4.Symptomatic Graular Cell Tumor Involving Intra- and Suprasellar Area: A case report.

Symptomatic parasellar granular cell tumor is a very rare tumor. To the best of our knowledge, 43 cases was be found in the English literatures. We recently experienced a case of a parasellar granular cell tumor in a 61-year-old female who had bilateral temporal hemianopsia and severe panhypopituitarism. The tumor was composed of diffuse sheets of polygonal cells with abundant eosinophilic PAS positive granular cytoplasm. In the immunohistochemical and ultrastructural examinations, the tumor failed to show any evidence of Schwann cell or glial differentiation. These findings suggest that granular cell tumor has heterogenous cell types of origin.

5.A study on type 1 allergy to house dust mite in patients with atopic dermatitis.

The authors investigated the radioallergosorbent test (RAST) for the specific IgE and the prick test with hose dust mite in 129 patients with atopic dermatitis(AD) wit4 four or more family numbers. RAST and prick test results were described according to family background of respiratory atopy(RA) and severity of AD. The results are summarized as follows : l. A comparison between the prick test results and RAST reactions gave a concordance of 88%. The prick test was more sensitive than the RAST. 2. The level of specific IgE in patients with personal history(PH) or family history(FH) of RA was significantly increased compared to those in patients with only AD. 3. In patients without PH and FH of RA, the level of specific IgE in patients with FH of AD was significantly increased compared to those in patients without FH of AD. 4. The level of specific IgE did not reflect the severity of AD. From the above results, it could be concluded that type I allergy to house dust mite is not directly related to AD and it occurs predominantly in those patients with AD wha have a predisposition to RA.

6.Clinical Study of Ipsilateral Femur and Tibia Fractures

Twenty five cases of fractures of the femur & tibia on the same leg (floating knee) in 24 patients were treated in St. Mary's Hospital, Catholic Medical College and Center during the period 1977-1985. We studied all of these patients, divided by four groups according to the methods of treatment, retrospectively with analysis of treatment and end result. Our policy of treatment for these multiple fractures in single extremity was directed toward early weight bearing, active and passive knee exercise by early open reduction and rigid internal fixation for both fractures. The results obtained were as follows: l. Of 25 cases, 20 patients were male and 5 were female. 2. The right lower extremity has constituted 56% of the cases. 3. Average age of patients were 37 years. 4. Most common level of fractures was on middle one third of femur (71.4% ) and proximal one third of tibia (46.2%). Of these 25 cases, three cases of femur and one case of tibia were segmental fractures, and one case of femur and five cases of tibia were open fractures initially. 5. Of 28 cases of femur fracture, 23 were given operative treatment and five conservative method. But out of 26 cases of tibia fracture, each half cases of fracture were treated operatively and conservatively. 6. We analysed the final results of treatment by the time of fracture union roentgenologically and five lower limb functions clinically at the termination of treatment. The average time for fracture union was 20.5 weeks for femur and 21.9 weeks for tibia roentgenologically. The acceptable clinical results could be achieved in 18 cases (72%) of early open reduction and rigid internal fixation for both tibia and femur as early as possible after accident for early knee exercises. 7. The status of fractured tibia was the most important factor to treat these multiple fractures in single extremity because of troublesome to start knee exercise and weight bearing.

8.Age - Related Changes of Glycosaminoglycan in Senescence - Accelerated Mouse ( SAM ) Skin.

BACKGROUND: The various manifestations of intrinsic cutaneous aging may reflect the age-related changes of dermal ground aubstances, the important components of the dermis. OBJECTIVE: This study was undertaken to observe whether there is any age-relsted change, quantitatively and qualitatively, in dermal glycosaminoglycan(GAG) METHODS: The Senescence-A.ccelerated-Mice(SAM) strain "Prone" to develop senescence(P/8), aged 4 days(P-Y a group, n=5) and 4 months old (P-0 group, n=5) were used, with SAM strain Resistance to senescence (R/1), aged 4 montha old (R-0 group, n=5). The whole skin of SAM was incubated in 0.1 sodium phosphate buffer(NaPB), 2M guanidine-HC1, and 4M guanidine-HCl, sequencially, for the extractiop of GAG. The amount of GAG was measured by using alcian blue and by methylene blue metachrometic assay. Uronic acid(UA) was estimate l employing carbazole reaction. Extracted skin protein profiles were analyed by SDS-PAGE. RESULTS: 1. The total contents of GAG per wet weight of skin, as measured uairq, alcian blue, was highter in R-0 group than R-Y and P-Y group, which is statistically significant. 2. The methylene blue metaehromatic assay yielded highter absorbance values in 2M guanidine-HCl extract than NaPB ext:racts. 3. The total contents of UA decreased with aging in R strains, buttriking increase was noted in the P strain. 4. On SDS-PAGE, the protein profiles of NaPB extracts showed similarity to serum protein. 125 kDa protein band was noted only in guanidine-HCl extracts. 37.40kDa protein bands were appeared in 2M, 4M guanicline-HCl extract of R-0 group. But. there was no significant difference in both strains. CONCLUSION: These findings suggest that macromolecules, such as GAG, is one of the target molecules of the cutaneous eging process, and these change muy be related to the age-related changes of dermal water content.

10.A Case of Irritant Dermatitis due to Pulsatilla Koreana.

Pulsatilla koreana is a member of the buttercup familly(Ranuculaceae) which contains the unsaturated lactone, protoanemonin(C3H4O2), formed after injury to the plant by the breakdown of the glycoside ranunculin. It has been prescribed as a herbal medicine or a folk medicine for antipyretic, analgesic, anti-inflammatory, astringent and hemostatic effects in Korea. A 20 year-old famale patient presented with confluent vesicobullae on the erythematous base with a burning sensation on her upper back. This was caused by contact with crushed Pulsatilla koreana, a folk medicine for the treatment of left facial nerve palsy, that had occurred 10 days before her visit. A provocation patch test with Pulsatilla koreana upon a heathy male volunteers upper back showed strong positive reactions with the stalk and leaf in fresh and frozen stat.e and after thaw ing resptively. However no reaction was seen when the folk medicine was used in a dried form or with the root in any state. This case indicates that misadvice and ignorant folk remedies could cause misfortunate results to innocent people.
