2.Two cases of natural human infection by Echinostoma hortense.

Two cases of human Echinostoma hortense infection were found in Seoul Paik Hospital. Their stools revealed echinostomatid eggs. After treatment with praziquantel (single does of 10-12 mg/kg) and purgation with magnesium salt, total 21 flukes were collected in one case. The flukes were 5.9-7.5 mm long, had 27-28 collar spines around their head, laterally deviated ovary and two tandem testes. They were identified as E. hortense Asada, 1926. The cases are 38-year and 20-year old men residing in Seoul, whose hometown is Chungsong-gun and Seungju-gun, Kyongsangbuk-do respectively. They had eaten raw flesh of various kinds of fresh water fishes (both cases) and/or salamander (latter case), which are considered possible sources of this fluke infection. They experienced abdominal discomfort and/or diarrhea. Hematology revealed 22 percent eosinophilia in the latter case, but it became 5 percent after the treatment. Echinostomatid eggs were not found after the treatment in both cases. These are the 4th and 5th human cases of E. hortense infection in Korea.

3.Bilateral Fractured of the Fabella

The fabella is the seasmoid bone occurring in about 10–30% of individuals and is located in the head of the lateral tendon of the Gastrocnemius muscle. Fracture of the fabella is a very rare condition. Since first reported by Jacob Sagel in 1932, only 5 cases of fabella fracture had been reported. However, there has been no bilateral case. We report a first case of bilateral fracture of the fabella with review of literature. This case was combined with rupture of anterior cruciate ligament and lateral collateral ligament on the right knee.

4.A case of natural human infection by Echinostoma cinetorchis.

A human case of Echinostoma cinetorchis infection was found in Seoul Paik Hospital on August 1987. Large trematode eggs, 98.9 x 62.7 micro-m in average size, were detected in stool examination. The eggs had a small-sized operculum and abopercular wrinkling. After treatment with praziquantel(single dose of 16.2 mg/kg) and purgation with magnesium citrate, an adult fluke was collected. It was 8.0 mm long, equipped with 37 collar spines around head crown, and had only one testis just behind the Mehlis' gland. It was identified as E. cinetorchis Ando et Ozaki, 1923. The patient is an 18-year old man residing in Seoul, and was hospitalized due to nephritis. He had eaten raw meat juice of fresh water fishes such as top minnows(Oryzias latipes) and loaches, or tadpoles, which are considered possible souce of this fluke infection. This is the 4th human case of E. cinetorchis infection in Korea.

5.Sucessful Treatment of A Giant Craniopharyngioma:Combined Preoperative Stereotactic Cyst Drainage and Microsurgical Radical Excision: A Technical Note.

Even though the craniopharyngioma was benign neoplasm pathologically, actually it is a clinical malignancy because its troublesome, harzardous and difficult surgical excision with high surgical mortality, morbidity and also high recurrency, especially in cases of giant sized tumor which diameter was over the 3 cm. The authors present a technical note of a giant craniopharyngioma which was mixed tumor of huge sized cystic portion and calcified solid portion in 10 years old female patient, and its diameter was 9 cm. The surgical technique of complete radical excision of tumor was staged combined method as initial percutaneous gradual drainage of cystic fluid by EVD system which was placed by stereotactic method and followed by successful radical extirpation of remaining cyst wall and solid mass by pterional-trans-sylvian microsurgical approach. There was no complication during and post-operatively.

6.Middle Cerebral Artery Aneurysms Associated with Intracerebral Hematoma:A Surgico-Clinical Study of 21 Cases.

Surgico-clinical study was done in the cases of 21 middle cerebral artery aneurysms associated in intracerebral hematoma. Compare with other aneurysms, hematoma associated middle cerebral aneurysms have more higher mortality and morbidity because of the profound initial bleeding effect and also combining brain edema and brain shift due to hematomal space occupying. Relative early surgery which was one stage operation as removal of hematoma and followed by direct clipping of aneurysm was recommended. Surgical mortality was 38% and morbidity was 14.3%, and the post-operative morbidity cases were vegetative state in all.

7.Interrlationship between Left Ventricular Mass and Diurnal Variations of Blood Pressure in Patients with Esssntial Hypertension.

