2.Clinical study of pigmemted Nevi on the palms,soles,and genitalia in Korean.

BACKGROUND: There are reviews with regard to the treatment of pigmented nevi present on the palms, soles, and genitalia. One advocated that pigmented nevi on thesa locations should be prophylactically removed to avoid their tuture development into malignant melanoma, while the other believes that this is not practicable. OBJECTIVE: Our purpose was to determine the incidence rates and clinical characteristics and to discuss the management of pigrnented nevi, developed on these areas of the Korean men. METHOD: We examined 1,537 Korean men, between 19 and 21 rears old, to evaluate the pigmented nevi on their palms, soles, and genitalia. RESULTS: Atleast 1 pigmented nevus appeared in 314(20.4%) men on the palms, 189(12.3%) on the soles, and 117(7.6%) on the genitalia. The sizes(mean diameter) of nevi on genitalia, soles, and palms were 4.5mm, 3.1mm, and 2.3mm respectively. The color of the nevi was darkest on the penile shaft(4.3 point) and then glans penis(3.9 points), soles(3.2 points), palms(2.5 points) in decreasing orders of the points. Most of the nevi was consistent with type 1 by Levers clinical classification and their incidence were 357 on palms, 211 on soles, and 128 on genitalia. Among the 21 nevi of type 2, 13 nevi appeared on the soles, and noine of type 3 and type 5 were found. The distribution of the nevi on the palms was of a rather uniform and scattered pattern, whereas, on the soles, they were distinctly concentrated in the midfoot. Tkie nevi on the glans penis appeared larger and more irregular in outline than those on the penile shaft. CONCLUSION: The plantar nevi were larger, darker, and had greater incidence of elevated type compared with the palmar nevi. From this observation, particular attenticn should be paid to the presence of pigmented nevi on the soles.

3.Two Cases of Thyroglossal Duct Fistula.

Thyroglossal duct cysts result from failure to obliterate the embryonic throglossal duct. When these cysts drain through the skin, they produce thyroglossal duct fistulas, which re often infected. We report two cases of thyroglossal duct fistula in an 8-year-old boaid a 41-year-old man who had a infected cystic mass and sirius on the anterior midline neck, respeticely. The cyst which is lined by a columnar epithelium was identified microscopically in a surgically n se ted specimen.

7.The Effects of Doxapram on the Pulmonary Function during Total Intravenous Anesthesia with Propofol According to Nalbuphine Pretreatment.

BACKGROUND: Total intravenous anesthesia with propofol can cause respiratory depression and apnea especially during induction of anesthesia. To study the possibility of reversal of respiratory depression during anesthesia with propofol, pretreated with nabuphine or not, the respiratory effects of doxapram to spontaneously ventilating patients were investigated. METHODS: Patients were divided into 4 groups - saline-propofol-saline group (SPS), saline-propofol- doxapram group (SPD), nalbuphine-propofol-saline group (NPS), and nalbuphine-propofol-doxapram group (NPD). After saline or nalbuphine pretreatment, anesthesia was induced with propofol and then doxapram or saline was intravenously injected. Apneic time interval, blood pressure, heart rate, respiratory rate, minute ventilation, end tidal CO2 partial pressure and oxygen saturation were measured in every minutes during induction of anesthesia. Percent changes of each values were compared. RESULTS: There is no differences in apneic time intervals in each groups. The percent change of first minute ventilation in SPD group after doxapram injection unchanged significantly compared with those depressions of SPS, NPS and NPD group (p<0.05). Respiratory rates increased in SPD and SPS groups after laryngeal mask insertion. There is no differences in minute ventilation, respiratory rate and end-tidal CO2 concentration between nalbuphine pretreated groups regardless of doxapram injection. CONCLUSIONS: Doxapram has effect in increasing minute ventilation after propofol induction within first few minutes, but it cannot reverse respiratory depression during propofol induction pretreated with nalbuphine.

The cAMP-dependent protein kinase(PKA) is an intracellular enzyme with serine-threonine kinase activity that plays a key role in cell growth, differentiation, and apoptosis in eukaryotes. In order to understand the PKA signal transduction pathway regulating cell life cycle and identify its role, we focused on the characterization of up-/down-regulated genes by PKA using the differential display polymerase chain reaction. Seven differentially expressed sequence tags(DEST) have been obtained. Among these DESTs, 2DESTs were homologous to the sequence of genes from BLAST search result. KC1-5 DEST that was up-regulated in A123.7 cells was highly corresponded to mouse apoptosis-related gene(MA-3) or mouse mRNA for topoisomerase inhibitor suppressed(TIS). MA-3 was induced in various types of apoptosis, specially in NGF-deprived apoptotic PC12 cells, TIS was down-regulated in the RVC lymphoma cells incubated with topoisomerase inhibitor that induces DNA strand breakages. PG1-1DEST that was highly expressed in PC12 cells was corresponded to transposon Tn103'-end. Tnansposon Tn10 was up-regulated in differentiated myeloblastic ML-1 cells by 12-O-tetradecanoylphorbol-13-acetate. This study illuminates that MA-3/TIS was down-regulated by PKA activity, and transposon Tn10 was up-regulated by it.
