2.The study of pathogenesis of palmoplantar keratoderma.

We investigated the biochemiesl change of keratin by the methods of SDS-PAGE and Two-dimensional gel electrophoresis, and observed electron microscopic ultrestructural changes in five Unna-Thost palmoplantar keratoderma patients and two normal adults. The results are summarized as follows : 1. The increased bands of 51 kd and newly appearing 48 kd, 56 kd keratins were observed on the SDS-PAGE and compared to the normsl control. 2. The newly appearing 48 kd(acidic) paired with 56 kd(basic) keratins and 51 kd keratin and the disappearance of 59 kd(basic), 64 kd(basic) keratins were observed on the two-dimensional gel electrophoresis and compared to the normal control. 3. The variable sized, numerous, globular, irregularly beam-shaped and granular kerstohyaline granules were scattered in the granular cell and corneocyte. Numerous ribosomes were noted between the clumped tonofibrils and around the keratohyaline granules. The lipid droplets were seen in the corneocytes and granular cells on the electron microscope.

6.Overlap Syndrome: A case showing severe CNS involvement.

We describe a patient with overlap syndrome in whom systemic lupus erythematosus and scleroderma were combined. The patient expired due to severe central nervous system involvernent and pneumonia. Pertinent neuropsychiatric signs of the patient included generalied seizure, impairment of recent, memory, dysarthria, mental deterioration, Balints syndrome, right sided hemiparesis and right sided central type facial weakness. Hrain MRI and cerebral angiogram slowed cerebral vasculopathy. We suggest that a long-term follow-up is necessary for the confirmation of ciagnosis of a connective tissue disease, because the evolution, transition and overlapping features among the group of connective tissue diseases may commonly occur over time.

7.Skin Graft and Chronic Osteomyelitis

Authors presented the gratifying result of treatment of chronic osteomyelitis with overlying skin defect and/or poor skin condition, which developed the wide skin necrosis after saucerization and resulted the second skin graft necessary. Five cases were treated by primary extensive saucerization of tibia which left a cavity with bleeding walls. The exposed bony surface with bleeding walls was covered by delayed free skin graft when the good granulation covered the bony surface. Through this experience, delayed free skin graft is justified the best procedure in these cases to cover the post-surgical raw bone surface and to have primary healing of the bone infection. A method of hemicylindrical pedunculated skin graft to obliterate the deeply excavated surgical bony defect is introduced.

8.A Clinical Analysis of 7 Patients with Neuro - Behcet's Syndrome.

Neurologic studies were done on 7 neuro-Behcet's syndrome patients, admitted from June 1985 to June 1986. They were from 30 to 43 years of age with mean age nf 35. 3;3 were male and 4 female. Symptoms fitting more than 3 of the 4 major diagnostic criteria included in Shimizu's classification preceded the neurological symptoms in all 7 patients and they progressed to neuro-Behcet syndrome approximately 5 years after the appearance of the first major syrnptoms of Behcet's syndrome, Severe headache was seen in all 7 patients. Ataxia, dysarthria, increased DTR, and herniplegia. were other rnajor signs found and psychiatric symptoms were present in 2 cases. The cases were neurologically classified as brain stem syndrome in 3 male patients, meningo-encephalitic syndrome in 1, and mixed type in 3. Characterletic laboratory findinge were lncreaeed mononuclear celle in CSF, peripheial blood leukocytosis, and decreased lymphocyte blastogenesis.

9.Spontaneous Testicular Hemorrhagic Necrosis Masquerading as a Testis Tumor .

Spontaneous testicular hemorrhagic necrosis is a rare disease usually associated with testicular torsion. Partially involved and suspicious testis tumor cases are also defined from orchiectomy specimens. Herein, a spontaneous hemorrhagic necrosis, without any testicular torsion, but with involvement of the whole testicle and epididymis, is reported. A 21 year old patient, who presented with a painless left testicular enlargement of several days duration was believed, based on physical examination, ultrasonography and elevation of serum LDH, to have a testicular tumor. Diagnosis was made only after radical orchiectomy and histopathological examination.

10.Evaluation of Right Ventricular Function with Quantitative Radionuclide Ventriculography in Chronic Obstructive Lung Diseases.

A reproducible noninvasive technique for measuring right ventricular ejection fraction (RVEF) was developed using first pass quantitative radionuclide angiocardiography. Tests were carried out in the right anterior oblique position with a computerized multicrystal scintillation camera with high count rate capabilities. RVEF was calculated on beat to beat basis from the high frequency components of the background-corrected right ventricular time-activity curve. The following results were obtained; 1) In 10 normal adults, RVEF averaged 50.9+/-8.2. In 20 patients with chronic obstructive lung diseases (COPD), RVEF was 37.9+/-6.1% and significantly lower than that of normal persons(P<0.005). 2) There was meaningful correlation between RVEF and forced expiratory volume (FEVI) in patients with COPD (r=0.51). And there was significant difference of RVEF between 13 patients with FEVI less than IL/min (6 patients with right ventricular failure, 7 patients with patients without that) and 7 patients with FEVI IL/min or more (35.7+/-6.0%, 42.1+/-3.2% respectively. P<0.005). 3) All 6 patients with clinical manifestation of right heart failure had abnormal RVEF and had FEVI less than IL/min, which was significantly lower than that of 14 patients without clinical manifestation of right heart failure (33.6+/-4.8%, 39.8+/-5.6%, respectively. P<0.005). 4) In 14 patients without clinical manifestation of right heart failure, 11 patients among whom 7 patients had FEVI less than IL/min, had abnormal RVEF.
