5.Maternal and cord blood leptin levels in normal pregnancies comparing to pregnancy induced hypertension: Relation to birth weight.

PURPOSE: Leptin, a product of the ob gene, is a 16-KDa protein that is mainly expressed in the adipose tissue and involved in the regulation of body weight. Elevated levels of serum leptin is noted in pregnant woman and the placenta is the site of nonadipose tissue production of leptin. In this study, we investigated the effect of serum leptin concentrations on fetal growth to estimate the effect of leptin on fetal growth. METHOD: Leptin concentrations were measured in venous and arterial cord blood and maternal serum at birth using a specific radioimmunoassay employing human recombinant leptin(Human Leptin RIA kit; Linco research, St. Louis, Mo). Thirty two full term pregnant women(n=32) had no medical complications and delivered the healthy babies(male=14, female=18). RESULTS: Serum leptin levels were 1.51 - 19,36 ng/ml(mean 7.16, SD 3.76) in arterial cord blood and 1.59 - 16.18(mean 7.47, SD 4.08) in venous cord blood and there was no difference between arterial and venous cord blood. Serum concentrations in arterial and venous cord blood were positively correlated with birth weight(r=0.7181, 0.6970; p<0.0001). There was no correlation in maternal BMI, maternal serum leptin and cord blood leptin concentrations. CONCLUSION: These findings suggest that cord blood leptin independently contributes to fetal body weight regardless of maternal serum leptin and body weight.

6.Comparison of Post-Health Care of Workers with Noise Induced Hearing Loss between Medium Sized Industries and a Large Automobile Industry.

In order to compare post-health care between medium sized enterprises(50-300 employees) and a large enterprise(an automobile manufacture plant) and to evaluate factors related to post?health care performance of workers with noise induced hearing loss, this study was carried out. One hundred and eighty nine workers were surveyed to the questionnaire on general and occupational characteristics, receipt of the examination result, perception and attitudes toward special health examination, contentedness with post-health care performance on the examination result and reason not to have post-health care performance. The results were as follows: 1. The employee in large enterprise perform post-health care higher than medium sized enterprises and main control method was taking ear protective device. The 27.5% of workers with NJHL did not perform any post-health care. 2. Comparing with large enterprise, medium-sized enterprises had more interview with workers of NJHL and some enterprises performed more health education and more improvement of working environment. But there were a few medium-sized enterprises which did not provide ear protective device and had indifference manager. 3. The variables that significantly related to the post-health care of manager side was receipt of health examination result(p < 0.05), the post-health care of worker side were size of enterprise, age(p <0.05) and receipt of health examination result(p <0. 1).

7.Clinical observation of aseptic meningitis associated with mucocutaneous lymph node syndrome.

Clinical observation was carried out for 147 patients with mucocutaneous lymph node syndrome (MCLS) who were admitted to the Department of Pediatrics, Wonju Christian Hospital during the period from June, 1983 to June, 1992. A spinal tap was performed on 114 of these patients and the following results were obtained. 1) The incidence of aseptic meningitis in patients with MCLS was 52.6%. (60 cases of the 114 cases who had spinal taps (52.6%).) 2) The predominent age group for aseptic meningitis was 6 months to 1 year of age (35%). 3) For the majority, the value of glucose and protein in the CSF were within normal limit or only mildly elevated. 4) The neurologic manifestations associated with aseptic meningitis in patients with MCLS were irritability (78.3%), vomiting (25.0%), nuchal rigidity (11.7%), convulsion (5.0%) and facial nerve palsy (1.7%) in that order.

8.Monitoring of Tracheal CO2 Tension during High Frequency Jet Ventilation for Laryngomicrosurgery.

BACKGROUND: The monitoring of end-tidal CO2 tension (PETCO2) during high frequency jet ventilation (HFJV) has been unsatisfactory because of a small tidal volume and slow response time of CO2 analyser, although several authors have reported strategies of successful PETCO2 measurement during HFJV. The aim of this study was to assess the validity of tracheal CO2 tension (PtCO2) as a PaCO2 during HFJV. METHODS: We studied 24 patients undergoing laryngomicrosurgery during HFJV (rates: 100/min; I:E= 0.2; driving pressure: 0.25-0.35 MPa) through a 12 Fr. polyethylene injector placed 6-7 cm below the vocal cord. A gas sampling line was placed longitudinally against the injector and they were wrapped with aluminum foil. Continuous capnography was recorded during 20 minutes of HFJV. Every 5 minutes of HFJV, PtCO2 was obtained from the plateau value of CO2 wave after the stopping of JV and arterial blood gas analysis was done at 20 minutes of HFJV comparing PaCO2 to PtCO2. A Pearson's product moment correlation and regression analysis between PtCO2 and PaCO2 and the agreement between the two methods using Bland-Altman method were assessed. RESULTS: A regression analysis (R2=0.928) and a Pearson's product moment correlation (r=0.965, P<0.001) indicated a strong correlation of PtCO2 and PaCO2 during HFJV. The difference against a mean scatter diagram showed a relative good agreement between the two method (mean difference: 1.58 (SD 2.22) mmHg; limit of agreement: 2.86 and -6.02). CONCLUSIONS: PtCO2 obtained from a plateau of CO2 wave on capnography after interruption of HFJV can accurately reflect PaCO2 during HFJV in relative.
