1.Management and Surgical Treatment of Pressure Sore.

Decubitus means 'a lying down', so pressure sore is a more correct term because tissue necrosis and ulcers are caused by excessive and continuous pressure. Pressure sores are easily developed in patients with chronic illness such as unconsciousness, spinal cord injury, anemia, diabetes, or malaise. So these patients need special attention, and the prevention is most important. Nowadays infected pressure sores are very difficult to control due to highly resistant bacterias such as MRSA (methicillin resistant Staphylococcus aureus). Early-stage pressure sores can be healed with local conservative treatments or skin grafts, but advanced and deep pressure sores need flap surgery with good blood supply and enough tissue padding over bony prominence. In postoperative state. the recurrence of pressure sores is very common occurring in about half of patients, so nursing care should be focused to relieve pressure. We have to recover patients with pressure sore as early possible as in order to relieve the patients physically and psychologically.

3.Surgical treatment of facial paralysis by using static ancillary procedures.

There is no method of single surgical treatment for facial paralysis, because of its variable condition and variable patient's demand. We usually have been performed the free vascularized gracilis muscle transfer to achieve facial reanimation in cases of the complete facial paralysis. However. most of the patients who visit outpatient room are incomplete facial paralysis. In these cases, we performed the several auxiliary operations for the correction of a drooping eyebrow, paralyzed eyelids, paralysis of the lower lip, and for similar paralytic deformities. To correct a drooping eyebrow, we performed simple eyebrow lifting and upper blepharoplasty. Gold plate implantations and Kuhnt-Szymanowski's method were performed for the lagophthalmos and ectropions individually. To correct a paralyzed lower lip, tensor fascia lata graft was performed with the face lifting. The authors also discuss their operative methods and results of various surgical methods for treating facial paralysis.

6.Atypical Abdominal Pain.

Abdominal pain usually originates from the abdomen, but the lack of evidence of any abdominal disease suggests the pain originate from the thorax or have metabolic and neurogenic causes. Especially, neurogenic abdominal pain is recurrent and sustained causalgic pain, regardless of diet and without abdominal distension or rigidity, may be accompanied by signs of neurologic dysfunction. We have experienced a very rare case of abdominal pain arising from the intramedullary cavernous hemangioma of spinal cord. We report this case with a review of the literature.

8.Democidiosis in Rosacea Type.

The authors present seven cases of Democidiosis in rosacea type and the subject of human demodetic infestation has been reviewed. According to our experience, demodex folliculorum, although a normal inhabitant of the sebaceous glands in small numbers, when encouraged to multiply by the patients excessive use of facial cream and avoidance of soap, can produce democidiosis. The diagnosis was easily confirmed by characteristic clinical appearance and simple microscopy. The characteristie clinical appearances are indurated non-comedous papules and pustules with unique cherry pinkish red erythema. The response to treatment, consisting of daily soap and water cleansing of the face, followed by the local application of a suitable antiparasitic remedies is simple and gratifying. But the erythema was not respond to antiparasitic preparation.

It has been known that the interconnection between the gervous, endocrine and immune system are largely mediated through regulatory soluble factors such as neruopeptides, cytokines and hormones. Capsaicin, the pungent principle of hot peppers, is a neurotoxin that affects primary sensory neurons of the C and A-b type and depletes primary sensory neurons (polymodal nociceptors) of neuropeptides like tachykinin. In this study capsaicin was used to explore the possible role of the neruons on the expression of cellular and humoral immune responses and TNF-a prodcution. 10.Association of Herpes Zoster and Lymphosarcoma: Report of one Case.

Herpes zoster is generally believed to be caused by the activation of varicella-zoster virus present in the body since an original infection with varicella. According to the thesis, the virus remairis in a latent state in the cells of the sensory ganglia until immunity has waned sufficiently to pezmit multiplication of the virus and clinical infection. There are a number of reports that in patient with malignant disease, especially Jymphoma, a frequenry af association of zoster is greater and severity of its symptom is increased to be marked contrast to the benign uneventful course of zoster seen in healthy person. The author observed one case of zoster seen in patient with malignant disorder, and frequency of association of zoster in malignant disease and its etiological factors are reviewed in the literature.
