2.Alterations of Cerebral Blood Flow and Cerebrovascular Reserve in Patients with Chronic Traumatic Brain Injury Accompanying Deteriorated Intelligence.

PURPOSE: The purpose of this study was to evaluate alterations of regional cerebral blood flow (CBF) and cerebrovascular reserve (CVR), and correlation between these alternations and cognitive dysfunction in patients with chronic traumatic brain injury (TRI) and normal brain MRI findings. MATERIALS AND METHODS: Thirty TBI patients and 19 healthy volunteers underwen1 rest/acerazolamide brain SPECT using Tc-99m HMPAO. Korean-Wechsler Adult Intelligence scale test was also performed in the patient group. Statistical analysis was performed with statistical parametric mapping software (SPM97), RESULTS: CBF was diminished in the left hemisphere including Wernicke's area in all patients with lower verbal scale scores. In addition, a reduction in CBF in the right frontal, temporal and parietal cortices was related with depressed scores in information, digital span, arithmetic and similarities, In patients with lower performance scale scores. CBF was mainly diminished in the right hemisphere including superior temporal and supramarginal gyri, premotor, primary somatomotor and a port of prefrontal cortices, left frontal lobe and supramarginal gyrus. CVR was diminished in sixty-four Brodmann's areas compared to control. A reduction in CVR was demonstrated bilaterally in the frontal and temporal Iobes in patients with lower scores in both verbal and performance tests, and in addition, both inferior parietal and occipital lobes in information subset. CONCLUSION: Alterations of CBF and CVR were demonstrated in the symptomatic TBI patients with normal MRI finding. These alterations were correlated with the change of intelligence, of which the complex functions are subserved by multiple interconnected cortical structures.

Nodular pseudosarcornatous fasciitis is a benign, fibrohlastic proliferative disease. This case report is one of a hard nodule which a 35-year-old male suddenly discovered on his right forearm 10 days prior to being aeen. The nodule showed characteristic histological findings of nodular pseudosarcomatous fasciitis.

7.A Case of Phaeomycotic Subcutaneous Abscess Caused by Wangiella Dermatitidis.

We present here a case of phaeornycotic subcutaneous abscess caused by Wangiella dermatitidis in a 34-year-old male, who had multiple asymptomatic subcutaneous masses of 7 months duration over the neck and right axilla. ]n this case, we could observe typical gross colony morphology of W. dermatitidis. which showed creamy greyish, yeast-like colony with aerial mycelia after 3 to 4 weeks. ]n histopathologic study, we found mixed cell granuloma and fungal structure in biopsy specimen. We comfirmed W. dermatitidis by exoantigen test, and treated the subcutaneous lesions by surgical excision and ketoconazole with good result. This case is the first reported in Korea.

8.A Case of Neurilemmomatosis.

Neurilemmomatosis is a clinical entity consisting of multiple cutaneous neurilemmomas, central nervous system tumors, and neurologic disorders. Since Shishiba et al first described the disorder in 1984, several cases have been reported. We report a Korean case of neurilemmomatosis showing multiple neurilemmomas of the skin and spinal cord, with associated motor and sonsory nerve disturbance. Histologic, immunohistopathologic and electron microscopic studies confirmed the diagnosis.

10.Various Epidermal Changes Overlying A Dermatofibroma.

Various epidermal changes overlying a dermatofibroma can be induced through the proliferation of pluripotential epidermal cells by the stimulation of mesenchymal factors. The authors report a case of dermatofibroma in a 55-year-old male who had a reddish brown, asymptomatic nodule of 5 years duration on his left forearm. The biopsy specimen revealed a dermatofibroma of cellular type associated with epidermal changes: (1) acanthosis; (2) elongation of rete ridges; (3) thin interlacing epidermal strands with increased pigmentation resembling seborrheic keratosis or fibroepithelioma; (4) immature hair follicles and sebaceous lobules; (5) hair germ like buddings; (6) superficial basal cell epithelioma-like proliferations.
