Gangrenous skin disease is characterized by rapidly progressive necrosis of the subcutaneoua tissue. It is mainly caused by Group A b-hemolytic Streptococcus. We are now reporting three cases of gangrenous skin diseases including gangrene due to Vibrio vulnificus, streptococcal gangrene, and ecthyma gangrenosum. Their treatment was as follows: early surgical debridment, the correction of fluidelectrolyte depletion, and antibiotics. When fresh granulation tissues were seen on the wound base, we performed so called Island skin graft. 3 to 5 weeks later, there were good cosmetic and functional results. These results reveals that among several curative measures of gangrenous skin diseases early surgical debridment is the most important one. We belive many dermatologists should be concerned about skin surgery progressively.

BACKGROUND: The stratum corneum presents a significant barrier to transdermal drug delivery. Approaches to improve percutaneous absorption of drugs have included iontophoresis and skin penetration enhancers. Oleic acid has been studied as a skin penetration enhancer for drugs, primarily via its action mainly on the stratum corneum lipid structure. OBJECTIVE: The purpose of this study was to assess the interaction between oleic acid and stratum corneum lipids in vivo. METHODS: Male hairless mice were treated topically with oleic acid. Barrier function was assessed by transepidermal water loss measurement and ultrastructural observation with ruthenium tetroxide (RuO₄) staining. RESULTS: Oleic acid in propylene glycol had a profound effect on epidermal barrier function and was found to be concentration dependent. Moreover, ultrastructural examination with RuO4 post-fixation demonstrated that there were marked alterations in the stratum corneum lipid structure. CONCLUSION: This study provides direct evidence that oleic acid increases the epidermal permeability through a mechanism involving the stratum corneum lipid membrane perturbation via the lacunae formation within the stratum corneum.

Telangiectasia is characterized by permanently dilated small vessels usually arising from the suprapapillary plexus of venule capillaries, or arterioles. It may be etvlogically divided into the primary and secondary types. Rosacea, varicose vein, prolonged sun xvsure, radiation, and physical trauma may be the causes of secondary telangiectasia. We report herein a case of secondary telangiectasia associated which operation for a femur fracture.

A 61-year-old male was presented with oozing crusted erythematous skin lesions on the ex-tensor site of the right forearm, the extensor site of the left forearm, left calf, right shin, and the right portion of the buttock. He was diagnosed as having multiple pagetoid Bowen's disease by H&E staining, special staining and by an electron microscopic examination. He was treated with a topical application of 5-flurouracil cream and cryosurgery. The skin lesions were mostly healed after 7 months. We herein report a case of multiple pagetoid Bowen's disease on the sun-protected area that may be caused by arsenic ingestion.

Baker's cyst is found as most frequent cystic mass around the knee occurring between medial head of gastrocnemius muscle and semimembranosus muscle. The proximal or posterolateral extension of the cyst had been rarely reported and the cyst into the surrounding muscular tissue extremely rare. Intramuscular Baker's cyst that we report was found between lateral head of gastrocnemius muscle and popliteus tendon, and then extended into the plantaris muscle. With review of the literature, we here report a very rare case of plantaris muscular extension of Baker's cyst.

The taxonomy of the Alternaria spp. has been a subject, of controversy because of their high variability in conidial morphology and polymorphism displayed even in pure cultures. The published Korean literature on the genus Alternaria is scattered and fragmentary, and pertains to about 25 species with special emphasis on occurrence and pathogenicity, but mycological studies on this group of fungi are insufficient. This is the first series of detailed and consolidated account of Korean species of Alternaria, which includes 11 species; viz., Alternaria brassicae, A. brassicicola, A. cinerariae, A. dauci, A. dianthi, A. dianthicola, A. helianthi, A. helianthinffciens, A. iridicola, A. japonica and A. protenta. Detailed diagnostic descriptions, specific characterizations, taxonomic discussions and illustrations for each species are presented.

Neurologic studies were done on 7 neuro-Behcet's syndrome patients, admitted from June 1985 to June 1986. They were from 30 to 43 years of age with mean age nf 35. 3;3 were male and 4 female. Symptoms fitting more than 3 of the 4 major diagnostic criteria included in Shimizu's classification preceded the neurological symptoms in all 7 patients and they progressed to neuro-Behcet syndrome approximately 5 years after the appearance of the first major syrnptoms of Behcet's syndrome, Severe headache was seen in all 7 patients. Ataxia, dysarthria, increased DTR, and herniplegia. were other rnajor signs found and psychiatric symptoms were present in 2 cases. The cases were neurologically classified as brain stem syndrome in 3 male patients, meningo-encephalitic syndrome in 1, and mixed type in 3. Characterletic laboratory findinge were lncreaeed mononuclear celle in CSF, peripheial blood leukocytosis, and decreased lymphocyte blastogenesis.
