1.Anxiety and depression of the Korean residents in China.

In order to survey the reality of anxiety and depression among the Koreans residing in china, a study was conducted between January and March of 1991, on the residents of Yun-Kil city, with subjects of 472 Koreans and 479 Chinese. The evaluation was based on the questionnaires, named Combined self-rating anxiety depression scale (CADS), distributed among the subjects. ANOVA and t-test were applied for data processing. The results were as follows: There was not significant difference in the mean of total scores between the two groups. The scores of Koreans were 29.70±7.03, while those of Chinese were 29.45±9.01. The score of the CADS above 50 (clinically significant level) was seen in 12 (2.54%) Koreans and 21 (4.38%) Chinese. The anxiety-depression scores relating to the items of indigestion and decreased appetite, sleep disturbance, apprehension, decreased libido were relatively high among the Koreans. The items appeared low in scores among the Koreans were faintness, fear, suicidal rumination, hopelessness, paresthesias. The highs among the Chinese were facial flushing, anxiousness, dissatisfaction, suicidal rumination. The items appeared low among the Chinese were fear, faintness, paresthesias, weight loss, suicidal rumination. In the compatison of evaluation by items between the two groups, the items placing the Koreans significantly higher over the Chinese are indigestion & decreased appetite, sleep disturbance, apprehension, decreased libldo. The Chinese marked significantly higher in facial flushing, anxiousness, dissastisfaction, suicidal rumination. Those in the case of female (p<0.01 respectively), less than twenty years old (p<0.01 respectively), dissatisfied with family relationship (p<0.01 respectively), with past history of psychiatric hospitalization (Koreans p<0.01, Chinese p<0.05), pessimistic toward future, present, past self image (p<0.01 respectively) had significantly higher scores in both groups. In religion, neither group showed significant difference. In religion, neither group showed significant difference. In marital status, the Koreans showed a higher degree of divorce and separation and the Chinese in singleness (p<0.01 respectively). The Korean were higher in illiteracy and the Chinese had more college education (p<0.01 respectively). In place of growth, The Koreans showed not much difference in the areas while more Chinese grew up un large cities (p<0.01). More Koreans lived in the dormitory while the Chinese were engaged more in self-cooking (p<0.01 respectively), In pocket money per mouth, more Koreans were less than 1 dollar while the Chinese were between 7 and 10 dollars (p<0.01 respectively). There were no significant difference between two groups about religion.

2.The Use of Hydroxychloroquine in Patients with Pemphigus Erythematosus.

Pemphigus erythematosus (PE) is a superficial type of pemphigus, which can be aggravated by sunlight (espicially UV light). Because of the known side effects of corticosteroids, we evaluated the efficacy of hydroxychloroquine as a corticosteroid-sparing agent and/or the effect of a single-drug regimen in two patients with PE with photosensitivity. We obtained a good therapeutic response with hydroxychloroquine in these two patients with PE. This drug could be used in selected patients with pemphigus who are prednisolone/ immunosuppressive-resistant or who have certain degrees of photosensitivity.

4.Visual Rehabilitation of Optic Atrophy Patients with Low Vision Aids.

In oder to evaluate the efficacy of low vision aids in patients with optic atrophy, we analyzed sex, age distribution, preexisting conditions, visual acuities before and after low vision aids use. The sorts and magnifications of prescribed low vision aids were studied in patients with optic nerve atrophy whose visual acuities were not corrected with conventional glasses or contact lens. Our results indicate that distant and near visual acuities with low vision aids were significantly improved in patients with optic atrophy(p<0.01, paired ttest). We think that low vision aids are required to rehabilitate the visually impaired patients caused by optic atrophy because of high probability of visual rehabilitation.

5.Outcomes of Palivizumab Prophylaxis for Respiratory Syncytial Virus Infection in Preterm Children with Bronchopulmonary Dysplasia at a Single Hospital in Korea from 2005 to 2009.

