1.Acute Gastric Anisakiasis: A case report.

We report a case of a 41-year-old female patient who suffered from the acute abdominal pain for several hours after eating raw sea-fishes. After the fibergastroscopy and the abdominal C-T scan, the clinicians found a gastric submucosal mass and performed the explolaparotomy to get an wedge of stomach. Sections revealed a larva surrounded by phlegmonous inflammation with intense eosinophilic infiltration in the widened gastic submucosa. The larva presented the characteristics of the Anisakis: two lateral chords with renette cell, thich smooth cuticle and well developed musculature.

2.The Morphology of the Mandibular Canal in Korean.

Ninety-three dry mandibles in Korean adults were radiographed and fifty-two hemimandibles were sectioned in an attempt to analyze the intramandibular courses of the mandibular canal and the internal structure of the mandibles. On radiographs, the intramandibular course of the mandibular canals was most in which located in the middle portion of the mandible, the shape of the mandibular canals was almost round and the average diameter of the mandibular canals was 2.0 +/- 0.4mm, encapsulated by thin cortical plates of 0.5mm thickness on sectioned specimens. In regard of facio-lingual relation, the prevalence of pattern which the mandibular canal ran nearly to the lingual cortical plate, and turned abruptly towards the buccal side was 70% of observed specimens. According to superior-inferior relation, the prevalence in which mandibular canals ran inferiorly, anteriorly and then, had an angle superiorly in the second mandibular molar region was 39.0%. Taken all together, in most of the case, the anterior border of the mandibular canal is located around the premolar region, and the mental foramen is opened nearly the second premolar. Therefore, it may be suggested that osteotomy has to be taken at the anterior place next to the canine region, at the 2.4mm below to the mental foramen on genioplasty.

3.Radiologic analysis of ossification of the posterior longitudinal ligament of the spine

Since calcification or ossification of the posterior longitudinal ligament of the spine was first described inJapan in 1960 by Tsukimoto, Terayama used the term of OPLL(Ossification of Posterior Longitudinal Ligament) in1964 and mainly reported in Japanese. But recently, the incidence of the OPLL has been reported increasing amongthe non-Japanese. Because of the OPLL may be associated wtih severe neurologic symptoms, which need to havesurgical decompression, exact diagnosis and analysis are necessary. The OPLL can be diagnosed by simple spine,conventional tomography, myelography and CT. Authors analysed radiologic findings of the OPLL in 8 patients, whowere diagnosed by simple spine. Conventional tomography, myelography and CT, and then performed spinal operation,at Hanyang University Hospital from March 1980 to June 1983. The results were as followings; 1. The age range wasbetween 45 and 63 years and most prevalent age was 6th decades(63%). 2. All of the patients were male. 3. All ofthe OPLL occurred in the cervical spine and predominant at the level of C3,4 (48%). 4. The most common length ofthe OPLL was 2 vertebral level. 5. Except one, which was discontinuous at the level of disc space, all of the OPLLwere continuous in length. 6. 7 of 8 OPLL were located in the midline, but one was in right. 7. The OPLL seemed tocorrelate with spondylotic changes. 8. Of all diagnostic procedure, CT provided better and exact visualization ofthe lesion with axial scan and sagittal reconstruction.

4.A Case of Congenital Arteriovenous Fistula at Left Subclavian Artery Accompanied by Notching on Superior Aspect of Rib.

High output heart failure due to congenital arteriovenous fistula is a rare disease and is difficult to treat. A 27 year old man, suffering from dyspnea, palpitation, lower extremity edema, and abdominal distension admitted May, 1987. At admission there were continuous thrill and bruit at left supraclavicular and infraclavicular area. The heart sound was irregular and rapid, and grade IV/VI pansystolic murmur was audible at the apex. Chest PA showed marked cardiomegaly with pleural effusion and notching at superior aspect of left 2nd rib. Selective left subclavian artery angiogram showed extensive congenital arteriovenous fistula at the area of left subclavian artery and its branches. After setrial feeding artery embolization and palliative operation, the fistula size decreased much with clinical improvement.

7.Role of Apoptosis in the Differentiation of the Parietal Epithelium of the Renal Corpuscle in the Developing Rat Kidney.

