3.A Case of Proliferating epidermoid cyst.

Proliferating cutaneous epithelial cyst is classified into two subtypes, proliferating trichilemmal cyst and proliferating epidermoid cyst, depending on the mode of keratinization or the origin of the tumors. Proliferating epidermoid cyst is a subepidermal cystic tumor showing, at least in focal areas, a cystic wall lined by epithelium typically seen in epidermoid cyst. We report a case of proliferating epidermoid cyst in a 58-year old man who had a 3x4 cm sized egg shaped cystic tumor on the occipital area of the scalp. Histopathologic examination revealed the tumor was a large epidermoid cyst and a portion of epithelial proliferation projected from the cyst wall into the dermis. The epithelial proliferation was surrounded by paucicellular fibrous stroma.

4.Two Cases of Giant Cell Deficient Giant Cell Tumor of Tendon Sheath.

Giant cell tumor of tendon sheath is the second most common tumor involving the hands. It usually presents as a slowly growing, lobulated, painless subcutaneous mass. Histologically, the tumor is composed, in variable proportion, of histiocytoid mononuclear cells, multinucleated giant cells, xanthoma cells, and collagenized stroma. Although numerous characteristic multinucleated giant cells are usually noticeable in the tumor, they may be difficult to find, or be entirely absent. We present two cases of giant cell tumor of tendon sheath in which giant cells was not and rarely found respectively.

5.The Effects of Vascular Endothelial Growth Factor on Capsule Formation around Silicone Implant in Rats.

BACKGROUND: The purpose of this study was to examine the effects of a single administration of vascular endothelial growth factor (VEGF) in promoting the angiogenesis and thereby reducing the formation of capsular contracture. METHODS: We treated 24 female Sprague-Dawley rats with (1) 5 mM Tris Buffer and 150 mM NaCl 0.1 cc, (2) VEGF 15 µg/0.1 cc, (3) VEGF 150 µg/0.1 cc during placement of the implant, or (4) VEGF 150 µg/0.1 cc and VEGF 300 µg/0.2 cc. We histopathologically measured the thickness of the capsule and the number of blood vessels. RESULTS: All experimental groups had a significant difference in the thickness of the capsule compared to the control group (P<0.001). There was no significant difference between experimental group 2 and experimental group 3. The number of blood vessels formed around the capsule was significantly greater in all the experimental groups compared with the control group (P<0.05). There was no significant difference between the experimental groups. There was a significant negative correlation between the thickness of the capsule and the number of blood vessels (Spearman's correlation coefficient, 0.2732; P<0.0001). CONCLUSIONS: A single administration of VEGF reduced formation of the capsule and increased the vascularity around the implant, supporting the hypothesis that prevention of tissue ischemia can be a treatment strategy for capsular contracture.

6.A Case of Dermatofibroma associated with Follicular Basal Cell Hyperplasia.

Various epidermal changes may be present in the overlying epidermis of dermatofibroma. Follicular basal cell hyperplasia is a relatively rare epidermal change found in 2-23% of dermatofibroma and it should be differentiated from basal cell carcinoma. We report a case of dermatofibroma associated with focal basal cell hyperplasia and a review of the literature.

8.Vardenafil Improves Lower Urinary Tract Symptoms in Patients of Erectile Dysfunction.

PURPOSE: It is well known that male erectile dysfunction and lower urinary tract symptoms are common in aging men. We evaluated whether treatment for erectile dysfunction with vardenafil might improve lower urinary tract symptoms. MATERIALS AND METHODS: From January 2004 to May 2005, 34 patients with erectile dysfunction attending the outpatient clinic were treated with vardenafil and followed up 1, 3, and 6 months after the treatment. They completed the International Index of Erectile Function (IIEF) and the International Prostate Symptom Score (IPSS) questionnaire at baseline and at each follow-up (1, 3, 6 months). RESULTS: After treatment with vardenafil, IIEF and IPSS were significantly improved at 1 month compared to baseline,and the improvement was maintained during the 3 to 6 month follow-up period. There was no correlation between IPSS and IIEF score either before or after treatment. There was no statistical significance between the severity of IPSS and the response to vardenafil. There were mild side effects, including 2 patients with headache, 2 patients with facial flushing, and 1 patient with eye discomfort. CONCLUSIONS: Vardenafil appears to improve not only erectile function but also lower urinary tract symptoms in men with erectile dysfunction. However, the severity of IPSS does not appear to predict the response to vardenafil therapy.

9.Radiofrequency Catheter Ablation in Patients with Atrial Flutter.

BACKGROUND: Atrial flutter is a common arrhythmia for which no entirely satisfactory treatment is available. Despite the growing number of antiarrhythmic agents available for arrhythmia prophylaxis many patients are either intolerant of drug treatment or achieve inadequate relief from their symptoms. Recently, catheter ablation using radiofrequency energy has been used to result in high success rate for immediate prevention of atrial flutter but significant recurrence rate. We report our initial experience on radiofrequency cather ablation(RFCA) of atrial flutter in 8 patients. METHODS: The electrophysiologic approach guided by the earliest artial activation was used in the first patient and then anatomically guided approach in the remaining patients. The end point of RFCA was both demonstration of conduction block across the linear lesion at the atrial isthmus between the inferior vena cava and the tricuspid ring and noninducibility of atrial flutter with atrial burst pacing and extrastimulation up to 3 during isoproterenol infusion. RESULTS: Eight consecutive patients underwent RFCA.All were male and mean age was 53+/-22 years. Initial success was achieved in 7 patients(88%). During the follow-up period of 4+/-2.3 months, early sympomatic recurrence occurred in 2/7 patients(29%) within 1 month after initial success and the second ablation procedure was successfully performed in one patient. Overall success rate at the end of the follow-up period was 6/8(75%). there were no serious complications during and after the procedure. CONCLUSION: 1) radiofrequency catheter ablation is safe and highly effective treatment modality for prevention of atrial flutter. 2) Demonstration of conduction block across the lesion at the atrial isthmus should be achieved as an endpoint.
