Clinincal and radiological findings were analysed in 63 patiets with Osgood-schlatter Disease diagnosed atNational Police Hospital during the period from Jan. 1975 to 1981. The results were obtained as follows; 1. Thedisease was most common between 16 and 20 years, being 39 cases in a total of 63 cases. 2. The ratio of male andfemale was 31:1. 3. Involvement was more often unilateral (45 cases ) than bilateral (18 cases), and the left side(29 cases) was more common than the right side (16 cases). 4. Roentgenogram revealed one or two separated bonyfragments in most cases. 5. Type II by Woolfry and Chandler classification was most common type. 6. The mostcommon clinical findings were pain and tenderness on tibial tuberosity. 7. Trauma history was positive in 11 casesin a total of 63 cases.

Cardiovascular measurement on PA teleroentgenogram of the chest is simple, but very useful in the diagnosis,prognosis, and therapy of hypertensive patients. The authors have measured and calculated cardiothoracic ratio,Lt, to Rt. cardiac diameter ratio, and Lt, aortic knob width to thoracic diameter ration for evaluation of theeffect of blood pressure and age in hypertensive group. We used materials of 70mm fluorography of 255 hypertensivemen above 150/90mmHg chosen from national police hospital during Jan. from Aug. 1981. Their age ranged from 20 to50 yeras. The results were as follows. 1. Cardiomegaly (above 50 percents in cardiothoracic ration) ofhypertensive men is 26%. 2. The cardiothoracic ration increased progressively with blood pressure and age, butpooly correlated between cardiothoracic ration and age than blood pressure. 3. The Lt. to Rt. cardiac diameterration gave similar trend to the cardiothoracic ration. It indicates that hypertensive cardiac enlargement mainlydepned on Lt. Ventricular enlargement. 4. The Lt. aortic knob width to thoracic diameter ration increasedprogressively with blood pressure and age, and had linear correlationship with blood pressure and age. Therefore,prominence of aortic knob is the most significant finding in hypertensive cardiovascular disease.

Background: and Purpose To determine the imaging characteristics and cutoff value of18F-florapronol (FC119S) quantitative analysis for detecting β-amyloid positivity and Al- zheimer’s disease (AD), we compared the findings of FC119S and 18F-florbetaben (FBB) positron-emission tomography (PET) in patients with cognitive impairment. Methods: We prospectively enrolled 35 patients with cognitive impairment who underwent FBB-PET, FC119S-PET, and brain magnetic resonance imaging. We measured global and vertex-wise standardized uptake value ratios (SUVRs) using a surface-based method with the cerebellar gray matter as reference. Optimal global FC119S SUVR cutoffs were determined using receiver operating characteristic curves for β-amyloid positivity based on the global FBB SUVR of 1.478 and presence of AD, respectively. We evaluated the global and vertex-wise SUVR correlations between the two tracers. In addition, we performed correlation analysis for global or vertex-wise SUVR of each tracer with the vertex-wise cortical thicknesses. Results: The optimal global FC119S SUVR cutoff value was 1.385 both for detecting β-amyloid positivity and for detecting AD. Based on the global SUVR cutoff value of each tracer, 32 (91.4%) patients had concordant β-amyloid positivity. The SUVRs of FC119S and FBB had strong global (r=0.72) and vertex-wise (r>0.7) correlations in the overall cortices, except for the parietal and temporal cortices (0.4Conclusions Quantitative FC119S-PET analysis provided reliable information for detecting β-amyloid deposition and the presence of AD.

Since the first description of arteriovenous fistula (AVF) secondary to lumbar disc surgery by Linton and White in 1945, many cases of the accidental injury to the great vessels anterior to the lumbar area have been reported. We experienced a unique case of AVF between the internal iliac artery and internal iliac vein following disc surgery. Diagnosis of traumatic AVF in a patient with multiple varicose dilatation of vein in the left thigh and inguinal area was heralded by the presence of unique, unusual arterial signals in the area of dilated veins during the leg Doppler scan. 3D-CT angiography clearly confirmed the presence of a fistular tract between the internal iliac artery and vein. Using the Seldinger technique, a PTFE stent graft was inserted into the internal iliac artery to cover the fistular site nonsurgically. After the procedure, venous dilatation decreased and the patient felt comfortable. Follow-up 3D-CT angiography one month after procedure showed complete obliteration of fistuar tract and patent blood flow through PTFE stent graft in the internal iliac artery.

Although bladder cancers are very common, little is known about their molecular pathogenesis. It is known, that p53 alteration is found in about 60%p of muscleinvasive bladder cancer, necessiating aggressive therapy and poor outcome. We examined the nuclear expression of p53 protein, using D07 monoclonal antibody in the urine samples, from 31 patients with transitional cell carcinoma of the bladder to investigate the correlation of p53 overexpression with histologic grades and depth of invasion. The positive rate of p53 protein was 27%o in superficial bladder tumor, but increased up to 71% in the invasive bladder carcinomas. The overexpression of p53 protein increased according to Mostofi grading system from 18% in grade I, 45% in grade Il, and up to 100% in grade ill. The p53 expression tended to be higher in the invasive and high grade bladder cancers than in the superficial and low grade ones(p<0.05). These results suggest that immunohistochemical analysis of the urine specimen in the bladder cancer patients could be a useful method of screening for the presence of p53 mutant protein. The mutant p53 protein expression may be an indicator of bladder cancer with more proliferative potential and/or aggressive biologic behavior.

It is well recognized that radiation can be carcinogenic for a wide variety of tumors, especially, in breast, thyroid, and bone marrow which appear to be radiosensitive. The criteria for establishing the dignosis of radiation- induced malignancy are the knowledge of prior irradiation and the appearance of a malignancy in the irradiated area. We report a case of carcinoma of the thyroid and esophagus following prior neck irradiation for thyroid mass.

OBJECTIVES: Vaginal birth or trial of labor after previous cesarean section has become one of the most remarkable changes in obstetric practice. The safety and efficacy of a trial of labor and vaginal birth after previous cesarean are well documented. The purpose of this report is to predict the likelihood of vaginal birth in patients undergoing a trial of labor after previous cesarean delivery using factors known at the time of hospital admission. METHOD: In this retrospective study, 120 women who attempted vaginal birth at Kyung-Hee University Hospital from March 1997 to March 1999. An attempt to identify possible prognostic factors for success of such a trial was made and we evaluated the variables of significant predictive value and the patients' characteristics in the success group and failure group of women who attempted VBAC. RESULT: 87 cases(72.5%) in 120 cases succeeded in VBAC and 33 cases(27.5%) failed. In this comparative groups in VBAC, there was significant difference in CPD index(cephalopelvic disproportion index) and Bishop score, but no significant difference in gestational age, the estimated fetal weight by sonography and newborn birth weight. CONCLUSION: In this study, Bishop score and CPD index and age may be useful and valid predictor of success in VBAC and this information could be particularly valuable. The CPD index may prove most important in determining if a vaginal birth should occur after a cesarean section because it can clearly identify some patients who need a repeat cesarean section.
