70 cases free fiap were performed from March 1978 to July 1985. The donor fiap were dorsalis pedis flap in 34 cases, groin flap in 8 cases, osteocutaeous flap in 3 cases, gracilis flap in 17 cases, and latissimus dorsi flap in 8 cases respectively. The success rate was 90% in total including partial success. The causes of failure were 2 cases infection necrosis, and 1 case kinking of drain vessel. The gracilis and latissimus dorsi myocutaneous flap were very satisfactory method for the reconstruction of the upper extremity flexion power, especially in the case of Volkmann's ischemic contracture. The neurovascular dorsalis pedis flap was regarded as excellent method for the heel pad reconstruction, and tactile sensation reconstruction of the hand.

The acute anterior cruciate ligament injury is one of the most common ligament injury of the knee joint and there are many controversies in its treatment. Warren reported that the primary repair for the acute anterior cruciate ligament injuries was good, but some had opposite oppinions. Authors studied 23 patients of actue anterior cruciate ligament injury who were treated by primary repair from January, 1982 to December, 1984. The results were as followings: 1. The most common associated injury was MCL injury and posteromedial capsular injury(13 cases; 56.5%). 2. The most common detachment site of ACL was femoral attachment(11 cases; 47.8%) . 3. A medial Hocky Stick incision was found good in repairing the combined MCL and posteromedial capsular tear with ACL injury. 4. The results were good in 74% by Cincinnati score and in 87% by objective score. It is concluded that primary repair of the acute anterior cruciate ligament tear is cosidered successful.

PURPOSE: Reported incidence of urinary incontinence after a radical prostatectomy (RP) varies between studies. This may be due not only to the definition of incontinence applied, but also how the information is acquired. We investigated the differences in perception of post robot-assisted laparoscopic RP (RALP) urinary incontinence acquired through doctor interviews and patient-reported questionnaires. MATERIALS AND METHODS: Of 238 consecutive men who underwent RALP by a single surgeon between July 2005 and February 2008, we evaluated 66 men using the International Consultation on Incontinence Questionnaire (ICIQ) at various time points after surgery. Each patient's ICIQ results were considered to be the patient's perceptions of urinary incontinence. The physician at the same time directly interviewed the patients about the number of pads used and considered complete continence to be equivalent to the use of no pads or safety liners. RESULTS: Of the 66 patients, the physician reported that 34 (51.5%) had obtained complete continence. However, analysis of the questionnaires of these 34 patients revealed that only 5 (14.7%) patients reported that they never leaked during the past 4 weeks. Most patients (11 patients, 32.4%) who did not use any pad did in fact reported leakage of a small or moderate amount of urine about once a day. CONCLUSION: Our results indicate that there are discrepancies in the perception of urinary incontinence between doctor and patient after RALP. Non-use of pads is not equivalent to obtaining complete urinary continence. Therefore, the number of pads used is not a good measure to determine the status of complete urinary continence.

PURPOSE: To evaluate the relationship between a limb-length discrepancy and the clinical and radiological outcomes after bipolar hemiarthroplasty for femur neck fractures in elderly patients. MATERIALS AND METHODS: Between August 2004 and January 2007, 60 out of 80 cases over 65 years who underwent cemented bipolar hemiarthroplasty for femur neck fractures with a more than 2 years follow-up were evaluated retrospectively by dividing the patients into two groups, > 5 mm (group 1) and < 5 mm (group 2) of limblengthening. Harris hip score, the activity of daily living(ADL), dislocation rate, pain by a visual analogue scale (VAS) and acetabular wear by radiographic measurements were analyzed. RESULTS: For each group, the Harris hip score in group 1 and 2 was 80.0 and 76.2, respectively. The ADL in group 1 and 2 was respectively, 36 and 29 preoperatively, 56 and 52 postoperatively. The visual analogue scale in group 1 and 2 was 3.53 and 2.23 (P=0.002), respectively, and acetabular wear was encountered in 6 and 8 cases in group 1 and 2, respectively. The VAS score was significant. There was no difference in acetabular wear and dislocation between the two groups (P>0.05). CONCLUSION: Limb-lengthening by hemiarthroplasty may be a cause of pain but with little effect on the overall functional outcome.

OBJECTIVE: To evaluate the effect of donepezil on cognitive function in patients with brain injury who had cognitive impairment. METHOD: Twenty one subjects with brain injury (18 males,3 females) were enrolled and classified into two groups, experimental and control group. There was no significant difference between two groups in age, postonset duration, and level of education. For the experimental group, one tablelet of Aricept(R) (5 mg of donepezil per tablet) was administered daily for 6 weeks. The baseline and follow up cognitive assessments were performed before and 6 weeks after. Mini- mental Status Examination (MMSE), Computerized Neuropsychologic Test (CNT), Judgement of Line Orientation (JLO), and visuospatial Wechsler memory test were used for the assessment of cognitive function. RESULTS: In baseline study, two groups showed no difference in their cognitive function. After 6 weeks, the experimental group showed significantly higher performance in forward digit span, verbal learning test, backward visual span, visual learning test, non-verbal Wechsler memory test, and judgement of line orientaion than control group (p<0.05). CONCLUSION: These results suggested that the administration of Aricept(R) was beneficial in improving the cognitive func tion, especially verbal and visual memory and visuospatial perception in patients with brain injury.

Discrete subvalvular aortic stenosis is a relatively uncommon cause of the left ventricular outflow obstruction, requiring early intervention. Conventional transthoracic echocardiography may fail in some patients due to insufficient imaging quality. In particular, in patients with a discrete fibrous membrane close to the aortic valve without narrowing of the left ventricular outflow tract, the echocardiographic detection of the membrane may be difficult. Transesophageal echocardiography allows a clear visualization of the aortic valve and the left ventricular outflow tract in virtually all patients, it can be performed rapidly with almost no risk, and it may therfore be helpful in establishing the diagnosis of discrete subaortic stenosis, in particular in patients where the conventional transthoracic approach fails. We have experienced two cases of discrete subaortic stenosis. One case of them was combined with hypertrophic obstructive cardiomyopathy in this report we discussed the utility of multiplane transesophageal echocardiography in patients with discrete subvalvular aortic stenosis.
