1.A Study on the Urinary Incontinence, Lower Urinary Tract Symptoms, Lifestyle and Sexual Matters of Women in an Urban Area.

PURPOSE: The purpose of this study was to identify the frequency of urinary incontinence and lower urinary symptoms, to identify lifestyle and sexual matters for UI of adult women in an urban area. METHOD: The sample consisted of 364 educated women from a women's health education program in a public health center. For data collection, the Bristol Female Lower Urinary Tract Symptoms Questionnaire developed by Jackson, et. al.(1996) and the demographic questionnaire were used. For data analysis, SAS 8.2 program was used. RESULT: The results were as follows: 1. the prevalence rate of UI was 73.63%. 45.88% of them were stress incontinence and 27.75% of them were urge incontinence. 2. Remarkable differences were found between the type of UI and LUTS in the case of urgency(x2=42.0585, P<0.001), unexplained incontinence(x2=59.0585, P<0.001), noctural incontinence(x2=18.8080, P<0.001) and catetherization(x2=10.7207, P= 0.004), burning sense(x2=27.7400, P<0.001). 3. Remarkable differences were found between the type of UI and lifestyle matters in the cases as follows fluid intake restriction(x2=31.0532, P=0.008), interference in physical activity(x2= 36.7481, P=0.001), interference in relation with others(x2=22.2729, P=0.034). 4. Remarkable differences were found between the type of UI and sexual matters with difficulty of sexual intercourse(x2=16.1898, P=0.002), and urine leakage during sexual intercourse(x2=17.9752, P=0.001). CONCLUSION: Based on the results, developing and carrying out an adequate UI care and education program is needed.

2.The Experience of Mind Control among Elderly in Korea.

PURPOSE: This study was conducted to understand the meaning of mind control as it is understood by elderly persons in Korea. METHOD: This study was conducted by engaging three elderly persons who are over the age of sixty five and presently residing in Seoul. Several in depth interviews were conducted with these individuals from March 2003 until November 2003. The contents of the interviews were analyzed using the Giorgi Method (1985), which places the emphasis on discovering meaning within the phenomenological context of an individual's experience. RESULTS: The analysis of the contents revealed the following four components with regard to the meaning of mind control for elderly persons (1) Settling the mind (2) Gradual lowering of expectations and hope for the success of offspring, (3) Having good thoughts and acting accordingly, and (4) Making up one's mind to follow the 'natural flow of life'. CONCLUSION: By allowing a wider and deeper understanding of the meaning of mind control for elderly persons the results of this study provide a basis for improving the care of elderly with a holistic perspective.

3.Modified Suture Lasso Technique for the Coronoid Process Fractures of the Elbow: Technical Note

The coronoid process of the ulna surrounds the distal humerus and plays a vital role in posterior dislocation and posterolateral rotational instability. Elbow stability may not be possible if there is a more than 50% loss. Anteromedial facet fractures can lead to instability, radioulnar synostosis. ulnohumeral arthrosis, severe stiffness. Therefore, surgical fixation is required to ensure adequate elbow stability and decrease the risk of posttraumatic ulnohumeral arthrosis. There are several approaches for the repair of coronoid process fractures.In most cases, the posterior approach was used to make a transosseous tunnel connecting the posterior aspect of the olecranon to fix the fracture site. During surgery, the additional dissection required to repair the coronoid might increase the infection risk, nerve injury, heterotopic ossification, and elbow stiffness. The author performed a modified suture lasso and plate fixation with good outcomes. This paper reports this case with a review of the relevant literature.
4.Intratendinous Tophaceous Gout Mimicking Cellulitis after Achilles Tendon Repair

A diagnosis of gout is often straightforward because gout has well known clinical presentations, laboratory analyses, and radiologic features. On the other hand, gout can mimic other diseases by showing a range of atypical clinical manifestations. This paper reports a 35-year-old male with no prior history of gout who developed tophaceous gout at his previously repaired Achilles tendon 11 years after surgery. He was initially misdiagnosed with cellulitis because of his atypical clinical features. This case is presented with a review of the relevant literature.
5.A Clinical Review of Neonatal Intestinal Obstruction.

BACKGROUNDS: Intestinal obstruction is one of the main causes of neonatal operations. It is caused by congenital factors, inherited diseases, and acquired diseases. A study was made to understand the status of neonatal intestinal obstruction cases at our hospital. METHODS: This was a clinical analysis of 53 cases of neonatal intestinal obstruction which had been experienced from March 1992 to February 1998 at the Department of General surgery, Sung-Ae General Hospital. RESULTS: 1) Operations for neonatal intestinal obstructions accounted for 42.4% of all neonatal operations. 2) There were 38 males and 15 females; the male-to-female ratio was 2.5:1. 3) Gestational period of 9 cases (16.9%) was less than 36 weeks and the weight in 10 cases (18.8%) wre below 2,500 gm at birth. 4) The main clinical symptoms on admission were vomiting or abdominal distension. 5) There were 49 cases (92.4%) of mechanical obstruction and 4 cases of a paralytic ileus. 6) Associated anomalies were present in 9 cases (16.9%). 7) The postoperative complication and motality rates were 30.1% and 7.5% respectively. CONCLUSIONS: Our clinical data might provide clinical suspicisions leading to early diagnosis and treatment.

