Fiddler's neck was first described by Peachey and Matthews in 1978 to denote a condition characterized by focal lichenification and pigmentation on the left side of violinist and violist neck just below the angle of the jaw. The etiologic facters are friction, local pressure, occlusion, poor hygiene and sweating. In only a few cases has a specific hypersensitivity to one or more of the ch!n rest material(wood, lacquer, dye) or the metallic fixing been found responsible. We report a case of 15-year-old female patient with Fiddler's neck who has played the violin for 8 years.

Korean Journal of Obstetrics and Gynecology 2000;43(6):987-991
OBJECTIVES: The most often perfomed major operation in gynecology is hysterectomy. Our purpose was to compare the indications, charateristics and outcomes of patients undergoing total abdominal hysterectomy and total vaginal hysterectomy and to help to establish guidelines to determine the route of hysterectomy. METHOD: The hospital charts of 400 women who underwent elective inpatient hysterectomy at Kyung-Hee University hospital from January 1994 to January 1999, were abstracted retrospectively. Data were collected regarding patients, age, parity, preoperative indications, the route of hysterectomy, uterine weight, operative and postoperative complications and the length of stay. The operative indications were benign uterine disease except from uterine prolapse. Bisection or combined morcellation were used in most cases to obtain reduction in uterine size. RESULT: Patients in whom the vaginal route was successful included 18% of those with uterine weights exceeding 280gm. There was statistically significant difference for uterine weight, operative time, bleeding amount, the length of stay in two camparative group. 4% of vaginal hysterectomy and 7% of total abdominal hysterectomy has documented operative complications. CONCLUSIONS: Vaginal hysterectomy is safe operation with few intraoperative and postoperative complications without notable blood loss. Vaginal hysterectomy allow one to shorten the operating time and allows early postoperative discharge of some patients from hospital. Skilled performance of vaginal hysterectomy is worth greater attention and should be used more often in gynecological study.

Korean Journal of Dermatology 1995;33(1):8-18
BACKGROUND: Chronic exposure to ultraviolet radiation(UVE) nduces photoaging characterized by dry, deeply wrinkled, inelastic, leathery, and irregulaity pigmented skin. UVR also induces solar keratosis and carcinoma, and is a contributing factor in melanoma. Sunscreens are used to prevent solar damage to skin and, if used on a daily lass should significantly reduce the incidence of the chronie photodamaging events. OBJECTIVE: We tried to evaulate the photoaging effects of UVR in the skin and the photoprotective effect of sunscreens. METHOD: We examined the gross and microscopic changes skin of albino hairless Skh : HR-1 mice exposed chronically to ultraviolet B(UVB) and suncreen-treated mice. RESULTS: The skin of the UVB-irradiated mouse shows chear, cteristic signs of photoaging, such as deep wrinkles across the back, and thickened and a hery skin. Histologically, the photoaged skin shows increased epidermal thickness, numeross fibroblasts and inflammatory cell infiltration in the upper dermis, and many enlarged keratering cysts in the lower dermis. By the 20th week, seven of the total of 9 mice(78%) in UVB irradiated mice developed at least one tumor. Histologically, the tumor is a papilloma, but the he are many dyskeratotic cells and loss of polarity in epidermis. Octyl methoxycinnamate or TiO ZnO Talc-treated mic show a significantly decreased wrinkling score, mimmal epidermal hyperplasia, slightly increased dermal cellularity, and lack of proliferation of cysts. The octyl dimethyl PABA-treatd mice shows significantly increased wrinkling score and marked inflammatory infiltration dermis. By the 20th week, only one mouse had developed a tumor in the octyl methoxy irmamate-treated group and no evidence of tumor was seen in the TiO ZnO Talc-treated group. In the octyl dimethyl PABA-treated group, five of 7 mice(71%) developed at least one tumor. CONCLUSION: The skin which is chronically exposed to UVB is subject to photoaging and photocarcinogenesis and regular use of an adequate sunscreen would prevent these photodamaging effects of UVB.

OBJECTIVE: We analyzed the clinical features of amniotic fluid embolism confirmed by autopsy or clinical dicision for making a plan of rapid diagnosis and intensive emergency care. STUDY DESIGN: We experienced 2 clincal cases in Ewha Mokdong Hospital and Severance Hospital at 1995, and reviewed the medical record & autopsy report of 15 cases in National Institute of Scientific Investigation from Jan, 1991 to Dec, 1995. RESULTS: The mean maternal age was 31+/-3 years. The mean gestational age was 40+/-1 weeks. The number of delivery were 1.4+/-0.8. The number of abortion history. were 2.6+/-2. The sexuality of fetus were six males (35 %), five females (29%), and unknown six cases (35 %) and the fetal mortality rate was 29 % (5 cases). The initial clinical symptoms and signs were hypotension (12 patients, 71%), vaginal bleeding(ll patients, 65%), cardiac arrest (6 patients, 35 %), dyspnea (5 patients, 29 %), and seizure (2 patients, 12%). In the case of normal spontaneous vaginal delivary (NSVD), the symptoms occurred during induction in 3 (18%), during delivery in 1 (6%), and after delivery in 9 (53%). In the case of Cesarean section, the symptoms occurred during section in 2 (12%), and after section in 1 (6%). There are one case whose symptoms occurred during 3rd gestational period. CONCLUSIONS: We should alert for the amniotic fluid embolism in the clinical findings of acute collapse and vaginal bleeding, respiratory symptoms, and seizure at any peripartum. With eary suspicion of this disease and aggressive intensive care we can reduce amniotic fluid embolism mortality rate and it's legal problem.

7.A Case of Dandy-Walker Variant Associated with Infantile Esotropia of Abnormaly Directed Extraocular Muscle.

Journal of the Korean Ophthalmological Society 1999;40(2):620-623
The Dandy-Wlaker syndrome is a developmental disoeder characterized by cystic dilatation of the fourth ventricle and agenesis or hypoplasia of the cerebellar vermis and is usually associated with hydrocephalus. This syndrome is also associated with intracranial malformations such as agenesis of corpus callosum, and extracranial malformations such as facial anormalies (cleft palate, cleft lip, and ocular anomalies), polycystic kidney, and cardiac anormalies. Reported associated ophthalmic anomalies are microphthalmos, coloboma, and congenital cataract. We experienced a case of infantile esotropia in abnormal insertion and direction of lateral rectus muscle as a variant of the Dandy-Walker syndrome accompanied by cleft palate and cleft lip in a 19-month-old male infant. Subsequent treatment of the 30delta esodeviation and left inferior oblique muscle overaction, including resection of the lateral rectus muscle in both eyes and the myectomy of the inferior oblique muscle in the left eye, yielded satisfactory results, so we report this case, with a review of the pertinent literature, as the first case of Dandy-Walker syndrome associated with esotropia with abnormal course of lateral rectus muscle in Korea.
