1.A Prospective Study of the Epidemiology of Out-of-Hospital Pediatric Cardiopulmonary Arrest.

PURPOSE: Data regarding out-of-hospital pediatric cardiopulmonary arrest have been limited to the retrospective study. This study was performed to analyze the epidemiology and outcome of out-of-hospital pediatric cardiopulmonary arrest(CPA) with the international consensus in a single hospital. METHODS: Children less than 15 years old who entered emergency department with CPA, between 1st March 2004 and 31st July 2007, were included this study. Data were recorded prospectively following Utstein's template. The characteristics and outcomes of patients were analyzed. RESULTS: Cardiopulmonary arrests occurredin total of 37 of 21,339 children presented to emergency department during 41 months study period. 15 out of 37 with CPA had return of spontaneous circulation (ROSC) after resuscitation, 4 survived to be discharged from the hospital. The rate of ROSC in the respiratory arrest is higher than the cardiac arrest. Trauma was the most common cause of out-of-hospital pediatric cardiopulmonary arrest. No children who had more 20 minutes of cardiopulmonary resuscitation (CPR) or took more than 3 doses of epinephrine survived. CONCLUSIONS: The 10.4% survival rate after out-of-hospital pediatric CPA is poor. The respiratory arrest is more favorable. Education of basic life support for publics is needed to increase the rate of ROSC and survival from out-of-hospital pediatric cardiopulmonary arrest.

2.A Case of Darier's Disease Associated with Guttate Leukoderma and Acrokeratosis Verruciformis.

The close similarity of acrokeratosis verruciformis(AKV) to the acral lesions of Dariers disease is well known. However, the exact relationship between the two disorders has not been satisfactorily resolved. A case of Dariers disease associated with guttate leukoderma and AKV is presented. The patient was a 17-year-old male who had dark cursted papules on the neck and lower abdomen, flat-topped keratotic papules on the dorsal surface of hoth hands, and white macules disseminated on the trunk and lower extremities of 7 years duration. The histopathologic examination of the lesion on the lower abdomen revealed parakeratotic plugs, corps ronds, and suprabasal clefts. The lesion on the dorsum of the hand showed compact hyperkeratosis, acanthosis, and papillomatosis with the pattern of church spires. The leukodermic lesion revealed decreased melanin content in the epidermal cells.

5.Erythrokeratodermia Variabilis with Alopecia Universalis.

We describe a 8-year-old girl with erythrokeratodermia varibilis (EKV). This diagnosis was supported by erythematous, configurate patches over the entire body. Their size, shape and location varyed with enviromental factors and emotional state. In addition, well defined brownish, hyperkeratotic plaques were scattered over the trunk and extremities. Also, this patient had alopecia universalis, which is not typical of EKV. Etretinate therapy resulted in an almost complete clinical clearing of the plaques; however, her alopecia universalis and erythematous lesions were not affected.

6.A Case of Kleine-Levin Syndrome.

Kleine-Levin syndrome (KLS) is a rare disorder and is diagnosed by recurrent episodes of hypersomnia, hyperphagia, and neurobehavioral dysfunctions. We present a case of a male 19 year-old with seven episodes of the above symptoms. All episodes occurred after a respiratory infectious illness and spontaneously resolved after a few days or a few weeks. A polysomnography showed prolonged sleep latency and decreased REM sleep (14.6%). A multiple sleep latency test revealed a slightly short mean sleep latency (8min 7sec) and three sleep-onset REM episodes in a series of four sleep latency tests across a one day period. The electrophysiological features of previously reported were reviewed.

7.A Case of Angio: Behcet's Syndrome, Arterial Occlusion Type.

We report herein a. case of angio-Behqets syndrome, arterial occlusion type in a 31 year-old male. He had a history of scrotal ulceration, frequent oral ulceration and ocular pain with decreased visual acuity. About 8 months ago, moderately dull pain developed on the volar aspect of left index finger. Subsequently, a bean sized bluish patch developed on the previous painful area and on the ipsilateral thumb tip. Subungual splinter hemorrhages developed on the distal portions of left thumb and index finger nails at the same time. The result of saline puncture test showed the positive reaction in both intradermal and subeutaneous injections. The hrachial angiography showed an obliteration of digital artery at the second proxirnal phalanx of ulnar side.

8.A Study on the T Lymphocyte Subsets and Serum IgE in Patients with Atopic Dermatitis.

This study was undertaken to elucidate any quantitative abnormalities of Tlymphocyte subsets (helper/inducer T cell; T cell and suppressor/cytotoxic T cell: T cell) and IgE levels in the peripheraI blood in atopic dermatitis patients by using monoclonal antibodies and enzyme immunoassay, respectively. In addition we have tried to ascertain whether there are any correlations betveen such immuiiologic abnormalities and the severity of the disease. The results showed that there were significant decrease in the mean percentage of T Cells and increase in the mean value of T/T cell ratios in patients with atopic dermatitis. These changes seem to relate to the severity of the disease when evaluated as groups. A significant increase in the mean value of serum IgE was found in atopic dermatitis patients and the change was related to the severity of the disease. But increase of serum IgE level did not correlated closely with the T cell abnormalities in each patients.

9.Clinical observation of Acute Carbamate Intoxication.

We observed and analysed relating problems concerned to 21 patients of the acute carbamate intoxications, visited the emergency room of cheonju Presbyterian Medical center, from Oct 1993 to july 1996. The results were as follows : 1) Male was more prevalent than female with a ratio of 2:1 in sex distribution and the highest incidence of age group was in third decade and sixth decade of age in male sex 2) the most common drug of intoxication was deltanet in 7 cases, the remainders were bassa in 3 cases, b.p in 3 cases, huradan, qratel and methomyl in 2 cases, each, in order of frequency. 3)Among the exposed carbamate in 13 patients with respiratory failure, detanet was the most common in 6 cases, b.p in 3 cases, methomyl in 2 cases, bassa and huradan each in 1 case. 4) The serum ChE activity of patients were significantly reduced compare to normal.8 patients were between 20~50% of normal,8 patients were above 50% of normal, 4 patients were between 10~20% of normal, only 1 patient was below 10~20% of normal. 5) In 13 patients with respiratory failure, seam ChE activities were below 50% of normal. 6) In all patients with respiratory failure except 1 case time to recovery of the respiratory failure was below 48hrs. 7) Mean dose of atropine administered within first full atropinization and fist 24 hours was significantly higher in patients with than patients without respiratory failure in staitistics(29.3+/-23.3mg, 54.5+/-49.5mg VS 7.6+/-3.6mg, 13.6+/-6.9m) 8) 3 patients among a total of 21 patients expired. So that motality rate was 14.3%. Among the expired cases, 1 patients discontinued treatment due to economic problems and only 2 patients died in hospital during the treatment.