The purpose of this study was to establish the role of apoptosis in the developing renal corpuscle in the prenatal rat kidney. Kidneys from 14-, 16-, 18-, and 20-day-old fetuses[E-14, E-16, E-18, and E-20] were preserved by immersion or perfusion via the heart using Bouin`s fixative. Apoptosis was detected`by in situ nick end labeling method using ApopTag kit. In kidneys from E-14, apoptotic cells and bodies were found only in the mesenchymal tissue surrounding the developing nephrons. In kidneys from E-16, E-18, and E-20, apoptotic cells and bodies were located mainly in the columnar distal epithelium of the renal vesicle[future parietal epithelium] as well as in the parietal epithelium of the renal corpuscles of S-shaped bodies, stage III and IV nephrons. Apoptosis was not observed in the proximal part of renal vesicles or in the podocytes in renal corpuscles. In contrast, strong bel-2 immunoreactivity was present in the proximal part of the renal vesicle and in podocytes in S-shaped bodies, but gradually decreased in stage III and IV nephrons. The distal part of the renal vesicle had weak staining for bcl-2, and there was no bel-2 immunoreactivity in the parietal epithelium of S-shaped bodies, and stage III and IV nephrons. We conclude that bcl-2 is involved in the regulation of apoptosis during the differen-tiation of the parietal epithelium of Bowmann`s capsule.

8.Biological activities and acute oral toxicity of citronella and lemongrass oil.

This study was performed to investigate the antibacterial, antioxidant, and termite repellent effects of citronella oil (CiO) and lemongrass oil (LO). When the antibacterial activity against Staphylococcus (S.) aureus with various levels of antibacterial resistance were tested, a 0.05% concentration of CiO and LO completely inhibited the growth of all tested S. aureus strains. Evaluation of the antioxidant effect demonstrated that the 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging activity of CiO was 2~3 times greater than that of LO. Among trial products made with various combinations of CiO and LO, a CiO : LO ratio of 6 : 4 had the most potent termite repellent effects. Assessment of acute toxicity of the trial product showed that the LD50 was more than 2,000 mg/kg. Based on the above results, CiO and LO have antibacterial, antioxidant, and termite repellent activities. Therefore, both compounds could be potential termites repellent reagents.

9.Pseudomyxoma Peritonei A case of long term survival after operation.

Pseudomyxoma peritonei may result from implantation of benign or malignant tumor in peritoneal cavity and is filled with gelatinous material (termed "Jelly Belly") in abdominal cavity. Its origin is usually an appendiceal or ovarian mucinous adenoma or cystadenocarcinoma, but other primary origin such as uterus, intestine, pancreas and stomach umor have been reported. Generally, pseudomyxoma peritonei is slowly progressive and has low grade malignant potential. This report presents a unusual long term survival after evacuation of 15,000 cc of gelatinous material from abdominal cavity which was the low grade mucinous adenocarcinoma and a review of the current literature, management and new its concept. The origin of pseudomyxoma peritonei of this case was most likely from appendiceal mvcinous adenocarcinoma.

10.Radionuclide Cisternographic Findings in Patients with Spontaneous Intracranial Hypotension.

PURPOSE: Radionuclide cisternography may be helpful in understanding pathophysiology of postural headache and low CSF pressure in patients with spontaneous intracranial hypotension. The purpose of this study was to characterize radionuclide cisternographic findings of spontaneous intracranial hypotension. MATERIALS AND METHODS: The study population consists of 15 patients with spontaneous intracranial hypotension. Diagnosis was based on their clinical symptoms and results of lumbar puncture. All patients underwent radionuclide cisternography following injection of 111 to 222 MBq of Tc-99m DTPA into the lumbar subarachnoid space. Sequential images were obtained between 1/2 hour and 24 hour after the injection of Tc-99m DTPA. Radioactivity of the bladder, soft tissue uptake, migration of radionuclide in the subarachnoid space, and extradural leakage of radionuclide were evaluated according to the scan time. RESULTS: Radionuclide cisternogram showed delayed migration of radionuclide into the cerebral convexity (14/15), increased soft tissue uptake (11/15), and early visualization of bladder activity at 30 min (6/10) and 2 hr (13/13). Cisternography also demonstrated leakage site of CSF in 4 cases and 2 of these were depicted at 30 min. Epidural blood patch was done in 11 patients and headache was improved in all cases. CONCLUSION: The characterstic findings of spontaneous intracranial hypotension were delayed migration of radionuclide and early visualization of the soft tissue and bladder activity. These scintigraphic findings suggest that CSF leakage rather than increased CSF absorption or decreased production may be the main pathophysiology of spontaneous intracranial hypotension. Early and multiple imaging including the bladder and soft tissue is required to observe the entire dynamics of radionuclide migration.
