1.Lesser Omental Internal Hernia with Strangulation: A Case Report.

Internal hernia is an unusual cause of the intestinal obstruction. The advances of CT make the diagnosis more easier than in the past. We report one case of internal hernia with herniation of the ileum into the lesser omenturn. The diagnosis could be made when abdominal radiographs showed fixed clustering of the small bowel loops in upper abdomen medial to the stomach. CT and ultrasound showed characteristic interposition of the ileum between the left hepatic lobe and the stomach.

2.DNCB Therapy in a Large Condyloma Acuminatum Resembling Buschke-Loewenstein Tumor.

We report herein a 53-year-old male patient with a large condyloma acuminaturn of 16 months duration on his penis. Clinically, an approximat;ely 3.5 X 2.0 Cm sized, asyrnptomatic, verrucous-surfaced, papilloma was present on his prepuce and glans. Elistological examination disclosed hyperkeratosis, marked acanthosis, papillomatosis, vacuolated upper malphigian cells, dilated capillaries, and mononuclear infiltrntes in the upper dermis. Trial of DNCB immunotherapy showed a dramatic effect on tumor regression.

5.Tuberculosis Cutis Orificialis.

We report herein a case of tuberculosis cutis orificialis in a 50-year-old man. He suffered from well demarcated, 2x5cm sized, slightly tender, pus discharging, perianal ulcer with bluish edges for 3 months. The biopsy specimen from the ulcer revealed tuberculoid structure with Langhans giant cells in the deep dermis and a few acid-fast bacilli. AFB cultures from the sputum, pus, and tissue were successful. Intradermal test with PPD showed positive result. Chest roentgenographic findings disclosed advanced pulmonary tuberculosis. Almost complete healing of the perianal ulcers occurred following 2 months of therapy using antituberculosis drugs.

6.The Effects of Nerve Growth Factor on Satellite Cell of the Dorsal Rott Ganglia in Streptozotocin-induced Diabetic Rats.

Diabetic neuropathy is an axonal degenerative disease characterized by progressive axonal atrophy and reduced axonal transport. We were interested in the potential neuroprotective effects of nerve growth factor against diabetic neuropathies. To this aim we studied the effect of nerve growth factor on satellite cells, which might play a trophic role toward the related neuron, of the dorsal root ganglion in the streptozotocin-induced diabetic rats by electron in microscope . Diabetes was induced in rats by the streptozotocin. And recombinant human NGF was administrated everyday for 10 consecutive weeks. The results obtained are as follows : 1.In the diabetic induced group, the satellite cells revealed irregular nuclei.The neuron-satellite cell interface was more irregular and plicated than that of control. Large vacuoles and dense bodies were observed and no defects were in the ribosomes and rough endoplasmic reticulum. In the vacuoles, medium electron dense, fiber -like materials were occasionally observed. 2. In the experimental group of diabetic rats treated with NGF for 10 weeks, nucleus was round and the neuron-satellite interface was more regular. Vacuoles and dense bodies were less seen than diabetic rats. In the cytoplasm, many microtubules were observed. In these studies, we considered that streptozotocin induces changes of the satellite cell structure and NGF might improve cellular changes of the satellite cell exposed with streptozotocin.

7.Spontaneous Testicular Hemorrhagic Necrosis Masquerading as a Testis Tumor .

Spontaneous testicular hemorrhagic necrosis is a rare disease usually associated with testicular torsion. Partially involved and suspicious testis tumor cases are also defined from orchiectomy specimens. Herein, a spontaneous hemorrhagic necrosis, without any testicular torsion, but with involvement of the whole testicle and epididymis, is reported. A 21 year old patient, who presented with a painless left testicular enlargement of several days duration was believed, based on physical examination, ultrasonography and elevation of serum LDH, to have a testicular tumor. Diagnosis was made only after radical orchiectomy and histopathological examination.

8.CT of Acute Pyelonephritis in Children: Comparison with Tc-99m DMSA Scintigraphy.

PURPOSE: The purpose of the study was to compare CT with scintigraphy in the detection of parenchymal lesionsof acute pyelonephritis in children, and to assess the diagnostic value of CT. MATERIALS AND METHODS: This studyinvolved 32 children with acute pyelonephritis ; their ages ranged from 1 month to 10 years. Renal CT , TC-99mDMSA planar and SPECT images, and medical records were retrospectively reviewed. We evaluated the number, size,shape, density, and location of pyelonephritic lesions, as seen on CT and scintigraphic images. RESULTS: In 43involved kidneys, 193 parenchymal lesions of acute pyelonephritis were identified. The results of CT were abnormalin 42 kidneys (98%), and those of scintigraphy, in 39(91%). CT showed single or multiple hypoenhancing parenchymallesions ; these were streaky(n=151), wedge-shaped (n=34), or oval (n=8), and ranged from about 3-30mm in maximumdiameter. Abscess (n=5), renal fascial thickening (n=6) and thickening of the bridging septae (n=7) wereassociated. Scintigraphic findings were diffuse or localized area of varying degrees of diminished corticalactivity, and these were more precisely identified on SPECT than on planar images. For the detection of 55 of 193pyelonephritic lesions, CT was more sensitive than scintigraphy ; 29 of the 55 lesions were less than 5 mm indiameter. CONCLUSION: For the detection of pyelonephritic lesions, particularly smaller ones, and for theevaluation of complications such as abscess formation, CT is more sensitive than Tc-99m DMSA scintigraphy. Weconclude that in children with subtle scintigraphic findings who are in serious clinical condition or in whomcomplications are suspected, CT is a useful tool for assessing a therapeautic plan and the prognosis of acutepyelonephritis.
