2.Acute Compartment Syndrome Induced by Rhabdomyolysis Due to Antipsychotic Drug Overuse

A 49-year-old male was found unconscious at his accommodation and visited the emergency room. He was on antipsychotic and antidepressant drugs (vortioxetine hydrobromide, mirtazapine, sertraline hydrochloride, quetiapine, and alprazolam) for schizophrenia and major depression. At the time of discovery there were signs of overdose of the drugs around the patient. A physical examination revealed, pain, pallor, and edema in the left buttocks and lateral thigh. Active ankle movements below the left ankle were not possible and sensations in the tibia and peroneal nerves were lost. The pressure in the buttock compartment was measured at 42 mmHg. Magnetic resonance imaging revealed edema and high intensity signals in the left hip muscles and surrounding soft tissue. An emergency fasciotomy was performed and partial restoration of the lower extremity sensation and muscle strength were achieved after 24 hours.
3.Lumbar Juxtafacet Cyst Treated with Direct Needle Aspiration Under the Guidance of Image Intensifier

A lumbar juxtafacet cyst is a rare disease that causes low back pain, radiculopathy and neurological claudication by compressing the nerve roots. A 34-year-old male complained of severe low back pain and radicular pain in the right lower extremity. Magnetic resonance images revealed a cyst at the lateral recess of the spinal canal between the L3-4 disc and posterior facet joint that extended to the L4 body level. Under the guidance of an image intensifier, needle aspiration of the cyst was performed, which extracted 1.5 ml of serous, yellowish colored fluid. After the aspiration, the symptoms subsided dramatically. The follow-up magnetic resonance images showed no recurrence of the cyst. To the best of the author’s knowledge, there are no reports of lumbar juxtafacet cyst treated with needle aspiration in Korea. This case is reported with a review of the relevant literature.

7.Efficient Experimental Design for Measuring Magnetic Susceptibility of Arbitrarily Shaped Materials by MRI

PURPOSE: The purpose of this study is to develop a simple method to measure magnetic susceptibility of arbitrarily shaped materials through MR imaging and numerical modeling. MATERIALS AND METHODS: Our 3D printed phantom consists of a lower compartment filled with a gel (gel part) and an upper compartment for placing a susceptibility object (object part). The B0 maps of the gel with and without the object were reconstructed from phase images obtained in a 3T MRI scanner. Then, their difference was compared with a numerically modeled B0 map based on the geometry of the object, obtained by a separate MRI scan of the object possibly immersed in an MR-visible liquid. The susceptibility of the object was determined by a least-squares fit. RESULTS: A total of 18 solid and liquid samples were tested, with measured susceptibility values in the range of −12.6 to 28.28 ppm. To confirm accuracy of the method, independently obtained reference values were compared with measured susceptibility when possible. The comparison revealed that our method can determine susceptibility within approximately 5%, likely limited by the object shape modeling error. CONCLUSION: The proposed gel-phantom-based susceptibility measurement may be used to effectively measure magnetic susceptibility of MR-compatible samples with an arbitrary shape, and can enable development of various MR engineering parts as well as test biological tissue specimens.

8.Two Cases of Uterine Tumors Resembling Ovarian Sex-cord Tumors: Rare Case of Uterine Tumor

Uterine tumors resembling ovarian sex-cord tumors (UTROSCT) are very rare tumors that occur mainly in the uterine fundus of women in reproductive age. These tumors can be classified into group 1 and group 2 by histological results. In group 1, epithelial-like differentiation is partially observed in the tumors. In group 2, sex-cord elements are predominant in uterine mural mass. We experienced UTROSCT group 1 in a 29-year-old woman who complained of severe abdominal pain that started one week after delivery and UTROSCT group 2 case in a 49-year-old woman who complained of dysfunctional uterine bleeding. We report two different types of UTROSCT cases that we experienced.

9.Passive Properties of the Urinary Bladder of Neonatal Rat Compared with Adult Rat.

PURPOSE: Bladder compliance increases during gestation, in which the role of collagen is considered important. However, it has been reported that the compliance paradoxically decreases without clear explanation after birth. Previous in vitro strip study showed that neonatal urinary bladder was stiffer than that of adult in rats. Herein, we extended our previous study. MATERIALS AND METHODS: Urinary bladders were removed from male Sprague-Dawley rats, 1 to 3-day old neonate and 12 week-old adult. Volume-pressure relationship was obtained using in vitro whole bladder study: in the consideration of anatomical difference in bladder wall thickness and zero pressure volume, all parameters were corrected as stress and stretch. Relative collagen content of the bladder was assessed and compared using image analysis after Masson?s Trichrome stainging in two groups. RESULTS: Volume-pressure analysis revealed that the patterns of increase of stress according to stretch were different between two groups; exponential coefficient of the slope of neonatal bladders was larger, which means neonatal bladder proved to be stiffer than that of adult. Collagen ratio to muscle in the bladder muscle layer was not different between adult and neonatal rats(p.>0.05), while adult rat bladder has more collagen deposition in mucosal layer(p<0.05). CONCLUSIONS: Our results from previous length-tension relationship and volume-pressure relationship demonstrated that neonatal bladder was stiffer than adult bladder, which was inexplainable with collagen contents. Other factors than collagen might be suggested in determination of passive properties in the development of urinary bladder.

10.Outcome of Chemotherapy with Pulmonary Tuberculosis Resistant to Isoniazid and Rifampin.

BACKGROUND: The treatment for multi-drug resistant tuberculosis(MDRTB) is encountered to be important clinically, but there are still a few reports about it all over the world. So, we evaluated the outcomes of only chemotherapy for the pulmonary MDRTB retrosptctively. METHOD: We reviewed the clinical courses of 63 patients with pulmonary disease due to M.tuberculosis resistant to rifampin and isoniazid who were under follow-up between March 1996 and June 1996 after hospitalization at our hospital between January 1993 and January 1996. We performed cohort retrospective study for all these patient's records. Their regimens were selected individually and preferably included four medications that they had not been given previously and to which the strain was fully susceptible. RESULTS: The 63 patients(mean age, 43.2 years) had previously received a median 5.1drugs. Fifty two(82.5%) patients responded to chemotherapy(as indicated by negative sputum cultures for at least three consecutive months); eleven patients(17.5%) had no response, as shown by continually positive cultures. In a univariate analysis, an unfavorable response was significantly associated with greater number of resistant drugs before the current courses of therapy(relative risk 21.5; 95 percent confidence interval, 1.2-3.0; p<0.05). The mean period of follow-up was seventeen months. There was no relapse subsequently among the patients with responses. There was no death related to tuberculosis. CONCLUSION: In this report from National Masan Tuberculosis Hospital in Korea, multi-drug resistant pulmonary tuberculosis responded relatively well to carefully selected regimens.
