1.Synovial Sarcoma of the Thumb: A Case Report

Synovial sarcoma is comparatively uncommon and highly malignant tumor, which usually arises in proximity to a joint and may affect the adjacent bones. Occurence in finger and severe bony involvement are rare. We present a case of synovial sarcoma of the thumb, because the tumor arose in an unusual site and was characterized by severe bony destruction.

2.Effects of Transforming Growth Factor-beta on Proliferation, Collagen synthesis, Migration and Metalloproteinase Secretion of Human Retinal Pigment Epithelial Cells.

PURPOSE: This study was performed to evaluate the effect of TGF-beta on proliferation, collagen synthesis, migration, and matrix metalloproteinase (MMP) secretion of human RPE cells in vitro. METHODS: The cultured human RPE cells were treated with either TGF-beta1 or TGF-beta2 in concentrations of 0, 0.1, 1, 10 ng/ml respectively. The cell number was measured in 3, 6, 9 days, and the collagen synthesis and cell migration was measured. [3H]-thimidine uptake assay was done to evaluate the change of DNA synthesis. And the secretions of MMP1, MMP2, MMP3, MMP9, TIMP1 (tissue inhibitor of metalloproteinase), and TIMP2 were measured by electrophoresis and Western blot analysis. RESULTS: TGF-beta1 and TGF-beta2 significantly inhibited the proliferation of RPE cells in a concentration -and time-dependent manner (p<0.05). [3H]-thymidine uptake was decreased by TGF-beta1 and TGF-beta2 in a concentration-dependant manner. The collagen synthesis of RPE cells was significantly increased by high concentration of TGF-beta1 and TGF-beta 2. However, the migration of RPE cells was not affected by TGF-beta. As the concentration of TGF-beta1 and TGF-beta2 increased, the secretions of MMP1, MMP2, MMP3 and MMP9 decreased, while the secretion of TIMP1 and TIMP2 increased after 72 hours. CONCLUSIONS: These results suggest that the TGF-beta1 and TGF-beta2 may have critical effect on the development of PVR and provide clues to possible therapeutic solutions for controlling PVR process.

4.Weight in children's minds: body shape dissatisfactions for 12-year old children.

BACKGROUND: Diet and weight concerns are commonplace and almost accepted features of life for girls during adolescence. Until recently, younger age groups haue largely been ignored, as these concerns were thought to be a product of pubertal development and sexual maturity. Girls under the age of fifteen therefore, have been assumed to be free of the pressures experienced by adult women. However, this belief is now hard to sustain. The object of the present study was to investigate the self-perception and body shape satisfaction in different weight categories of boys and girls aged 12-years old in Korea. METHODS: In May, 1995, one hundred and sixty seven boys and one hundred and twenty girls from two schools in Seoul completed assessments of body-esteem, self-esteem, body shape preference. The children's body weight and height were also measured. RESULTS: The heaviest children expressed the most discontent, having a low body-esteem, a desire for thinness. By the age of 12, girls boys already differ in their body shape satisfaction and differ in their body shape aspirations. There was a significant effect of weight category on the children's body esteem(boys(P =.005), girls(P=.0001). Children in both extreme categories, under-and over-weight, had lower body-esteem scores than those in other weight categories. However, it was the overweight children who had the lowest. reported body-esteem. Body esteem was highest among girls in the 'slightly underweight' category and highest among boys on the 'average weight' category. There was no effects of either weight category or gender on the childrens appraisal of self-esteem. A comparison of the points chosen on the silhouette scales to reflect current and preferred body shapes revealed clear gender differences. Of the girls, 63% placed their preferred body shape at a point thinner than their currently perceived shape, while only 15% chose a broader figure. In contrast,, 41% of the boys rated their preferred figure as broader than their current perception, and 37% as thinner than their current perception. CONCLUSIONS: This study has noted a relationship between body weight and self-perception in 12-year old children. The heaviest children expressed low body-esteem, a desire for thinness. This pattern was more characteristic of girls than boys. Even at this age, well before they have completed physical maturation, girls are aspiring to a body shape which is thinner than their average. This discontent experienced by the heaviest children on this sample was apparent in their lower body-esteem and the distance between their current and preferred body shapes. The girls preference was for thinness, while in the boys it was for a body shape which was broader than their current shape. From their responses, it would appear that the boys' desire was not for fatness, but for a more athletic and muscular build.

5.Brain Neurologic Disorder and Depression in Elderly.

To help understanding of depression in elderly with stroke, Alzheimer's dementia and parkinson's disease, the authors reviewed the articles and books in this area. Depression is a very popular symptom of elderly patients but the neurologic conditions make it difficult to differentiate and find out depression from neurodegenerative deficits symptoms in elderly who have neurological disease. Even though various research obstacles in this area, depression in elderly with neurologic diseases (stroke, Alzheimer's dementia, and Parkinson's disease) could be a gateway of understanding of pathophysiology of depressive disorder perse. In this article, epidemiological study, pathophysiological study, symptomatology and treatment consideration were intensively reviewed and summarized. The more constructed and systematized studies are needed to understand the depression in elderly with stroke, Alzheimer's dementia and Parkinson's disease.

6.Clinical Observation on the Renal Tuberculosis.

