1.Epidemiologic Study on Psoriasis.

This study was carried out to reveal the epidemiologic characteristics of psoriasis in Korea. Seventeen university hospitals were engaged in this study, and the results were as follows : 1. The number of psoriasis patients during the 10 years from January 1977 to March 1987 was 1.05% of all the outpatients; the male to female-ratio was 1.08:1. 2. In the prospective study, the number of psoriasis patients was 2.3% of all the outpatients in 1987, 2.8% of all the outpatients in 1988 and the male to female ratio was 1.26:1. 3. The most prevalent age of onset was 20-29 years ; 81.1% were living in urban areas; and a family history was obtainable in 19.8% with parents holding the highest rank of 7.8%. 4. The most frequent duration of disease activity was under 5 years in 63.3%; the most common age for the worst condition was 20-29 yrs. The most common area size of skin involvement at the peak of the worst condition was less than 10%. 5. The most common initiating skin lesion began on the scalp in 20.2%, on the nail in 17.2%, pruritus in 63.0% with joint symptoms in 10.8%, and Koebner phenomena in 50.6%. 6. Emotional stress was the most common aggravating factor in 34.4%. The method of treatment that attained the best result was the combination therapy of oral medication and a topical agent. The best treatment modality was said to be the application of ointment with p.o.medication by as many as 28.4% of patients who took the leading rank. Photo-chemotherapy, effective with few complications, was not used much as it was not well-known in Korea until after 1980. Most people had an accurate knowledge of psoriasis.

2.Adherence Syndrome After Weakening Procedure of Inferior Oblique Muscle.

Adherence syndrome may be due to fibrofatty proliferation creating adherence between infeiror orbital tissue, sclera, or inferior rectus muscle capule and inflammatory reaction initiated by the rupture of the Tenon's capsule and invasion of the extraconal fat. We report 4 cases of adnerence syndrome after weakening procedure of inferior oblique muscle in the cases of overacting inferior oblique muscle.

3.Surgieal Result of Intermittent Exotropia: Comparison of Sensory Anomaly.

In this study, among intermittent exotropia cases, 38 cases indicating sensory anomaly of diplopia and 27 cases indicating sensory anomaly of suppression, and tried to compare the results of operation. Cases of diplopia ranged from 3 years of age to 21 years of age averaging 4.5 years of age, and the follow up period after operation ranged from 2 months to 18 months averaging 6.5 months. The success rate of operation in cases of diplopia was 92%(35/38), and that in cases of suppression was 74%(20/27). Consequently, among intermittent exotropia patiens indicating sensory anomaly the case of diplopia can be said to be higher in the success rate of operation than that of suppression.

4.Congenital Systemic Cytomegalic Inclusion Disease: An Autopsy Case Report.

A case of systemic cytomegalovirus infection in a newborn baby is reported. This female baby was born after 36 weeks gestation to a 30 year-old mother who had no prenatal problem except for breech presentation and placenta previa for which Caesarian section was given. The mother has two healthy children. The clinical course of this baby was characterized by repeat spells of apnea, cyanosis and edema, that progressed to deepening jaundice and generalized petechiae. She died 58 hours after birth. A characteristic linear calcification along the ventricular wall of the brain enabled us to suggest congenital nature of cytomegalic inclusion disease in the patient. Postmortem exmaination showed numeorous cystomegalic inclusions in kidneys, pancreas, liver and thyroid. There was subependymal and periaqueductal calcification together with hydrocepalus and cerebellar hypoplasia.

5.A Case of 18-Trisomy Mosaicism with Parental Mosaicism.

A case of 18-Trisomy mosaicism in a male newborn infant who was born to a mother of 27 years old at Cheil Hospital was presented. This baby had grossly multiple anomalies that were characterized by prominatn occiput, microhpthalmia, palpebral fissure, micrognathia, high arched palate, low set ears, undescended testes, limitation of abduction of hip joint, syndactylism of finger, and overlapping of finger. We have been confirmedby chromosome culture and his mother chromosomal analysis revealed sex chromosomal mosaicism (45xo/46xxx). This is the first case report of 18-Trisomy mosaicism with parental mosaicism in Korea. A brief review of related literature is also pressented.

6.Analysis of Trends of 3 Major Benign Prostatic Hyperplasia Surgery in the Last 5 Years

PURPOSE:
The goal of this study is to analyze the trends in surgical management of Benign prostatic hyperplasia (BPH) in Korea during the last 5 years from 2014 to 2018.
MATERIALS AND METHODS:
We retrieved the medical statistics associated with the Healthcare Big Data Opening System available online. We analyzed the number of cases after 2014 for transurethral resection of prostate (TURP), photoselective vaporization of the prostate (PVP), and holmium laser enucleation of the prostate (HoLEP), respectively. We then analyzed and charted the cases according to the number of patients hospitalized or treated as outpatients, depending on age group, type of medical institutions, and the location of medical institutions.
RESULTS:
The number of patients with BPH has increased steadily. The number of TURP and HoLEP procedures steadily increased, while the number of PVP interventions decreased dramatically. The number of HoLEP cases increased by 22% from 2014 to 2018, which is the fastest rate among the three surgeries. In addition, the number of patients aged 75 years or older as well as the proportion of inpatient surgeries compared with outpatient treatments has increased. The number of cases undergoing TURP increased rapidly in general hospital and those treated with HoLEP increased in the general and tertiary referral hospitals.
CONCLUSIONS
Interventions using TURP and HoLEP has increased, and the age of patients undergoing surgery has increased gradually. Cases treated with TURP and HoLEP in general and tertiary referral hospitals showed an increasing trend from the metropolitan area to the province.

