2.The Relationship between Cerebral Reperfusion Flow and the Ischemic Histopathologic Damage after Incomplete Forebrain Ischemia in Rat Model.

BACKGROUND: Experimental data indicate that low-flow reperfusion following prolonged cardiocirculatory arrest may aggravate early cerebral microcirculatory repefusion disorders. We investigated the influence of cerebral repefusion flow change to the ischemic histopathologic damage of brain tissue after incomplete forebrain ischemia in rats. MATERIALS AND METHOD: Anesthetized Sprague-Dawley rats were undergone ligation of both infernal carotid artery by microvascular clamp for 10 minutes. After release of the clamp, reperfusion was started with several different flow levels (0, 10, 20, 30, 50, and 100%) of infernal carotid artery comparing to pre-clamping phase using flowmeter. After 15minutes of reperfusion, rat brains were prepared by perfusion-fixation with 3% formaldehyde. Under light microscopic examination of Hematoxylin-Eosin stained tissue slide, histopathologic damage was examined at cortex, putamen, and hippocampus regions. Categorical hisotopathologic damage scores were derived in each regions by manual counts of ischemic neurons. RESULT: The histopathologic damage scores were 0, 10. 2+/-0.5, 7.6+/-1.5, 5.9+/-1.4, 5.0+/- 2.8, 3.5+/-0.7, and 1.0+/-0.0 in control, 0, 10, 20, 30, 50, and 100% reperfusion groups, respectively(p<0.05). CONCLUSION: Our insults showed significant increment of brain histopathologic damage scores along with decreasing amount of cerebral reperfusion know after incomplete forebrain ischemia. We believe restoration of repefusion flow to pre-ischemic level would be a critical component in attenuation of brain ischemic damage.

3.Slumping tendency and rheological property of flowable composites.

The aim of this study was to develop a method for measuring the slumping resistance of flowable resin composites and to evaluate the efficacy using rheological methodology. Five commercial flowable composites (Aelitefil flow:AF, Filtek flow:FF, DenFil flow:DF, Tetric flow:TF and Revolution:RV) were used. Same volume of composites in a syringe was extruded on a glass slide using a custom-made loading device. The resin composites were allowed to slump for 10 seconds at 25degrees C and light cured. The aspect ratio (height/diameter) of cone or dome shaped specimen was measured for estimating the slumping tendency of composites. The complex viscosity of each composite was measured by a dynamic oscillatory shear test as a function of angular frequency using a rheometer. To compare the slumping tendency of composites, one way-ANOVA and Turkey's post hoc test was performed for the aspect ratio at 95% confidence level. Regression analysis was performed to investigate the relationship between the complex viscosity and the aspect ratio. The results were as follows. 1. Slumping tendency based on the aspect ratio varied among the five materials (AF < FF < DF < TF < RV). 2. Flowable composites exhibited pseudoplasticity in which the complex viscosity decreased with increasing frequency (shear rate). AF was the most significant, RV the least. 3. The slumping tendency was strongly related with the complex viscosity. Slumping resistance increased with increasing the complex viscosity. The slumping tendency could be quantified by measuring the aspect ratio of slumped flowable composites. This method may be applicable to evaluate the clinical handling characteristics of flowable composites.

4.Prognosis and Complications of Depressed Skull Fracture.

Complications such as infection, seizure, or intracranial hemorrhage are relatively common after depressed skull fractures. However, indications, methods, and efficacy of the surgical treatment are controversial. This study includes 113 patients with depressed skull fractures who were treated at Soonchunhyang University Chonan Hospital between 1989 and 1992. Methods of treatment, complications, and the prognosis were retrospectively collected and the prognostic factors were analysed. Depressed skull fracture was common below the age of 20 years(38%). Male to female ration was 5.3:1. Traffic injury was the most common of depressed skull fracture(68.1%). Glasgow coma score(GCS) on admission was 13-15 in 52%, 9-12 in 18.6%, and closed in 32%. Intracranial injury was accompanied in 43%. The frontal area was the most common site of depression(53%), and the parietal region was the next(28%). In 65% of open fractures and 28% of closed ones, the lesions were surgically corrected. Cranitomy with primary reconstruction was the most common operative procedure. The mean hospital stay was 26 days in surgically treated patients, and it was 17 days in conservatively managed patients. Prophylactic anticonvulsants were administered in 43%. Seizures occured in 6.2%, and infections were encountered in 3.5%. The outcome was favorable(good recovery and moderate disability) in 80% and the mortality rate was 16%. The outcome was better when the GCS on admission was high(P<0.005), the pupillary response was normal(P<0.005), and the lesion was treated by surgery(P<0.005). Seizures were more common when the GCS was low(P<0.005). Although infections were developed in surgically treated patients only, this difference lacked a statistical significance.

