2.Nonpigmenting Fixed Drug Eruption to Etodolac.

Nonpigmenting fixed drug eruption is a form of fixed drug eruption which heals without pigmentation. To leave no traces makes the diagnosis difficult. A 37-year-old man pressented the erythematous patches on both wrists and the left thigh. Several hours before the eruption, he had taken oral drugs for the treatment of osteomyelitis. He had had the same eruptions which were diagnosed as an allergic contact dermatitis, 8 months ago. The patch tests with medicated drugs were negative, but oral provocation with etodolac brought the same eruptions.

3.A study on type 1 allergy to house dust mite in patients with atopic dermatitis.

The authors investigated the radioallergosorbent test (RAST) for the specific IgE and the prick test with hose dust mite in 129 patients with atopic dermatitis(AD) wit4 four or more family numbers. RAST and prick test results were described according to family background of respiratory atopy(RA) and severity of AD. The results are summarized as follows : l. A comparison between the prick test results and RAST reactions gave a concordance of 88%. The prick test was more sensitive than the RAST. 2. The level of specific IgE in patients with personal history(PH) or family history(FH) of RA was significantly increased compared to those in patients with only AD. 3. In patients without PH and FH of RA, the level of specific IgE in patients with FH of AD was significantly increased compared to those in patients without FH of AD. 4. The level of specific IgE did not reflect the severity of AD. From the above results, it could be concluded that type I allergy to house dust mite is not directly related to AD and it occurs predominantly in those patients with AD wha have a predisposition to RA.

4.A Case of Allergic Contact Dermatitis to Resorcin in Skin Marking Solution Occurred During Radiation Therapy.

Resorcin is widely being used in the fields of the therapeutics, cosmetics, industry, but allergic contact dermatitis is an infrequent adverse reaction. We have experienced a case of allergic contact dermatitis to resorcin. A 59-year-old male with Kaposi's sarcoma on the right foot developed linear erythematous vesicular eruption along the marking areas 4 days after application of Castellanis paint, used as a skin marking solution for radiation therapy. He showed patch test positive reaction to Castellani's paint as well as its individual components, resorcin and phenol.

5.Effect of UVB Phototherapy on Pruritus of Pityriasis Rosea.

Pityriasis rosea is sometimes accompanied with considerable pruritus. Ultraviolet B irradiation is reported to be beneficial in relieving pruritus. The purpose was to evaluate the effect of UVB phototherapy on severe pruritus of pityriasis rosea. Nine patients with severe pruritus had UVB irradiation three times a week Five patients became symptom free and three showed a marked improvement. One dropped out. UVB phototherapy could be an effective modality for relieving pruritus in pityriasis rosea.

6.A Study on the T Lymphocyte Subsets and Serum IgE in Patients with Atopic Dermatitis.

This study was undertaken to elucidate any quantitative abnormalities of Tlymphocyte subsets (helper/inducer T cell; T cell and suppressor/cytotoxic T cell: T cell) and IgE levels in the peripheraI blood in atopic dermatitis patients by using monoclonal antibodies and enzyme immunoassay, respectively. In addition we have tried to ascertain whether there are any correlations betveen such immuiiologic abnormalities and the severity of the disease. The results showed that there were significant decrease in the mean percentage of T Cells and increase in the mean value of T/T cell ratios in patients with atopic dermatitis. These changes seem to relate to the severity of the disease when evaluated as groups. A significant increase in the mean value of serum IgE was found in atopic dermatitis patients and the change was related to the severity of the disease. But increase of serum IgE level did not correlated closely with the T cell abnormalities in each patients.

7.Air Cooling Effect on Pain Sensation during Dermatologic Laser Treatment.

BACKGROUND: Although most dermatologic laser systems were developed to damage target tissue selectively, the epidermal injury has been a major problem in treating skin lesions of colored people. Recently, to minimize the epidermal damage during laser treatment, superficial cooling is being used. Cooling is one of the well known methods to reduce pain sensation. OBJECTIVE: The purpose of this study was to evaluate the effectiveness of an air cooling in reducing pain associated with Q-switched Nd:YAG laser and pulsed dye laser treatment. METHODS: We measured the temperature of normal skin during air cooling and studied 13 patients treated with a 1064 nm Q-switched Nd:YAG laser or 585 nm pulsed dye laser treatment with and without air cooling system. The degrees of pain reduction rates with and without cooling were assessed by the visual analogue scales. RESULTS: 1. It was shown that cold air flow from 2 cm distance for 10 sec decreased skin temperature about 10.6 degreeC to 17.2 degreeC depending on the body sites. 2. The average of the pain scores was 38.08+/-21.05 on the cooled site as compared to 70.77+/-15.76 on the uncooled site. CONCLUSIONS: The cold air decreased the skin surface temperature like the other epidermal cooling system and it was very effective to reduce the pain on the cooled site associated with dermatologic laser treatment.

SUMMARY OF LITERATURE REVIEW: Many authors haute described the association between lumbar disc degeneration and segmental instability but it has not been delineated in detail. OBJECTIVES: To compare the MRI assessment of disc degeneration with the conventional plain X-ray evaluation of the intervertebral disc, in order to study lumbar segmental instability. MATERIALS AND METHODS: In 75 patients with low back pain and/or sciatica, we analyzed disc space height, angular displacement, and horizontal displacement on plain radiographs of the lumbar spine. These parameters were compared with the grade of disc degeneration as evaluated by magnetic resonance Imaging. STUDY DESIGN: We analyzed the association between grade of disc degeneration as evaluated by magnetic resonance imaging assessment and segmental instability as evaluated by plain radiographs of the lumbar spine. RESULTS: Disc space height was decreased In proportion to the grade of disc degeneration. Angular displacement was increased according to the grade of disc degeneration, but significantly less with severe degeneration, accompanied by a tendency to stabilization of the motion segment. Horizontal displacement was not correlated with the grade of disc degeneration CONCLUSIONS: The incidence of lumbar segmental instability increased in proportion to the grade of disc degeneration but significantly decreased with severe disc degeneration.
