1.The Factors that affect the Health Promoting Behaviors of Middle-aged Workers between Type D and Non-type D Personality.

PURPOSE: This study is to identify the factors that affect middle-aged workers' health promoting behaviors. METHODS: 214 middle-aged workers participated in surveys, and the data were collected from August to September 2012 and analyzed by using descriptive statistics, Pearson's correlation coefficient, and multiple regression analysis with PASW 18.0 program. RESULTS: It was found that among the factors, the degree of job stress (t=7.69, p<.001) and depression (t=6.23, p<.001) were significantly high for type D individuals, while non type D showed meaningful degree of self-related health status (t=-3.66, p<.001) and health promoting behaviors (t=-4.71, p<.001). The notable variable that affected the health promoting behaviors of the middle-aged workers was depression for both type D (beta=-.357, p= .029) and non type D (beta=-.325, p<.001) individuals, and this variable accounted for 24.1% and 18.2% respectively. CONCLUSION: The study revealed that the middle-aged workers showed high degree of job stress and depression that influenced on their health promoting behaviors. Thus, they are recommended to receive health management programs that offer treatment through consulting considering their individual personalities.

2.Factors Affecting Subjective Quality of Life of Hospice Volunteers.

PURPOSE: This study tested a model of hospice volunteer's motivations and outcomes to encourage more volunteer workers to participate in hospice care. METHODS: This included 200 volunteers who attended the Autumn Conference in 2007 and 132 volunteers from five hospice organizations. RESULTS: Volunteers' existential well-being affected an endogenous variable of volunteers' participation in volunteer activities. The durability of volunteer activity was affected by value motivation, the level of participation in volunteer activities and the level of job satisfaction. For volunteers' job satisfaction level, valid endogenous variables included existential well-being, social motivation, understanding motivation and value motivation. The durability of volunteer activities was indirectly affected through three different paths. Volunteers' existential well-being determined the level of their participation in volunteer activities, which in turn affected the durability of such activities. Social motivation influenced volunteers' job satisfaction, and then the durability of volunteer activities. Volunteers' understanding of motivation also influenced their job satisfaction, and then the durability of volunteer activities. CONCLUSION: Based on these results, this study proposes that the durability of volunteers activities could be improved by developing a program to improve volunteers' well-being and by providing volunteers with education on altruistic values, encouraging them to seek intellectual growth, and advocating them to continue volunteer activities based on close relationships with other volunteers.

3.Developing Job Description for Dietitians Working in Public Health Nutrition Areas.

The purpose of this study was to develop a standardized job description for dietitians working in the public health nutrition area. Work-oriented job analysis methodology was employed for the study purpose. Subjects of 38 dietitians currently working at health centers in 2002 were recruited. Based on the focus group interview with 7 public health nutritionists and 7 professors, information about task elements was collected. Questionnaires measuring work performance and self-perception of importance of the selected task elements were administered. Reliability and validity of this instrument were tested by Chronbach's alpha and factor analysis. SAS PC package program was used for the statistical analysis. The final developed job description for public health nutritionists included 5 duties, 20 tasks and 93 task elements. The results of this study can be summarized as follows; 1) 5 duty areas are A. plan and evaluation of public health nutrition services, B. developing nutrition education materials, C. implementing nutrition services, D. networking community, and E. self development. 2) Each duty area from A to E was composed with 6, 2, 6, 4, 2 tasks, respectively. 3) Each duty area from A to E was composed with 24, 8, 38, 14, 9, and 2 task elements, respectively.

4.Effects of Surgery Information Service on One-Day Surgery Patients' Anxiety and Satisfaction with Nursing Care.

PURPOSE: The purpose of this study was to determine the effects of surgery information service on one-day surgery patients' anxiety and satisfaction with nursing care. METHODS: The study used the nonequivalent control group time difference design. Sampling and measurement of the control group participants (n=30) was completed first. Later, participants in the experimental group (n=30) were sampled, intervened, and measured. The experimental group participants received the surgery information services twice: before and after the surgery, 20~30 minutes for each of the sessions. The anxiety was measured with the State-Trait Anxiety Inventory developed by Spielberger (1975), and modified by Kim & Shin (1978). The patient satisfaction with nursing care was measured with the Patient Satisfaction with Nursing Care Scale developed by La Monica and colleagues (1986), and modified by Shin (1999). The data was collected between February 1 and May 30, 2006. RESULTS: The results were as follows: 1) The level of anxiety in the experimental group was not significantly different from that in the control group. 2) The level of patient satisfaction with nursing care in the experimental group was significantly higher than in the control group (t=-4.53, p=.00). CONCLUSION: These findings suggested that the one-day vocal cord surgery information service could be a useful nursing intervention to improve patient satisfaction with nursing care, but not for controlling the anxiety of one-day surgery patients.

5.Comparative evaluation of biochemical and microscopic urinalysis in pediatric population.

