1.A Study of Phototoxicity Using the Candida Albicans Test and the Photohemolysis Test.

Background: If phototoxic:ity can be predetermined in vitro, the information will aid in prognosticating whether or not caimpounds have a photosensitizing pczrlial. OBJECTIVE: For the evaluation of the phototoxic potentials of reral drugs, we performed the Candida albicans test and the photohemolysis test. METHODS: The Candida altiicans test is based on growth inhibtion of Candida albicans after application of the drug and ultraviolet light A( UVA ) irradiatior and the photohemolysis test is based on hemolysis of red blood cells caused by irradiation ir the presence of photosensitizing compounds. In the Candida albicans test, clear zones around the drug patches were evaluated, which means positive results for the phototoxic potential of the drugs. In the photohemolysis test, the amounts of hemolysis were evaluated by measuring the relar absorbance at 540nm using a spectrophotometer. RESULTS: In the Candida albicans test, ibuprofen, naldix acid, chlorpromazine and thiodiphenylamine showed positive results, whereas others did not the photohemolysis test, griseofulvin, ibuprofen and nalicdixic acid showed increased amounts hemolysis at UVA and ultraviolet light B(UVB) irracliation, and chlorpromazine, thiodiphenylaiair, chlorothiazide and piroxicam showed increased amounts of hemolysis at UVA irradiation only. CONCLUSION: The results showed that both methods were goodness screening tests for demonstrating the phototoxicity of therapeutic drugs.

2.Radiological evaluation of cortical rim sign of the kidney

The rim sign in renovascular compromise may be defined as a thin nephrographic rim outlinning a kidney with another wise faint nephrogram. Renal infarction with gelform and blood clots was induced experimentally in 5 dogsand high dose IVP and CT were performed to evaluate renal cortical rim sign after renal infarction. One expireddue to anesthetic technical failure, and one was embolized partially. So we could be obtain the result on 3 dogs.The reults were as follows; 1. The high dose IVP films show cortical rim sign on 24 hrs and 72 hrs films in twodogs. The CT films show rim sign in 3 dogs. 2. The rim sign may be visualized, if the renal artery is occuludedenough to drop perfusion presssure below the authoregulatory range. 3. The high dose IVP and early nephrogramphase are necessary for the rim sign.

3.Fine Needle Aspiration Biopsy of the Lung in Children with Diffuse Pulmonary Lesions Suggesting Pneurnocystis carinii Pneumonia.

PURPOSE: The purpose of this study was to determine the following:the safety of fine needle aspiration biopsy in immunocompromized children with radiographic features of Pneumocystis car/nil pneumonia, its diagnostic rate in those groups and the appropriate radiographic stage for fine needle aspiration biopsy to prove the etiologic agent. METHODS AND MATERIALS: We retrospectively reviewed the patient records of 16 children with immune compromizing diseases who had undergone fine needle aspiration biopsy of the lung. They showed the infectious sign of the lung along with the radiographic pattern of diffuse pulmonary disease, suggesting Pneumocystis carlnil pneumonia. All patients had underlying lymphoreticular malignancies including 14 acute lymphocytic leukemia and 2 non Hodgkin's lymphoma. According to the radiographic pattern of biopsy site, parenchymal disease was categorized as fine reticulonodular density(n=4), ground-glass opacity(n=9) and compact consolidation(n=3). We assessed the diagnostic rate of Pneumocystis carinii pneumonia and complications in each of the three groups. RESULTS: A diagnosis of Pneumocystis carinii pneumonia was established by fine needle aspiration biopsy in 9 patients(56%) including 2 of 4 patients with fine reticulonodular density, 4 of 9 patients with ground-glass opacity, and all 3 patients with compact consolidation. Four patients(25%) developed pneumothorax, and three of them required tube insertion. There was no patient who developed hemoptysis. CONCLUSION: Fine needle aspiration biopsy is a safe and easy method that can yield Pneumocystis carinii organism at a relatively high rate in immunocompromized children with diffuse pulmonary lesions suopicions of Pneumocystis carinii pneumonia. We recommend performing fine needle aspiration biopsy regardlesss of radiographic patterns when Pneumocystis carinii pneumonia is suggested.