This study was performed to evaluate the utilization and outcomes of palivizumab in high risk children born prematurely with chronic lung disease (CLD). A retrospective review of 128 patients was conducted from September 2004 to March 2009 at the Ajou University Hospital. All patients were diagnosed with CLD, were born at < or =35 weeks of gestation, were <2 yr old at the onset of respiratory syncytial virus (RSV) season, and had received medical therapy within six months prior to the RSV season. Fifty-three patients did not receive palivizumab prophylaxis and 75 patients received at least one dose of palivizumab. There were no statistically significant differences between the patients with and without palivizumab prophylaxis with regard to demographic characteristics and risk factors for RSV infection. There were no systemic adverse responses. Compliance with the course of prophylaxis was 92.2%. Hospitalization associated with RSV occurred in 12 cases (22.6%) in the group without prophylaxis and in three cases (4.0%) with prophylaxis. Palivizumab prophylaxis significantly reduced the frequency of RSV-related hospitalization in preterm children with CLD. This is the first retrospective review of palivizumab prophylaxis in Korea. Palivizumab is effective and well tolerated in high risk prematurely born children.

6.Detection of Segmental Branch Renal Artery Stenosis by Doppler US: A Case Report.

In stenosis of a segmental branch or among multiple renal arteries, Doppler sampling of intrarenal arteries in the upper, mid and lower poles demonstrates strikingly different waveform patterns that might otherwise be overlooked. We report a case of segmental branch renal artery stenosis in which a pulsus parvus et tardus waveform was observed in a segmental branch of a renal artery. In this case, systematic analysis of Doppler waveforms of intrarenal arteries at more than three different locations facilitated a rapid and confident diagnosis of seg-mental branch renal artery stenosis.

8.Restless Legs Syndrome in Psychiatric Outpatients with Psychotropic Agents.

OBJECTIVES: The purpose of this study was to investigate the clinical characteristics and incidence of restless legs syndrome (RLS) in outpatients with psychiatric illness. METHODS: 146 adult patients (male 52, female 94) were selected from those who visited a psychiatric outpatient clinic. RLS was evaluated through an interview method using diagnostic criteria and a severity rating scale for RLS developed by the International RLS Study Group (IRLSSG). The authors also applied Beck's Depression Inventory (BDI), Beck's Anxiety Inventory (BAI), the Pittsburgh Sleep Quality Index, Barnes Akathisia Rating Scale and neurological examination. RESULTS: Thirty-four (23.3%) among 146 subjects were diagnosed as having RLS. Fifteen (44.1%) among those 34 RLS patients reported to have developed their RLS after starting their psychotropic agents and only 4 patients (11.7%) had currently been receiving treatment for their RLS. The RLS group showed higher BDI and BAI scores (p<0.01) and lower sleep quality (p<0.01) compared to those of non-RLS. The RLS group also showed a significantly higher co-morbidity of parasomnias (chi-square =8.5, p<0.01) and peripheral neuropathy (chi-square =5.2, p<0.05). CONCLUSION: The results from this study suggest that a substantial proportion of outpatients with psychiatric illness are suffering RLS. Our data suggest that clinicians should pay attention to the possible presence of RLS among their patients who are taking psychotropic agents.

9.A Clinical Report of 111 Internal Arteriovenous Fistulas in 111 Patient for 5 Years.

PURPOSE: Hemodialysis remains the most important support for patients with end stage renal disease, and vascular access is an essential component for their life. Since 1966, internal arteriovenous fistula (AVF) has been used widely today. If vessels were not available for AVF, the alternative would be used such as prosthetic graft. But in 1997, the National Kidney Foundation-Dialysis Outcome and Quality Initiative (DOQI) recommended increased use of native arteriovenous fistula to improve overall patency and curtail angioaccess costs. This retrospective study is to review our experience and to evaluate the overall patency rate and the influencing factors on the patency of the AVF. METHOD: From March 1995 through October 2000, 111 fistulas were created of 111 patients in Eulji university hospital. Among them, 106 cases were able to follow up survey. The statistical analysis used by SPSS package. RESULT: The male versus female ratio was 1.22:1 and the age distribution was occurred on from 3rd decade to 9th decade. the common causes of renal failure was hypertension, glomerulonephritis and diabetes (62.1%). the autogenous graft fistulas were performed in 101 cases (wrist/antecubital fossa. 101/3), Goretex graft fistula were 7 cases. The early graft failures were 12 cases (11.4%) and the causes was thrombosis or stricture, and immaturation, psudoaneurysm, venous hypertension in order of frequency. At 12, 24, 36 months, the assisted patency rates of AVF were 80.4, 76.5, 71.3%, respectively. CONCLUSION: We could get higher patency rate of AVF due to liberal use of native veins and aggressive intervention of the failing AVF as recommendation of DOQI.