6.Dosimetry and Medical Internal Radiation Dose of Re-188-DTPA for Endovascular Balloon Brachytherapy Against Restenosis after Coronary Angioplasty.

PURPOSE: Liquid beta emitter filled in angioplasty balloon could be used to perform endovascular balloon brachytherapy to prevent coronary artery restenosis. We investigated the dosimetry for Re-188-DTPA liquid-filled balloon and medical internal radiation dosimetry in case of balloon leakage. MATERIALS AND METHODS: We estimated radiation dose from an angioplasty balloon (20 mm lengfh, 3 mm diameter cylinder) to the adjacent vessel wall using Monte Carlo EGS4 code. We obtained time-activity curves of kidneys in normal dog and calculated Tmax, T1/2. Using MIRDOSE3 program, we estimated absorbed doses to the major organs (kidneys, bladder) and the whole body when we assumed that balloon leaked all the isotope contained. RESULTS: The radiation dose was 17.5 Gy at the balloon surface when we applied 3,700 MBq/ml of Re-188 for 100 seconds. Fifty percent of the energy deposited within 1 mm from the balloon surface. The estimated internal dose to the whole body was 0.005 mGy/MBq and 18.5 mGy for the spillage of 3,700 MBq of Re-188. CONCLUSION: We suggest that Re-188-DTPA can be used for endovascular balloon brachytherapy to inhibit coronary artery restenosis after angioplasty with tolerable whole body radiation dose in case of balloon rupture.

7.A Survey of the Model Nutrition Services in Community Health Centers.

The Korean government enacted the National Health Promotion Act in January, 1995 and proclaimed its regulations and rules in September 1995, which became the basis of the national health policy. The health promotion programs consist of education for health, prevention of diseases, improvement of nutrition, and practice of healthy life style as defined in that Act. The Community Health Act was amended in 1995, which included implementing nutrition services in community health centers. The purpose of this report is to summerize the nutrition services conducted in 32 community health centers. the main nutritional activities were as follows : 1) nutritional guidance by counseling and education for pregnant or lactating women, infants, preschool children, and those with chronic diseases, 2) collection, analysis, and interpretation of data collected from the community, on background conditions and target population for the assessment of community needs, 3) evaluation of nutritional status of population in the community 4) nutritional guidance for mass feeding in different institution including schools and welfare institutions. In order to meet the government's expectations and desires, the community health centers have made continuous efforts to put nutritional activities into practice in the community. However, there are constraints, such as relative staff shortages, lack of funds, and information which hampers the nutritional activities.

8.Supernumerary Tooth in Nasal Cavity: Report of 1 Case.

Supernumerary tooth, named mesodens, occur between the maxillary central incisors are generally found in pairs, although it is sometimes found singly. An inverted mesiodens may move toward the nasal cavity and erupt in the floor of the nose. Approximately 90% of all supernumerary tooth usually appear in the area of the maxilla, where they disrupt the position and eruption of normal teeth. A supernumerary tooth may closely resemble the teeth of the group to which it belongs, i.e., molars, premolars or anterior teeth.

9.Osteoporotic Vertebral Compression Fracture Associated with Pregnancy and Lactation in Young Women

Osteoporosis associated with pregnancy and lactation is a rare disease that can cause osteoporotic vertebral compression fracture (OVCF).Patients usually complain of severe back pain, which is easily mistaken for pain due to pregnancy, childbirth, and lactation, making a rapid diagnosis and treatment difficult. The authors diagnosed OVCF related to pregnancy and lactation through a physical examination, simple radiography, whole-body bone scan, magnetic resonance image, bone marrow density, and blood tests in a 29-year-old female patient and a 31-year-old female patient who presented with low back pain. This paper reports two cases of symptom improvement through a teriparatide injection, wearing thoracic lumbar sacral orthosis and taking calcium and vitamin D with a review of the literature.
10.Neglected Osteoporotic Vertebral Compression Fracture Treated with Postural Reduction and Minimally Invasive Transpedicular Fixation with Weekly Teriparatide

Osteoporotic vertebral compression fractures (OVCFs) are the most common osteoporotic fractures in postmenopausal women. One the other hand, they may be misdiagnosed if the deformity is not severe at the time of occurrence. In general, it is treated through absolute bed rest and braces. Vertebroplasty or kyphoplasty is sometimes performed when there is a severe decrease in the height of the vertebral body. In addition, transpedi cular fixation can be performed when there is an accompanying injury to the posterior complex or neurologic symptoms. In this case, the authors performed minimally invasive transpedicular fixation after achieving vertebral body height recovery through postural reduction in OVCF patient who were neglected for one month after the injury. After securing initial stability through screw fixation, complete bony union was achieved more quickly and stably through weekly teriparatide administration. The authors report this case with a review of the relevant literature.