The authors determined the value of performing urine microscopy on biochemically negative urine sepcimens in a pediatric population. The 746 urine specimens of patients who were either visited or admitted to the Department of Pediatrics, National Police Hospital, from March 1991 to July 1992 were reviewed on the basis of comparative evaluation of biochemical and microscopic urine analysis. Eight reactions of Clinitek(r) 200 were used as biochemical indicators, namely, pH, nitrite, albumin, glucose, ketone, urobilinogen, bilirubin, and blood. Out of 746 urine specimens studied, 186 were true positive. Thirty-six specimens reacted biochemically in the absence of positive microscopic findings (false postive), 492 specimens were true negatives. Thirty-two specimens had negative biochemical indicators, in spite of positive microscopy. The sensitivity of the biochemical parameters for predicting significant microscopy of urinary sediment is 85% and the specificity is 99%. The positive predictive value is 69.7% and the negative predictive value is 99.3%. The sensitivity of blood for RBC is 94%. and the specificity is 99%. The predictive value of a positive result is 84.6% and that of a negative result is 99.7%. We therefore conclude that urine microscopy is less necessary in biochemically negative urine specimens from asymptomatic pediatric patients, with hopefully resultant time and cost effectiveness.

Trichilemmal horn(trichilemmal keratosis) is a rare keratinizing tumor that resembles a cutaneous horn. Histologically, it is characterized by an abrupt maturation of keratinocytes into lamellar keratin without the formation of a granular layer. We describe a case of a trichilemmal horn on the right cheek of a 78-year-old woman.

7.Can the C-14 Urea Breath Test Reflect the Extent and Degree of Ongoing Helicobacter pylori Infection?.

PURPOSE: The C-14 urea breath test (C-14 UBT) is the most specific noninvasive method to detect Helicobacter (H) pylori infection. We investigated if the C-14 UBT can reflect the presence and degree of H. pylori detected by gastroduodenoscopic biopsies (GBx). MATERIALS AND METHODS: One hundred fifty patients (M:F=83:67, age 48.6+/-11.2 yrs) underwent C-14 UBT, rapid urease test (CLO test) and GBx on the same day. For the C-14 UBT, a single breath sample was collected at 10 minutes after ingestion of C-14 urea (137 KBq) capsule and counting was done in a liquid scintillation counter for 1 minute, and the results were classified as positive ( 200 dpm), intermediate (50~199 dpm) or negative (<50 dpm). The results of CLO tests were classified as positive or negative according to color change. The results of GBx on giemsa stain were graded 0 (normal) to 4 (diffuse) according to the distribution of H. pylori by the Wyatt method. We compared C-14 UBT results with GBx grade as a gold standard. RESULTS: In the assessment of the presence of H. pylori infection, the C-14 UBT global performance yielded sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy of 92.5%, 88.4%, 97.1%, 88.4% and 91.3%, respectively. However, the CLO test had sensitivity, specificity, PPV, NPV and accuracy of 83.2%, 81.4%, 91.8%, 81.4% and 82.7%, respectively. The quantitative values of the C-14 UBT were 45+/-27 dpm in grade 0, 707+/-584 dpm in grade 1, 1558+/-584 dpm in grade 2, 1851+/-604 dpm in grade 3, and 2719+/-892 dpm in grade 4. A significant correlation (r=0.848, p<0.01) was found between C-14 UBT and the grade of distribution of H. pylori infection on GBx with giemsa stain. CONCLUSION: We conclude that the C-14 UBT is a highly accurate, simple and noninvasive method for the diagnosis of ongoing H. pylori infection and reflects the degree of bacterial distribution.

9.Three cases of purpura fulminans.

Purpura fulminans is one of rare consumptive coagulopathy in children. The most common predisposing conditions of purpura fulminans are infectious disease like streptococcal infection and chickenpox. This disease is characterized by ecchymotic lesions that are usually distributed symmetrically on the lower extremities and buttocks. These ecchymotic lesions undergo necrosis, unless there is effective treatment. We experienced 3 cases of purpura fulminans which improved almost completely after early heparin administration. In the case 1, a 12 month old girl, purpura fulminans developed during sepsis and gastroenteritis. In the case 2, a 4 month old boy, purpura fulminans developed during acute sepsis. We reported 3 cases with a brief review of related literature.

10.Clinical Observation in 26 Cases of Indigenous Malaria in 1995.

OBJECTIVE: In Korea, control activities against the endemic malaria due to Plasmodium vivax had been done in 1960s. It is now widely accepted in Republic of Korea that indigenous malaria stopped its endemicity in the mid-1970s. But a vivax malaria case without history of being abroad was recognized in 1993. In addition to this case, 20 cases of indigenous malaria were detected in 1994. And they were progressively increased to about 80 cases in 19%. We report these recently developed cases. METHODS: In a 7-month period between June and December 1995, high fever patient were examined. Patients having a narcotic drug injection or those who had blood transfusion were excluded. Laboratory studies including blood cell count, peripheral blood smear and abdominal sonogaphy were taken. RESULTS: Over 3-month period from July through September 19%, 26 cases of new indigenous malaria were diagnosed. In a blood smear, plasmodium vivax was diagnosed. CONCLUSION: The twentysix new indigenous malaria cases occured. Because all the detected patients had no history of visiting endemic countries, they were not imported cases. Vivax malaria in the present patients can be contracted by infected mosquitos which came across the militarized zone (DMZ). Officially speaking, DMZ is a 4 km wide between the north and south borders, Hut the width is much narrower in many parts. However, we do not have any information on the malaria situation in the north Korea. In order to evaluate a significance of the recent indigenous malario occurrence, a surveillance system should be operated in these areas.