5.Sexual Dysfunctions in Stroke Patients.

BACKGROUND: Limited information about the effects of stroke on sexual functioning is available. The purpose of this study was to assess the influences of ischemic stroke on sexual functioning and to evaluate the clinical and psychological factors related to poststroke sexual dysfunctions. METHODS:Sixty-six male post-ischemic stroke patients and fifty-one age-matched healthy controls completed a self-administered questionnaire on sexual functioning. The localization of ischemic stroke was determined by neuroimaging findings as well as focal neurological signs. The degree of disability and the degree of depression were also assessed. RESULTS: All domains of sexual functioning, including erectile functions, orgas-mic functions, sexual desire, intercourse satisfaction, and overall satisfaction were decreased in poststroke patients. Patients with occipital lesions had a lesser degree of sexual dysfunctions than those with lesions affecting other areas (p<0.05). Also, sexual dysfunctions in stroke patients were related to the degree of physical disability and the presence of depression (p<0.05). CONCLUSIONS Sexual dysfunctions are common in stroke patients. The reasons for sexual dysfunc-tions after stroke are multifaceted, which include stroke lesion site, physiological factors, and psychosocial factors. (J Korean Neurol Assoc 19(4):342~348, 2001)

6.Liver Abscess in Childhood.

We have presented an analysis of cases of liver abscess from the record of the Department of Pediatrics, Yonsei medical College, during the 10 year 8 months period from Jan. 1969 through Sept. 1979. The incidence of admission in pediatric was 0.05025%. There was male preponderance. The most frequently encountered symptoms and sings were fever and chill, anorexia, hepatomegale, pain and tenderness on right upper quadrant of abdomen. The pathogenetic mechanisms were lodgement in the liver of hematogenously dissemination, ascending infection of cholangitis, trauma. Cryptogenic liver abscess was seen in two cases. There were 5 cases of pyogenic liver abscess and 3 cases of amebic liver abscess. Undetermined etiology was noted in 2 cases. Roentgenography including scan is the most helpful diagnostic tool available. Only one patient with post-traumatic liver abscess was expired.

7.Development of nano-fluid movement measuring device and its application to hydrodynamic analysis of dentinal fluid.

This study was aimed to develop an instrument for real-time measurement of fluid conductance and to investigate the hydrodynamics of dentinal fluid. The instrument consisted of three parts; (1) a glass capillary and a photo sensor for detection of fluid movement, (2) a servo-motor, a lead screw and a ball nut for tracking of fluid movement, (3) a rotary encoder and software for data processing. To observe the blocking effect of dentinal fluid movement, oxalate gel and self-etch adhesive agent were used. BisBlock (Bisco) and Clearfil SE Bond (Kuraray) were applied to the occlusal dentin surface of extracted human teeth. Using this new device, the fluid movement was measured and compared between before and after each agent was applied. The instrument was able to measure dentinal fluid movement with a high resolution (0.196 nL) and the flow occurred with a rate of 0.84 to 15.2 nL/s before treatment. After BisBlock or Clearfil SE Bond was used, the fluid movement was decreased by 39.8 to 89.6%.

8.New Chronic Osteomyelitis Model in White Rabbit: A Preliminary Report.