8.Loss of Heterozygosity at 1p, 7q, 17p, and 22q in Meningiomas.

OBJECTIVE: Allelic losses or loss of heterozygosity (LOH) at many chromosomal loci have been found in the cells of meningiomas. The objective of this study was to evaluate LOH at several loci of different chromosomes (1p32, 17p13, 7q21, 7q31, and 22q13) in different grades of meningiomas. METHODS: Forty surgical specimens were obtained and classified as benign, atypical, and anaplastic meningiomas. After DNA extraction, ten polymorphic microsatellite markers were used to detect LOH. Medical and surgical records, as well as pathologic findings, were reviewed retrospectively. RESULTS: LOH at 1p32 was detected in 24%, 60%, and 60% in benign, atypical, and anaplastic meningiomas, respectively. Whereas LOH at 7q21 was found in only one atypical meningioma. LOH at 7q31 was found in one benign meningioma and one atypical meningioma. LOH at 17p13 was detected in 4%, 40%, and 80% in benign, atypical, and anaplastic meningiomas, respectively. LOH at 22q13 was seen in 48%, 60%, and 60% in benign, atypical, and anaplastic meningiomas, respectively. LOH results at 1p32 and 17p13 showed statistically significant differences between benign and non-benign meningiomas. CONCLUSION: LOH at 1p32 and 17p13 showed a strong correlation with tumor progression. On the other hand, LOH at 7q21 and 7q31 may not contribute to the development of the meningiomas.

9.Surveillance of Stool Samples for the Presence of Enterobacter sakazakii among Korean People.

PURPOSE: Enterobacter sakazakii (E. sakazakii) infections are an important cause of life-threatening meningitis, septicemia, and necrotizing enterocolitis in infants. Dried infant formula milk is an important vehicle for E. sakazakii infection. E. sakazakii was isolated in Korea from dried infant formula milk. Although E. sakazakii infection of infants may occur in Korea, its prevalence has not yet been documented. Therefore, we determined the prevalence of E. sakazakii and documented symptoms. MATERIALS AND METHODS: Between March and October 2006, 1,146 stool samples were collected from patients at Uijeongbu St. Mary's Hospital. Each fecal swab was dissolved in 10mL of buffered peptone solution, and enriched culture was streaked onto Druggan-Forsythe-Iversen (DFI) agar. Presumptive E. sakazakii colonies that exhibited a blue-green color during culture on DFI medium were selected. The identity of colonies that developed yellow pigment during culture on TSA was determined using the Vitek system and PCR. RESULTS: We isolated 4 E. sakazakii strains whose 16S rRNA sequence alignments had a similarity of 99% with those of 3 E. sakazakii ATCC strains. CONCLUSION: This is the first report on isolation of E. sakazakii from stool samples and to document the symptoms of Korean patients.

10.Nutrient Intake and Related Factors in Middle-Aged Urban Adults.

Working towards the compression of morbidity, we attempted to find the way to contribute to desirable aging and/or healthy old age. By looking into the factors affecting nutrient intake in middle-aged adults, we sought to find ways to improve their health status, Two hundred and fifty middle-aged adults from 40 to 60 years of age residing in Seoul and its vicinity were recruited for the survey. Subjects were interviewed for general characteristics, dietary habits lifestyle, status and food intake by a trained interviewer. Two -day food record was included in addition to the 1 -day 24 hr recall. Date were analyzed for the relationship among nutrient intake, health status, socio-economic status dietary habits and lifestyle using a statistical program (SAS) Although the mean energy intake of the subjects did not reach the recommenced dietary allowances(RDA), mean protein intake was well above the RAD for both sex groups(111.2% and 129.1% for men and women, respectively) The caloric contribution of fat was 15 to 20 % of total energy intake among different groups. Riboflavin was the most deficient nutrient and its mean intake reached only 71.0% of RDA for mean This seemed to be due to the lower intake of milk and dairy products among middle-aged Koreans who are more accustomed to traditional meals. Income and exercise habit were the most important factors influencing the nutrient intake of middle-age adults. Also, BMI, smoking habit, level of education regularity of meal, and hours of sleeping influenced the nutrient intake, In conclusion, middle-age adults appeared to have similar problems with the elderly in terms of nutrient intake and this age group should to included when considering nutritionally vulnerable groups in addition to the elderly and young children. The improvement in the nutrition status of middle-age adults is even more important because that could lead into the better health the elderly.
