Trichilemmal horn(trichilemmal keratosis) is a rare keratinizing tumor that resembles a cutaneous horn. Histologically, it is characterized by an abrupt maturation of keratinocytes into lamellar keratin without the formation of a granular layer. We describe a case of a trichilemmal horn on the right cheek of a 78-year-old woman.

4.Financial Ratio Analysis for Developing Nursing Management Strategies in University Hospitals.

PURPOSE: The purpose of this study was to analysis the financial statements of university hospitals and to apply the results to build nursing management strategies. METHODS: Data on the financial statements of university hospitals were collected each hospital's homepage or internet search from February to June, 2010. Financial statements of 11 hospitals were analyzed using the 4 categories of financial ratio analysis method: liquidity, performant, growth and turnover. RESULTS: Overall results showed that the financial status the university hospitals were unstable, and many financial indicators did not meet financial standard ratios. Only 8 financial indicators of total 19 indicators satisfied financial standard ratios. CONCLUSION: The results of financial statements analysis suggest that nurse managers should develop the blue ocean strategy for diversification of nursing services to improve financial ratios of liquidity, performance, and growth. Using a unit-based just-in-time system for effective supply management would help to increase profits and to decrease costs of hospital by improving financial ratios of turnover.

5.Developing and Evaluating a Camp-style Leadership Enhancement Program for Nursing Students

PURPOSE: The aim of this study was to develop a camp-style leadership enhancement program (CLEP) and evaluate its effects to provide basic data for developing such programs for nursing students. METHODS: The study design was a randomized control pre/post-test. There were 35 participants each in the experimental and control groups for a total of 70 participants. The content and structure of the CLEP reflected 13 core concepts extracted using a systematic literature review. The program included a two-day camp-style program with eight modules consisting of lectures, individual and team activities, group discussions, team presentations, and feedback from the lecturer. The impacts of CLEP were measured using self, transformational, and servant leadership. RESULTS: Pre-homogeneity between the group participants' general and leadership characteristics was confirmed. After the CLEP, the experimental group showed a significant increase in self, transformational, and servant leadership. CONCLUSION: These findings suggest that universities utilize the performance-based self-learning CLEP to increase leadership among nursing students.

6.Soft and Hard Tissue Augmentation with/without Polydeoxyribonucleotide for Horizontal Ridge Deficiency: A Pilot Study in a Dog Model

Purpose:
To investigate the effects of simultaneous soft and hard tissue augmentation and the addition of polydeoxyribonucleotide (PDRN) on regenerative outcomes.
Materials and Methods:
In five mongrel dogs, chronic ridge defects were established in both mandibles. Six implants were placed in the mandible, producing buccal dehiscence defects. The implants were randomly allocated to one of the following groups: 1) control: no treatment; 2) GBR: guided bone regeneration (GBR) only; 3) GBR/PDRN: GBR+PDRN application to bone substitute particles; 4) GBR/CTG: GBR+connective tissue grafting (CTG);5) GBR/VCMX: GBR+soft tissue augmentation using volume stable collagen matrix (VCMX); and 6) group GBR/VCMX/ PDRN: GBR+VCMX soaked with PDRN. The healing abutments were connected to the implants to provide additional room for tissue regeneration. Submerged healing was achieved. The animals were euthanized after four months. Histological and histomorphometric analyses were then performed.
Results:
Healing abutments were gradually exposed during the healing period. Histologically, minimal new bone formation was observed in the dehiscence defects. No specific differences were found between the groups regarding collagen fiber orientation and density in the augmented area. No traces of CTG or VCMX were detected. Histomorphometrically, the mean tissue thickness was greater in the control group than in the other groups above the implant shoulder (IS). Below the IS level, the CTG and PDRN groups exhibited more favorable tissue thickness than the other groups.
Conclusion
Failure of submerged healing after tissue augmentation deteriorated the tissue contour. PDRN appears to have a positive effect on soft tissues.
7.Effect of substance P on production of interleukin-12 and interferon-gamma from peripheral blood mononuclear cells of chronic severe atopic dermatitis patients.