The purpose of this study was to develope a rabbit model for the chronic osteomyelitis, which is reproducible, controllable in quantity of bacteria and suitable for toxicologic research and therapeutic intervention studies. Osteomyelitis was induced in white rabbits by injecting varying numbers of S. aureus(ATCC 19636, 49230) and Alginate-CaCl2 into the proximal metaphysis of tibia. Three rabbits were used in each number of S. aureus respectively. The tibia were harvested at 8 weeks later and evaluation was done by clinical, radiological and histological findings. Clinical sings of infection consisted of soft tissue swelling and limping in rabbits, Radiologic findings were periosteal reaction, osteolysis, new bone formation in proximal tibias. Histology showed chronic active inflammation, debris of alginate, clusters of bacterial and granulation tissue. In ATCC 19636(more than 6X105) inoculated rabbits, osteomyelitis was established consistently in all three rabbits. Using Alginate-CaCl2 and ATCC 19636 Staphylococcus aureus, we made a new chronic osteomyelitis model, reproducible and controllable in quantity of bacteria.

9.The Effect on Age, Body Weight, Height, Body Mass Index and Smoking to Coagulation Factor VII, VIII and Fibrinogen in Pilots.

BACKGROUND: The pilots with cardiovascular events have a possibility for a risk of suffering from sudden incapacitation which is closely related to flying safety. The coagulation factors such as fibrinogen, factor VII, and factor VIII are possibly related to cardiovascular events. Several studies for general population have shown that an increase of those coagulation factors with age, a correlation of Factor VII and fibrinogen with BMI, and of fibrinogen with smoking. However, this study is to find out whether pilots' age, body weight, body mass index (BMI) and smoking are related to the baseline measurement of coagulation factor VII, factor VIII and fibrinogen. METHOD: Samples were taken from 21 pilots from Asiana Airlines: 11 smokers and 10 non-smokers. In order to measure the relationship between age, body weight, BMI, and the coagulation factors, Pearson correlation was used in this analysis. Independent two sampled t-test was used to analyze the correlation between smoking and the coagulation factors. RESULTS: Mean age, mean height, mean body weight and mean BMI of pilots were examined: 38 years, 171.81 cm, 70.67 Kg and 23.94 Kg/m(2). Mean fibrinogen, mean factor VII, and mean factor VIII were also obtained: 236.0 mg/dl, 92.93%, and 60.16%. The coagulation factor VII, factor VIII and fibrinogen were not significant related to age, body weight, BMI, smoking of pilots. CONCLUSION: This study has no correlation between age, body weight, BMI, smoking and the coagulation factors because the age of this study does not have pilots with over 60 years old and healthy behaviors (e.g., exercise, smoking, drinking, etc.) of most pilots are relatively well.

10.13 Cases of Leukoderma Punctata after Narrowband UVB Therapy in Vitiligo Patients.

BACKGROUND: Leukoderma punctata is a peculiar disorder with punctiform, hypopigmented and achromic spots. Histopathologically, a focal reduction of melanocytes and melanin granules are found in the depigmented macule. The phototoxic effect of phototherapy on melanocytes has been suggested as the most likely cause. This occurs mostly after application of systemic or topical psolaren on photochemotherapy. However, a case following narrowband UVB therapy is very rare. OBJECTIVE: The purpose of our study was to investigate the characteristics of leukoderma punctata patients and find the cause of leukoderma punctata. METHODS: We reviewed the medical records of leukoderma punctata patients, and a skin biopsy was taken from punctiform hypopigmented macules. The tissues were stained with hematoxyline and eosin, Fontana-Masson, and dopa-oxidase. RESULTS: The average age of patients was 51 years old. There was no difference between results of male and female patients. Involvement sites of patients were mostly the arms and legs. 76% of patients did not have preceding subjective symptoms. In eight of nine patients, a marked reduction of melanin and melanocytes was noted with both Fontana-Masson staining and dopa-oxidase staining. The average total period of PUVA therapy was 26 months and average total dose of PUVA irradiation was 351.2 J/cm2 in leukoderma punctata patients. Before leukoderma punctata developed, the average total period of narrowband UVB therapy was 6.9 months and average total dose of narrowband UVB was 22.45 J/cm2. The results suggest that leukoderma punctata developed in very small dose of narrowband UVB, although it did not develop with large doses of PUVA. CONCLUSION: Narrowband UVB (311 nm) is the most likely spectrum for development of leukoderma punctata.
