1.A Case of Pulsating Exophthalmos.

A case of pulsating exophthalmos is reported in a 50 years old Korean Woman. The complaints of this case were exophthalmos, chemosis on Rt. eye and intracranial bruit. The etiology of this case is considered due to toxemia of pregnancy and hypertension without trauma. A brief review of the literatures related with the etiology, symptom and therapy is made.

2.Comparison of Techniques for Correcting the Prominent Ear in Rabbits.

The relative merits of cartilage scoring versus suturing in techniques for correcting the prominent ear remains a subject of debate among leading plastic surgeons. We compared a variety of echniques for correcting the prominent ear in 72 rabbit ears using scoring, horizontal mattress sutures, and combination scoring and suturing. The ears were splinted with a right-angle fold for 3 postoperative weeks, and the animals were sacrificed after 8 weeks. Suturing techniques in combination with scoring maintained an angulation significantly closer to the desired 90 degrees than cartilage scoring or cartilage suturing only(p<0.05). Histological analysis demonstrated a significant increase in cartilage hyperplasia by suturing alone, whereas the other techniques achieved only mild to moderate increases. We recommend cartilage scoring and suturing through skin incision on severe degrees of prominent ear, and cartilage needle scoring and buried suturing through slit incision on mile-to-moderate degrees of prominent ear.

5.The effectiveness of hyaluronidase for calcium gluconate-inducing extravasation necrosis : a dose and time response.

Extravasation necrosis due to intravenously administered fluids and drugs is an increasing problem in hospital practice. The incidence of extravasation is variable but skin necrosis is a potentially devastating complication of intravenous therapy. Local injection of hyaluronidase has been recommended for several types of infusion extravasations. The previous studies found hyaluronidase to be effective in the prevention of necrosis following intradermal nafcillin, 12% dextrose, sodium bicarbonate, aminophylline or vince alkaloids.The objectives of the study are to determine the dosage of hyaluronidase which is effective in reducing extravasation necrosis caused by 10% calcium-gluconate and to establish how soon after this extravasation it must be given to retain its effectiveness.Study I evaluated control versus only normal saline group and normal saline with hyaluronidase groups (dose:75,150,300,450 units; all in volume 2 ml, treatment delay:immediate, 15-minutes delay, half hour delay, one hour delay, three hour delay). Size and rate of eschar were compared between groups. Study II was undertaken to examine the evolution of calcium-gluconate induced soft tissue injury in the rabbit. The histologic findings of extravasation sites were compared between groups. A statistically significant protective effect was found in the treated group versus the nontreated group within 15 minutes to 30 minutes. The most effective protection was achieved by the immediate injection of 300 units dosage of hyaluronidase.In conclusion, in the 10% calcium gluconate-induced extravasation, the given data suggest that one can expect the most protective effect with a 300 units dosage of hyaluronidase and within half-hour delay in the treatment group.

8.Nature of Stromal Cells in Cerebellar Capillary Hemangioblastoma: Immunohistochemical analysis.

The origin of the stromal cell of cerebellar hemangioblastoma has long been studied electron microscopically and immunohistochemically. The results and theories about the stromal cell origin are variable and plentiful. However, the exact origin of the stromal cell remains controversial. The present study is aimed to elucidate the nature of the stromal cell of cerebellar hemangioblastoma. Ten cases of hemangioblastoma in Severance Hospital were used for immunohistochemical analysis of the stromal cell. The immunohistochemical staining of GFAP, S-100 protein, NSE, alpha-l-antichymottypsin, cytokeratin, CD 68, factor VIII related antigen, and synaptophysin were performed. The results were as follows; GFAP and S-100 protein were stained mainly but weakly in bellar capillary spindle cell and cellular process. NSE was stained mainly in foam cells, and 6 cases among them revealed strong reaction. ct-l-antichymotrypsin was stained in a few foam cells of 5 cases. Cytokeratin, CD 68, factor VIII related antigen, and synaptophysin showed negative reaction. Based on these results, it is considered that the origin of the stromal cell is histiocytic or neurogenic rather than glial. The weak positivity of GFAP and S-100 protein may support the neurogeriic origin but ct-l-antichymotrypsin positivity does not support the possibility. The positivity of GAP and S-100 protein supports the phagocytic action of histiocytic cell and suggests histiocytic origin rather than neurogenic.
9.The Effect of Alcohol on the Blood Lipid Level of Korean Adult Men.

This study was conducted to evaluate the relations between alcohol intake and blood lipid level in a group of 1,138 Korean adult men, ages 20 to 69 years. Total number of each of a variety of drinks in the previous two weeks was obtained by questionnaire and converted into grams of alcohol consumed in a week. The levels of blood lipid such as Cholesterol, HDL-Cholesterol, Triglyceride and LDL-Cholesterol were examined by enzyme method. We also observed the effects of various variables such as age, body mass index, smoking, exercise and blood pressure on blood lipid level. The results obtained were as follows; 1. Average weekly alcohol intake was 129.0+/-167.4 gm and that of 30-39 age group was the highest as 149.3+/-170.4gm. 2. Levels of Cholesterol, Triglyceride and LDL-Cholesterol tended to increase with increasing age, but level of HDL-Cholesterol showed no significant relationship with age. 3. The positive linear regressions of alcohol intake on HDL-Cholesterol and Triglyceride were noted statistically significant. 4. Multiple regression analysis demonstrated that the effect of alcohol on HDL-Cholesterol and Triglyceride was statistically significant and the contribution rates were 5.0% and 0.8%, respectively. And, in the case of HDL-Cholesterol, the alcohol intake was the most significant independent variable.