BACKGROUND: In hypertensive patients, the left ventricular hypertrophy(LVH) is very important as an independent risk factor along with developing complications. The present study was attempted to assess whether LVE assessed by echocardiography is related to diurnal variations of blood pressure in patiens with essential hypertension. METHOD: After 24hr ambulatory blood pressure monitoring, echocardiographic parameters were investigated in 30 healthy normotensive subjects and 17 patients with diurnal variation of blood pressure and 19 patients without diurnal variation respectively. RESULTS: Left ventricular mass index was higher in essential hypertensive patients than normotensive subjects. In patients without nocturnal fall in systolic blood pressure, left ventricular mass tended to be higher than in patients with a nocturnal fall without statistic significance. In the hypertensive patients with nocturnal fall, there was a correlationship between LVMI and changes in systolic blood pressure, but no correlation between left ventricular mass index and changes in diastolic blood pressure. In the hypertensive patients without nocturnal fall, changes of both systolic and diastolic pressure did not affect LVMI. CONCLUSION: It is suggested strongly that left ventricular hypertrophy may occur highly in the hypertensive patients without nocturnal(diurnal) variation in blood pressure and may be associated with changes in diastolic and systolic blood pressure. But in hypertensive patient with nocurnal fall, left ventricular hypertrophy may be associated with changes in systolic blood pressure.

8.A case of colonic adhesion caused by appendiceal origin pseudomyxoma peritonei.

Mucocele of the appendix is an uncommon disorder, usually found incidentally during ultrasonography or radiographic studies. It occurs predominantly in the sixth or seventh decades,and has a distinct female predominance. Pseudomyxoma peritonei is a rare condition resulting from a ruptured mucin-producing lesion of the appendix or ovary. It is not easily diagnosed clinically before operation and only histopathologic finding provides the correct final diagnosis. Treatment should consist of evacuation of the mucinous ascites and removal of the mucocele. Reoperations for correction of intestinal obstruction may be needed. The role of chemotherapy is uncertain, and external radiation is probably of no value. We experienced a case of colonic adhesion caused by pseudomyxoma peritonei that originated from the vermiform appendix. This patient was 75 year old female who had suffered from lower abdominal pain associated with constipation and tenesmus for 5 days. We performed ileocecal resection and ileocecal anastomosis. We report this case with brief review of the literature.

9.The effect of progressive tensional force on mRNA expression of osteoprotegerin and receptor activator of nuclear factor kappa B ligand in the human periodontal ligament cell.

Tooth movement is a result of mutual physiologic responses between the periodontal ligament and alveolar bone stimulated by mechanical strain. The PDL cell and osteoblast are known to have an influence on bone formation by controlling collagen synthesis and alkaline phosphatase activation. Moreover, recent studies have shown that the PDL cell and osteoblast release osteoprotegerin (OPG) and the receptor activator of nuclear factor kappa B ligand (RANKL) to control the level of osteoclast differentiation and activation which in turn influences bone resorption. In this study, progressively increased, continuous tensional force was applied to PDL cells. The objective was to find out which kind of biochemical reactions occur after tensional force application and to illuminate the alveolar bone resorption and apposition mechanism. Continuous and progressively increased tensile force was applied to PDL cells cultured on a petriperm dish with a flexible membrane. The amount of PGE2 and ALP synthesis were measured after 1, 3, 6 and 12 hours of force application. Secondly, RT-PCR analysis was carried out for OPG and RANKL which control osteoclast differentiation and MMP-1, -8, -9, -13 and TIMP-1 which regulate the resolution of collagen and resorption of the osteoid layer. According to the results, we concluded that progressively increased, continuous force application to human PDL cells reduces PGE2 synthesis, and increases OPG mRNA expression.

10.Clinico-pathologic Study on Multiple Squamous Epithelial Neoplasia of the Esophagus.

PURPOSE: Multicentric occurrence of esophageal squamous neoplasm has been known to be significant in terms of its pathogenesis and production of detectable early lesions. This study was performed to establish the incidence, pattem and clinico-pathologic features of multiple squamous epithelial neoplasia of the esophagus in Korea. MATERIALS AND METHODS: Forty-two consecutive cases of esophageal squamous cell carcinoma surgically treated at Korea Cancer Center Hospital in 1991 were studied. For pathological analysis, whole esophagectomy specimens were sectioned, micrascopically examined, and reconstructed. Age, sex, alcohol and smoking history, tumor location, stage, lymph node metastasis and survival were compared among different neoplastic conditions. RESULT: Fifteen cases (35.7%) showed multiple squamous lesions, 6 (14.3%) with multiple carcinomas and dysplasias, and 9 (21.4%) with single carcinomas with separate dysplasias. Intraepithelial lesions contiguous to main tumors were commonly observed (61.9%). Lugol`s solution staining pattern was compatible with epithelial pathology. Lymph node metastasis rate was significantly higher in cases with multiple carcinomas. Smoking history was significantly more common in patients with solitary catcinomas only. CONCLUSION: The multicentric occurrence of squamous epithelial dysplasia and carcinoma in the esophagus was confirmed in Korean patients, supporting the concept of field carcinogenesis at this region. However, lack of evidence for strong environmental influence in the patients with multiple lesions suggests yet another risk factor.