Renal tuberculosis has been one of the most frequent disease in the urological field of Korea for a long period of time. A clinical observation of renal tuberculosis was made on 71 patients admitted to the Department of Urology, Han-Il Hospital from January 1972 to December 1981. The results were summerized as follows: l. These 71 patients (79 kidneys)stood for 6.7% of the total inpatients. 2. Positive urine AFB were demonstrated in 41 (57.7%)of 71 patients on direct smear and 9 (42.9%)of 21 patients on culture. 3. The excretory urograms of 75 tuberculous kidneys (67 patients)revealed absence of visualization in 28.0% and delayed visualization in 24.0%. The classification based on Lattimer (1953)with 67 IVPs and 30 RGPs was 6.7% in Group 1, 41.3% in Group 2, 22.7% in Group 3, 29.3% in Group 4 and absent in Group 0. 4. Nephrectomy was performed in 30.4% (24 patients)of 71 patients. 5. One of 3 hypertensive patients was significantly decreased blood pressure after nephrectomy.

7.Epidermal Grafting after Removal of Recipient Epidermis by CO2 Laser Ablation in Vitiligo.

BACKGROUND: Epidermal grafting using cryotherapy for recipient sites is in widespread use. However the peripheral hypopigmented haloes that occur around the recipient sites require prolon gation of the treatment period. OBJECTIVE: We used a CO2 laser to remove the epidermis of the recipient sites for betteri results. METHODS: We treated lie localized vitiligo patients with CO2 laser to remove t.he epidermis and grafted suction blister rooves. We observed repigmentation and complications 1 month later. RESULTS & CONCLUSIONS : The superiority of this method is demonstrated by the fact 1) all prodedures can be completed on the day of operation 2) the incidence of hypertrophic scar and peripheral hypopigmented halos can be observed.The problems of this method are 1) uneven repigmentation of recipient. sites 2) hyperpigrnentation of recipient sites

8.Partial Unconstitutional Clauses in the Korean Public Sanitation Administration Law.

In 2007, the Public Sanitation Administration Law in Korea was partially amended to subdivide cosmetology into general and skin cosmetology. Thus starting from 2008, a skin cosmetology qualification requirement license was newly created in Korea. Instead of improving the health standards concerning public health, the amendments have exaggerated the monopolistic right of trade of the Korean Central Esthetician's Association with the assistance of the Division of Sanitary Policy in the Korean Ministry for Health. In addition, the amended ordinances stipulate that cosmetologists can analyze skin conditions, hence allowing medical diagnostic procedures by non-medical personnel which has serious health problems causing serious conflict with Korean Medical Law. Legislation precedents from around the world demonstrate that medical supervision by physicians is absolutely necessary with fastidious requisitions in order to safeguard public health. However, in Korea there is a subversive attempt by some esthetician groups to limit the employment of estheticians by medical institutions in order to safeguard their private privileges. The newly amended Public Sanitation Administration Law should be revised in order to preserve the fundamental rights of the people, to enhance public health, and to uphold the well being of the people instead of catering to special interest groups.

10.Limb Salvage Surgery with Tumor Prosthesis for the Malignant Bone Tumors Involving the Proximal Femur.

PURPOSE: As well as patient survival, the restoration of postoperative function such as ambulation is important in limb salvage operations for treatment of malignant bone tumors involving the proximal femur. The authors analyzed clinical outcomes of limb salvage operations using tumor prostheses for metastatic or primary malignant bone tumors in the proximal femur. MATERIALS AND METHODS: From February 2005 to January 2014, 20 cases (19 patients) with malignant bone tumor involving the proximal femur with pain or complicated pathologic fracture were treated with segmental resection and limb salvage operations with tumor prostheses. Mean age was 63.1 years (range 35-86). Fourteen patients were male and six ones were female. The mean follow-up period was 20 months (1-94 months). There were 15 cases of metastatic bone tumor, 4 cases of osteosarcoma, and 1 case of multiple myeloma. The primary tumors of the metastatic bone tumors included 4 lung cancers, 3 hepatocellular carcinomas, and 3 renal cell carcinomas. Other primary tumors were breast cancer, thyroid cancer, colon cancer, prostate cancer, and malignant spindle cell tumor, each in 1 case. Modular tumor prostheses were used in all cases; (Kotz's(R) Modular Tumor prosthesis (Howmedica, Rutherford, New Jersey) in 3 cases, MUTARS(R) proximal femur system (Implantcast, Munster, Germany) in 17 cases). Perioperative pain was assessed with Visual Analogue Scales (VAS). Postoperative functional outcome was assessed with Musculoskeletal Tumor Society (MSTS) grading system. RESULTS: Out of 20 cases (19 patients), 11 cases (10 patients) survived at the last follow-up. Average postoperative survival of the 9 deceased patients was 10.1 months (1-38 months). VAS score improved from pre-operative average of 8.40 (5-10) to 1.35 (0-3) after operation. Average postoperative MSTS function score was 19.65 (65.50%, 7-28). The associated complications were 2 local recurrences, 3 hematomas, 3 infections, 2 scrotal swellings, and 1 dislocation. There was no case of periprosthetic fracture or loosening. CONCLUSION: Limb salvage operation with tumor prosthesis is an appropriate treatment for early pain reduction and functional restoration in malignant bone tumors in the proximal femur with pain an/or complicated pathologic fractures.
