1.Transfemoral Internal Spermatic Vein Occlusion for Varicocele Using a Stainless Steel Spring Coil.

Transvenous embolization of the internal spermatic vein appears to be at least as effective as open surgical correction in treating men with varicocele and it has several advantages ; (1) it requires only local anesthesia and a short hospital stay ; (2) there is no risk of damage to the spermatic artery : (3) the veins can be occluded accurately whatever the arrangement of any collateral vessels. We reported a technique for non-operative management of varicocele by a occlusion of the internal spermatic vein using stainless steel spring coil.

2.A Case of Epidermoid Cyst of Testis.

Benign tumors of the testis are rare, accounting for one percent of all testicular tumors including epidermoid cyst of the testis. Herein we report one case of epidermoid cyst of the testis and discuss the pathological criteria for diagnosis and clinical management of the lesion.

3.Comparison of screw-in effect of three NiTi file systems used by undergraduates.

The purposes of this study were to compare the apical terminus width of simulated curved root canal prepared with three NiTi file systems used by undergraduates for evaluation the effects of flute angle and pitch or radial land on reducing screw-in effect and to determine more safe NiTi file system for inexperienced operators. Fifty inexperienced undergraduate students prepared 150 simulated curved root canals in resin blocks with three NiTi file systems ; ProFile(R), Hero Shaper(R), K3(TM). The electric motor set at a speed of 300 rpm and torque of 30 in a 16 : 1 reduction handpiece was used. The simulated root canal was prepared to ISO #25 sizes with each file system. The scanned images of pre- and post-instrumented canal of resin block were superimposed. To evaluate the screw-in effect of three NiTi file systems, apical terminus width of root canal was measured from superimposed images and statistical analysis was performed. There were significant differences in three NiTi file systems. ProFile(R) had significantly smaller width than Hero Shaper(R) and K3(TM) (P < 0.05), but no significant difference was observed between K3(TM) and Hero Shaper(R). Under the condition of this study, active file system (Hero Shaper(R), K3(TM)) with variable pitch and helical angle had more screw-in effect than passive file system (ProFile(R)) with constant pitch and helical angle. It seems that the radial lands play more important role in reducing screw-in effect.

4.The Difference of Neural Activity in Viewing between one's own Emotional Situation and Other's: A Neuroimaging Study about Perspective-Taking.

OBJECTIVES: This study aimed to investigate the difference of brain activities in viewing a common emotional situation according to perspective-taking. METHODS: Using fMRI, brain activities were measured while performing the task viewing a common emotional situation on either self-perspective or third-person perspective in fourteen healthy men. The relatively activated brain areas on each perspective were investigated, then the relationship between the activated brain regions and the scores of self report about some emotion or empathic ability were explored. RESULTS: The relatively activated brain areas on self-perspective were bilateral paracentral lobule(BA 5), right postcentral gyrus(BA 3), right precentral gyrus(BA 4), left superior temporal gyrus(BA 22), left medial frontal gyrus(BA 6), whereas on third-person perspective right inferior frontal gyrus(BA 47), left caudate body and tail, right superior temporal gyrus(BA 38), right medial frontal gyrus(BA 8) were activated. The relative activity of left superior temporal gyrus on self-perspective was positively correlated with the score of Beck Depression Inventory. CONCLUSION: This study demonstrated that the activated brain region according to perspective-taking were different while viewing a common emotional situation. The depressive feeling would have an influence on the brain activity related to perspective-taking.

5.Leptomeningeal Carcinomatosis in Solid Tumors; Clinical Manifestation and Treatment.

PURPOSE: Leptomeningeal carcinomatosis occurs in about 5% of patients with solid tumor and is being diagnosed with increasing frequency as patients live longer and as neuro-imaging studies improve. In general, the most commom cancers that involved the leptomeninges are breast cancer, lung cancer, and malignant melanoma. MATERIALS AND METHODS: We investigated 25 patients presented with multiple neurologic symptoms and signs who were diagnosed with leptomeningeal carcinomatosis at the Yonsei Cancer Center from January 1990 to December 1999. RESULTS: The primary disease of leptomeningeal carcinomatosis were stomach cancer (10 cases), breast cancer (7 cases), lung cancer (5 cases), unknown primary cancer (2 cases) and common bile duct cancer (1 case). All patients were presented with multiple neurologic symptoms and signs involving the central nervous system (CNS), cranial nerve or spinal nerves. Twenty-one of twenty- five patients were treated with intrathecal chemotherapy, radiotherapy, or combination therapy. Fourteen of them (66.7%) experienced improvement or stabilization of neurologic symptom and sign. The median survival was 122 days (10-2190). CONCLUSION: In conclusion, although early diagnosis and active treatment of leptomeningeal carcinomatosis may improve the quality of life in selected patients, the median survival was relatively short. Therefore, new diagnostic and therapeutic strategy for leptomeningeal carcinomatosis were needed.

6.An fMRI Study Regarding the 'Theory of Mind' in Korean Schizophrenic Patients Using a First Order False Belief Task.

OBJECTIVES: The aim of this study was to investigate brain activation during a Korean language-based 'theory of mind (TOM)' task and fMRI in Korean schizophrenic patients. METHODS: Fourteen Korean schizophrenic patients and 15 normal controls participated in this study. For all participants, several clinical states and psychosocial functions were evaluated. The subjects were then scanned while performing Korean language-based fMRI tasks. The tasks were comprised of conditions-first order false belief (TOM task), physical causality, and unrelated situations. Imaging data were analyzed using SPM2 software (uncorrected p<0.005, extent threshold kappa=10). RESULTS: 1) Compared with the control group, the patient group showed significantly poorer performance on the TOM task, and no significant correlation between TOM and empathic abilitiesy. 2) In the patient group, there were no significantly activated brain regions associated with the TOM task as compared to the physical causality task. With respect to between-group differences, the patient group showed significantly less activation of the left medial frontal region (primarily BA 8) and signifcantly different activation of the left precuneus (BA 7) associated with the TOM task. CONCLUSION: These results suggest that Korean schizophreniac patients show different brain activity associated with TOM functions, especially with respect to the Korean language-based first order false belief tasks.
