Actinomycosis is a chronic suppurative and granulomatous bacterial infection characterized by contiguous spread, abscess formation and sinus tract formation. There are four clinical forms according to the lesional site, as 1) cervicofacial, 2) thoracic, 3) abdominal, and 4) disseminated form. Recently, we experienced a case of 54 year-old patient with left mandibular actinomycosis. The pathognomonic findings of actinomycosis is sulfur granule with multiple filaments in Gram-stain and the treatment of actinomycosis is surgical excision of mass or sinus tract with massive antibiotics (esp. Penicillin) therapy for 6 to 12 months.

We experienced one case of severe pituitary dwarfism in a 10 years old female girl. Magnetic resonance image (MRI) revealed transection of the pituitary stalk stalk with the formation of high intensity ectopic posterior lobe located at the median eminence and agenesis of an anterior lobe of pituitary gland. The serum growth Hormone (GH) response to clonidine and L-dopa revealed severe GH deficiency. The patient had responses to TRH, normal TSH and partial prolactin response, respectively. There was not response LH and FSH to GnRH. The morning cortisol concentration and serum T4 concentration were decreased below the normal range. These findings and no hyperprolactinemia suggested the presence of a vascular connection between the pituitary gland and hypothalamus, which is not visible on MRI. Sofar, the primary cause of idiopathic pituitary dwarfism in many patients is injury to hypothalamus by perinatal insults. In this patient, there was no history of perinatal insults and postnatal head trauma but transection of the pituitary stalk. We report a case of severe pituitary dwarfism due to agenesis with brief review of related litereature.

PURPOSE: This study compared the effectiveness and ease of operation of two different training methods, response sequence based training (RBST) and chest compression first training (CCFT), for cardiopulmonary resuscitation (CPR). METHODS: Eighty-five railroad workers were divided into two groups: those who applied the usual CPR training method (response sequence based training, 44 people) and those who applied our CPR training method (chest compression first training, 41 people). The objective skill performances were evaluated by using a mannequin (JAMY-IV RECO, Kyotokagaku, Japan). RESULTS: There was no statistical difference in CPR performance and artificial ventilation success rate between the RSBT group and the CCFT group. However, the artificial compression success rate and compression depth of the CCFT group compared to the RSBT group was statistically higher (p=0.006, 0.001). In addition, the wrong-hand-position rate of the RSBT group compared to the CCFT group was statistically higher (p=0.000). Furthermore, instructor satisfaction with practical training operations in the CCFT group compared to the RSBT group was statistically higher (p=0.001) and instructors reported that students were focused and easily capable of performing the CCFT training method. CONCLUSION: The chest compression first training method is easy to perform and not too complex to operate, making it effective in student performance and instruction. More effective CPR training is expected based on this method.

BACKGROUND: The diagnosis of dementia is dependent on the decline of cognitive and functional ability. To measure the functional ability, a Korean version of Instrumental Activities of Daily Living (K-IADL) was developed and validated. However, the characteristics of the instrument were not fully evaluated. The study was conducted for the purpose of investigating which of the variables were associated with the K-IADL scores. METHODS: The study group from Namwon, Korea, consisting of 235 participants aged 65 years and over, was given the K-IADL to evaluate the functional ability of the participants. The independent variables included sociodemographic data (age, sex, education), vascular factors (blood pressure [BP], history of hypertension and type 2 diabetes mellitus, smoking, alcohol intake, and heart rate), and anthropometric measurements (height and arm length). RESULTS: Age, female gender, low educational attainment, high systolic BP, shorter height and arm length were significantly associated with higher K-IADL scores in univariate analyses. Among the results, age, education, systolic BP, and arm length showed independent associations with the K-IADL in multivariate analysis. CONCLUSIONS: The K-IADL was not influenced by gender in adjusted regression analyses, however education and arm length affected the functional scores, which could suggest a possibility of functional reserve. Also, it was suggested that vascular risk factors could be associated with functional ability.

Dysmenorrhea is the most common gynecologic condition in women during the reproductive period. Severe dysmenorrhea pain affects their social activities, sleep, and quality of life. Nevertheless, the proportion of women with dysmenorrhea do not receive adequate medical counseling or pharmacological treatments. Primary dysmenorrhea is diagnosed clinically, and the secondary causes that can cause pelvic pain should be identified. The treatment of choice for primary dysmenorrhea is non-steroidal anti-inflammatory drugs (NSAIDs). In order to maximize the therapeutic effect, it is necessary to ensure that the appropriate medication is administered in a proper way. NSAIDs can cause adverse effects, including gastrointestinal disorders. If side effects occur or are anticipated with NSAIDs, the use of hormonal contraceptives may be recommended when contraception is considered. In addition to these pharmacological treatments, heat, dietary, and behavioral therapies have been tried and reported to have some effects. However, further research is required for robust conclusions.

Purpose: The purpose of this study was to identify the factors that influence the consumption of convenience foods among Korean adolescents. Methods: This study had a descriptive cross-sectional study. Data were obtained from the 15th (2019) Korea Youth Risk Behavior Web-based Survey, which involved a nationwide representative sample of 57,303 middle- and high-school students. Convenience-food consumption was defined by frequency of adolescents consuming convenience foods obtained from convenience stores, supermarkets, and cafeterias over the previous 7 days. The analyzed variables were related to sociodemographic, mental health, and health behavior. The Rao-Scott χ2 test was applied to examine the difference in the rate of consuming convenience foods obtained from convenience stores according to each factor. Hierarchical logistic regression was conducted to examine the factors that influence convenience-food consumption among Korean adolescents. Results: It was found that 29.1% of Korean adolescents consumed convenience foods obtained from convenience store more than three times per week. The significant influencing factors were female sex; low subjective academic achievement, and subjective household economic status; high perceived stress; low subjective sleep sufficiency; experience of depression; suicidal ideation; lower physical activity; skipping breakfast; lower consumptions of fruit, milk, and vegetables; higher consumptions of soda drinks, sweet drinks, caffeine, and fast food; lower water intake; current smoking and drinking; drug use; and experience of violence. Conclusion These findings provide a better understanding of the sociodemographic, mental-health, and health-behavior factors that influence the consumption of convenience foods among Korean adolescents. We suggest that differentiated policies, strategies, and nutrition education need to be developed and implemented, in order to address the above-mentioned factors and thereby reduce such behaviors among Korean adolescents.

Primary splenic tumors are rare and mainly found incidentally on radiologic studies. Among them, sclerosing angiomatoid nodular transformation (SANT) of the spleen is a new entity defined as a benign pathologic lesion. Most SANTs have no clinical symptoms and are occasionally accompanied by other splenic diseases such as malignancies. So, the exact diagnosis of the nature of the splenic tumor is mandatory for further treatment. But, preoperative diagnosis is not easy since it is difficult to obtain the tissue from the spleen for pathological study. Recently, laparoscopic splenectomy has become the more standard procedure for the spleen for diagnosis and treatment. Here, we report a rare case of SANT diagnosed following laparoscopic splenectomy.