4.Treatment of Tracheobronchial Stenosis with a Self-Expandable Metallic Stents.

PURPOSE: We analysed the role of modified Gianturco self-expandable stents in the treatment of tracheobronchial stenosis in 13 patients. MATERIALS AND METHODS: We inserted modified Gianturco self-expandable stents under the fluoroscopic and bronchoscopic guidance. There were stenosis in the trachea(n--2), the right main bronchus(n=2), and the left main bronchus(n=9). The causes of the stenosis were endobronchial tuberculosis(n=10), intubation granuloma (n=l), restenosis after surgical reconstruction(n=2). RESULTS: Dyspnea or wheezing was improved within 1 or 2 days following the procedure. There were 32% and 22% respective increase in average FEV1 and FVC. Lung perfusion scan showed 9.6% increase of perfusion in the involved lung. No complications related to the procedure were encountered. During follow-up period of up to 31 months, 2 patients showed tracheal or bronchial restenosis, at 3 and 6 months, retrospectively. There was a distal migration of the stents in one case. CONCLUSION: During the follow up period after stent insertion, improvement of the obstructive changes and dyspnea persisted in 10 out of 13 patients. The modified Gianturco self-expandable metallic stents may be a good choice for the treatment of tracheobronchial stenosis, either as a primary treatment, or when the reconstruction failed.

6.A Case of Myxoid Malignant Fibrous Histiocytoma.

Malignant fibrous histiocytoma is the most common soft tissue sarcoma in adult life and shows a mixed proliferation of both fibroblsstic & histiocytic cells. We report a case of myxoid malignant fibrous histiocytoma involving right lower leg in a 68-year-old male patient, who presented with a childs palm-sized nodular mass of 3 years duration. Histoapthologically, the tumor had varying proportion of myxoid cellular components. The myxoid areas showed spindle and pleomorphic cells arranged in a vague storiform pattern, abrupt transition between a solidly cellular area and a myxoid area, and lipoblast-like cells. The cellular areas showed spindle cells arranged in a storiform pattern characteritic of the typical pleomorphic malignant fibrous histiocytorna.

7.Two Cases of Dyschromatosis Symmetrica Hereditaria.

We report two cases of dyschromatosis symmetrica hereditaria. One is a 12-year- old female and her family had five individuals with similar pigmentary disorder through three generation. The other is a 22-yesr-old female who had no family history of similar disease. Fontana-Masson stain revealed increased and decreased melanin granules in the basal cell layers in the pigmented and depigmented lesions respectively.

8.A Case of Virilizing Adrenal Cortical Carcinoma.

Adrenal cortical carcinoma is a rare disease both in adults and in children. Most of these tumors are functional, especially in children, producing endocrine syndromes such as virilization, Cushing syndrome, hyperaldosteronism or feminization. We experienced a case of adrenal cortical carcinoma in a 7yr old boy who showed features of virilization such as rapid growth rate, penile enlargement, and pubic hair. This case was diagnosed with typical hormonal findings and abdominal MRI and confirmed by pathologic findings. He was successfully treated by total left adrenalectomy and has been followed up without problem over eight months.

10.A Case of Targetoid Hemosiderotic Hemangioma.

Targetoid hemosiderotic hemangioma is a benign vascular tumor with a peculiar clinical presentation and histologic features sometimes difficult to differentiate from the patch stage of Kaposi's sarcoma, progressive lymphangioma, eosinophilic hemangioma, retiform hemangioendothelioma. We report a case of targetoid hemosiderotic hemangioma on upper back and right upper arm of a 28-year-old male. He has a 5mm × 5mm sized slightly raised violaceous papule surrounded by an ecchymotic halo on upper back and right upper arm. We took a biopsy of the lesion and identified it as targetoid hemosiderotic hemangioma. This is a case of targetoid hemosiderotic hemangioma with typical clinical and histopathological findings.
