3.Prevalence and risk factors of cerebral white matter changes and silent infarcts on brain computed tomography scans among community-dwelling healthy adults: The PRESENT project

Cerebral white matter changes (WMCs) and silent brain infarcts (SBIs) are common radiologic
findings in neurologically asymptomatic elderly people, but are associated with an increased risk
of subsequent stroke. We investigated the prevalence and risk factors for these cerebral changes on
brain computed tomography (CT) in 480 community-dwelling healthy Korean adults without stroke
or dementia, who were recruited for an early health program. Cerebral WMCs were defined as the
presence of approximately 5 mm wide ill-defined and moderately hypodense lesions, and SBIs were
defined as the presence of >2 mm wide well-defined hypodense lesions. Of the 480 patients, 49 (10.2%)
had cerebral WMCs and SBIs findings on brain CT. The prevalence of WMCs and SBIs increased
with age: the prevalence was 2.4%, 9%, and 32% for subjects in their 50, 60s, and 70s, respectively.
In addition, hypertension, abdominal obesity, increased levels of homocysteine and high sensitivity
C-reactive protein were significantly associated with cerebral WMCs and SBIs. Our study suggests
that regular monitoring of risk factors is required to prevent cerebral WMCs and SBIs and decrease
the incidence of stroke and dementia in healthy individuals.

5.Postoperative Severe Hemorrhage Due to Disseminated Intravascular Coagulation: A case report.

Disseminated intravascular coagulation (DIC) is a pathological syndrome in which activation of coagulation cascade leads to fibrin clot formation, consumption of platelets and coagulation factors, and secondary fibrinolysis. We report a case of severe postoperative hemorrhagic diathesis due to DIC. A 59-year-old man was scheduled for reduction of tibia fracture and anatrophic nephrolithotomy of staghorn calculi. On the fifth postoperative day, second operation was performed for nephrectomy due to perirenal hematoma. Two days later, third operation was performed for hemostasis because of the continuous bleeding. Coagulation tests showed positive DIC profiles of thrombocytopenia, hypofibrinogenemia, increased fibrin degradation products, and prolonged prothrombin time and thrombin time. The patient recovered uneventfully and discharged on the 59th postoperative day.

6.Sparganosis within the Thigh.

Sparganosis is an infection that is caused by sparganum, the migrating plerocercoid larva of Spirometra spp. Manson first reported this condition in 1882. In Korea, Uemura reported the first case in 1917 in the muscle facia of a Korean farmer during the amputation of his leg. Since then, more than 100 cases have been reported. However, the number of unreported cases of sparganosis would be considerable. Human sparganosis is widely distributed in Japan, China, Taiwan, Korea, Philippine, Malaysia, India, Australia, Africa, France, England and USA. Most human cases are from Southeast Asia including Japan and Korea. Sparganosis commonly involves the subcutaneous tissue, muscles, fascia with occasional migration into the brain. We report 3 cases of sparganosis, a 42 year old man, a 55 year old man and a 41 year old woman, who all presented with a tender mass in the right thigh.

7.Comparison of Isoflurane and Propofol Anesthesia on Postoperative Nausea, Vomiting and Recovery after Tonsillectomy in Children.

BACKGROUND: The purpose of this study was to compare prospectively two different anesthetic techniques with isoflurane or propofol for postoperative nausea, vomiting and recovery after tonsillectomy in children. METHODS: Sixty children, ASA physical status I, were assigned randomly to one of two groups. In group I, anesthesia was induced with thiopental 5 mg/kg and maintained with isoflurane 1~1.5 vol%. In group P, anesthesia was induced with fentanyl 1 mcg/kg, propofol 2 mg/kg and maintained with propofol infusion 5~10 mg/kg/hr. Both group received vecuronium 0.1 mg/kg for tracheal intubation and were ventilated with 33% O2 in N2O. The time to extubation, time to eye opening, PACU time, incidence and numbers of postoperative nausea and vomiting, and degree of sedation were recorded as well as perioperative complications. RESULTS: There were no significant difference in the duration of anesthesia and PACU time between two groups. The time to extubation and eye opening of group P were significantly shorter than group I (p<0.05). The degree of sedation and incidence of postoperative nausea and vomiting of group P were significantly lower than group I (p<0.05). But the frequency of intraoperative bradycardia was significantly higher in group P than group I (p<0.05). CONCLUSIONS: Propofol-fentanyl anesthesia results in less nausea and vomiting during postoperative period and more rapid recovery compared to isoflurane anesthesia and may be recommended in children undergoing tonsillectomy and adenoidectomy.

9.A Case of Clear Cell Sarcoma of the Anterior Chest.

Clear cell sarcoma of tendon and aponeurosis is a rare malignant tumor. It occurs chiefly in young adults, predominates in women and is most common in the regions of the foot and ankle. We report a case of clear cell sarcoma of tendon and aponeurosis in s 22-year-old man. he pstient had had a asymptomatic, normal skin colored, relativerly hard, dome shsped nodule on the anterior chest for 6 months. Histopsthologic findings revealed uniform pattern composed of compact nests of round or fusiform cells which had clear cytoplasm and were surrounded by delicate framework of fibrocollagenous tissue, and the individual tumor cell had a fairly regular appearance of possessing round to avoid vesicular nucleus with prominent basophilic nucleolus. One year after surgical excision and post operative radiotherapy, there was no recurrence.
