1.IgG4-Related Sclerosing Sialadenitis: Report of Three Cases.

Chronic sclerosing sialadenitis, Mikulicz disease or Kuttner tumor has been recently recognized as a spectrum of IgG4-related sclerosing disease. IgG4-related disease is characterized by a high serum IgG4 level and tissue infiltration of IgG4-positive plasmacytes. We report three cases of chronic sclerosing sialadenitis with variably associated systemic involvement. All patients presented with a submandibular mass or swelling, and all the resected submandibular glands showed diffuse lymphocytic infiltration, lymphoid follicles, and septal fibrosis. Two of the specimens revealed numerous IgG-positive plasma cells, most of which were IgG4-positive on immunohistochemical staining. One of them was associated with dacryoadenitis and hypophysitis. The other patient had ureterorenal lesions. Immunohistochemical study was unavailable in remaining one case, but the histologic features along with elevated IgG level and associated pancreatitis supported the diagnosis. All patients received steroid therapy postoperatively and are doing well. Salivary gland involvement in IgG4-related fibrosclerosis should be recognized in systemic medical pathology.

2.A Case of Clear Cell Sarcoma of the Anterior Chest.

Clear cell sarcoma of tendon and aponeurosis is a rare malignant tumor. It occurs chiefly in young adults, predominates in women and is most common in the regions of the foot and ankle. We report a case of clear cell sarcoma of tendon and aponeurosis in s 22-year-old man. he pstient had had a asymptomatic, normal skin colored, relativerly hard, dome shsped nodule on the anterior chest for 6 months. Histopsthologic findings revealed uniform pattern composed of compact nests of round or fusiform cells which had clear cytoplasm and were surrounded by delicate framework of fibrocollagenous tissue, and the individual tumor cell had a fairly regular appearance of possessing round to avoid vesicular nucleus with prominent basophilic nucleolus. One year after surgical excision and post operative radiotherapy, there was no recurrence.

5.Bilateral Second Branchial Cleft Fistulas: A Case Report.

The branchiogenic anomalies are common congenital cervical anomalies. They usually arise from an incomplete obliteration of the branchial apparatus during fetal development. These anomalies may be confused with and mistaken for other potentially more serious lesions, such as cystic hygromas, teratomas, and lymphomas. The anomalies are typically discovered in the pediatric and adolescent population and develop unilaterally. Bilateral manifestations are very rare and have a familial tendency. In addition, anomalies such as intrauterine and postnasal growth retardation, premature aging and unusual faces may be related with bilateral occurrence. This is the first report in the Korean language literature of bilateral branchial fistulas.

6.Secondary Tongue Cancer in Patients with Chronic Graft-Versus-Host Disease(GVHD) after Bone Marrow Transplantation: Report of Two Cases.

We report two patients with chronic graft-versus-host disease (GVHD) after bone marrow transplantation (BMT) who developed cancers in the mobile tongue. The first patient, a 15-year old female who had undergone bone marrow transplantation for Fanconi's anemia five years ago, developed chronic GVHD on the oral cavity 1.5 years after the transplantation and have suffered from it ever since. Recently, she developed squamous cell carcinoma in the tongue. The second patient, a 50-year old male who had received the bone marrow transplantation due to severe aplastic anemia, developed chronic GVHD on the oral cavity 5 months later and have suffered from it thereafter. He developed squamous cell carcinoma of the tongue 15 years later after BMT. These cases imply that cancers including tongue neoplasms may develop in patients with GVHD after bone marrow transplantation. A thorough examination of head & neck and close follow-up are recommended to patients treated with bone marrow transplantation, particularly to those with GVHD.

10.In-vitro investigation of the mechanical friction properties of a computer-aided design and computer-aided manufacturing lingual bracket system under diverse tooth displacement condition

OBJECTIVE:
The purpose of this study was to compare the static (SFF) and kinetic frictional forces (KFF) of a computer-aided design and computer-aided manufacturing lingual bracket (CAD/CAM-LB) with those of conventional LB (Con-LB) and Con-LB with narrow bracket width (Con-LB-NBW) under 3 tooth displacement conditions.
METHODS:
The samples were divided into 9 groups according to combinations of 3 LB types (CAD/CAM-LB [Incognito], Con-LB [7th Generation, 7G], and Con-LB-NBW [STb]) with 3 displacement conditions (no displacement [control], maxillary right lateral incisor with 1-mm palatal displacement [MXLI-PD], and maxillary right canine with 1-mm gingival displacement [MXC-GD]; n = 6/group). While drawing a 0.016-inch copper or super-elastic nickel-titanium archwire with 0.5 mm/min for 5 minutes in a chamber maintained at 36.5℃, SFF and KFF were measured. The Kruskal-Wallis method with Bonferroni correction was performed.
RESULTS:
The Incognito group demonstrated the highest SFF, followed by the 7G and STb groups ([STb-control, STb-MXLI-PD, Stb-MXC-GD] < [7G-MXC-GD, 7G-MXLI-PD, 7G-control] < [Incognito-MXLI-PD, Incognito-control, Incognito-MXC-GD]; p < 0.001). However, there were no significant differences in SFF among the 3 displacement conditions within each bracket group. Within each displacement condition, the Incognito group demonstrated the highest KFF, followed by the 7G and STb groups ([STb-control, STb-MXLI-PD] < Stb-MXC-GD < 7G-MXLI-PD < [7G-control, 7G-MXC-GD] < [7G-MXC-GD, Incognito-MXLI-PD, Incognito-control] < [Incognito-control, Incognito-MXC-GD]; p < 0.001). MXC-GD exhibited higher KFFs than MXLI-PD in the same bracket group.
CONCLUSIONS
The slot design and ligation method of the CAD/CAM-LB system should be modified to reduce SFF and KFF during the leveling/alignment stage.