BACKGROUND: In atopic patients, allergen-specific T cells express the Th2 phenotype. It is known that interleukin-12 (IL-12) is a major cytokine for the induction of Th1 responses and that a reduced release of IL-12 in atopic dermatitis patient is closely related to reduced production of interferon-gamma(INF-gamma). Recently, it has been reported that substance P (SP) positive nerve fibers express increased IL-12 and that elevated levels of this neuropeptide were seen in AD patients. OBJECTIVE: This study was designed to examine the effects of SP and its fragment on the production of IL-12 and INF-gamma from peripheral blood mononuclear cells (PBMCs) of chronic severe atopic dermatitis (AD) patients, and moreover to establish neuro-immunomodulatory mechanisms in chronic severe AD patients. MATERIALS AND METHOD: The PBMCs from fifteen chronic severe AD patients and non-atopic individuals were stimulated with Staphylococcus aureus Cowan I strain. After stimulating the PBMCs, the effects of SP and SP fragments (SP1-4 and SP4-11) on the production of IL-12 and INF-gamma were analysed by enzyme-linked immunosorbent assay. RESULTS: The levels of IL-12 and INF-gamma were significantly decreased in un-stimulated AD patients than in non-atopic controls. In chronic severe AD patients, SP and SP4-11 significantly increased the production of both IL-12 and INF-gamma compared to non-atopic controls. The enhancing effect of SP and SP4-11 was most significant at the concentrations of 10-8M and 10-6M. When the spantide, an SP antagonist, was added to SP and SP4-11 stimulated PBMCs, the productions of IL-12 and INF-gamma were downregulated in chronic severe AD patients but not in non-atopic controls. In contrast, SP1-4 fragment did not show any specific cytokine production. CONCLUSION: These data suggests that SP and SP4-11, a carboxyl terminal of SP, are involved in the immunomodulation of chronic severe AD patient by regulating IL-12 and INF-gamma production, while SP1-4, an amino terminal of SP, has no effect on the cytokine production in AD patients.

8.Comparison of the Phakic and Pseudophakic Axial Length.

To determine the actual axial length of the pseudophakic eye, we mea-sured the axial length using ultrasound before and after cataract surgery in 157 eyes of 97 patient.The measured axial length of the pseudophakic eye with silicone or acrylic intraocular lens was longer than the axial length measured before operation.There was linear correlation between axial length measured before and after operation in cases of PMMA, silicone, or acrylic intraocular lens.The axial length corrected with ultrasonic velocity and central thickness of the intraocular lens was approximate to the mea-sure before operaion.So, we concluded that axial length is not actually elongated after operation and the exact axial length may be obtained by correcting the measured axial length of the pseudophakic eye using ultra-sonic velocity and central thickness of the intraocular lens.

9.Clinical observation of aseptic meningitis associated with mucocutaneous lymph node syndrome.

Clinical observation was carried out for 147 patients with mucocutaneous lymph node syndrome (MCLS) who were admitted to the Department of Pediatrics, Wonju Christian Hospital during the period from June, 1983 to June, 1992. A spinal tap was performed on 114 of these patients and the following results were obtained. 1) The incidence of aseptic meningitis in patients with MCLS was 52.6%. (60 cases of the 114 cases who had spinal taps (52.6%).) 2) The predominent age group for aseptic meningitis was 6 months to 1 year of age (35%). 3) For the majority, the value of glucose and protein in the CSF were within normal limit or only mildly elevated. 4) The neurologic manifestations associated with aseptic meningitis in patients with MCLS were irritability (78.3%), vomiting (25.0%), nuchal rigidity (11.7%), convulsion (5.0%) and facial nerve palsy (1.7%) in that order.

10.ST-Segment Elevation Myocardial Infarction as a Result of Coronary Artery Ectasia-Related Intracoronary Thrombus in a Patient with Liver Cirrhosis.

Coronary artery ectasia (CAE) is a rare condition defined as the dilatation of coronary artery to at least 1.5 times larger than the normal adjacent coronary artery. Clinical manifestations of CAE vary, ranging from asymptomatic to ST-segment elevation myocardial infarction (STEMI). Because of its rarity and clinical diversity, the best treatment strategy and prognosis for CAE remain unclear. We describe a case of STEMI caused by intracoronary thrombus formation within an ectatic area in a patient with liver cirrhosis (LC). The patient was successfully managed by thrombus aspiration only, without balloon angioplasty or stent implantation, and maintained by dual antiplatelet therapy with aspirin and ticagrelor, a potent new P2Y12 inhibitor.
