 3.62, 8.48+ 6.37 units/mg protein respect,ively in the psoriatic skin. Activities of toal SOD, Mn-SOD and Cu, Zn- SOD were 17.63+9.10, 3.38+ 1..85, 1.25+8.16 units/mg protein respec,ively in the normal skin. There was a significant differenc between psoriatic and normal skins in C-SOD activity(p<0.05). Second, the changes of total SOD, Mn-FOD and Cu, Zn-SOD activities showed no significant differences between the age groups. Third, in sun exposed area and unexposedire there were no significant differences between the psoriatic and normal skins in SOD activities. Elut in the normal skin, there were significant differences between sun exposed area and unexposed area in total SOD and Cu, Zn-SOD activities. CONCLUSION: There is a signiificant difference between psoriatic and normal skins in Cu, Zn-SOD activity. Namely, the Cu, Zn-SOD activity is significantly decreased in psoriasis.

7.Immunohistochemical Comparison of androgen Receptor Expression from Lesional and Non - lesional scalp of Androgenetic alopecia.

BACKGROUND: Androgens stimulate the growth of hair in various are of skin including the pubis, axilla and beard, whereas in the scalp androgens promote baldness in genitically predisposed individuals. Androgens bind to specific intracellular receptors, which modulate saecific gene expression and so affect hair loss. Androgenetic isldness is first apparent on the frontalegion, with recession of the hair line. As this progresses, aloperia also develops on the vertex. But the osterior and lateral scalp area is spared, even in the most advanced eases, and even in old age. OBJECTIVE: The purpose of our study is to find out the difference of androgen receptor expression in lesional and non lesional scailp of androgenetic alopecia patients by iminunohistochemical staining. METHODS: Biopsy specimens were taken from its lesion(vertex) and its non lesion(occiput) of 3 patients with androgenetic alopecia and 4 normal controls. The deteatic n of androgen receptor was performed by an immunohistochemical technique(streptavidin peroxidasc method) using monoclonal antibody against androgen receptor. RESULTS: Androgen receptors were strongly expressed in eccrine land and sebaceous gland, and weakly expressed in epidermal cells, hair infundibulum, hair bulb epith-lium and dermal hair papilla. CONCLUSION: There was no significant difference of the expression if androgen receptors in lesional and non lesional scalp of androgenetic alopecia.

8.A case of coexistence with linear and plaque porokeratosis.

We report herein a case of the coexistence of linear and placue porokeratosis in a 20-year-old female patient. The patient showed the typical clinical picures of linear and plaque porokeratosis on the right wrist, dorsum of the hand, dorsolater; I aspect of the fingers and axilla, respectively. Histopathologically, these lesions showed the typical cornoid lamella. We tried to remove the lesions with 75 100% trichloroacctic acid but could not gain a sat.isfact.ory effect.
