1.A study on the Stress and Fatigue of Research Workers.

In order to investigate the relationship between stress and fatigue of research workers, we surveyed 246 research workers from September 15 to October 20, 1997. The results were as follows ; 1. Females, 20~29 years of ages, singles are associated with significantly higher stress symptoms and fatigue symptoms 2. The research workers who drink nearly everyday complained significantly lower stress symptoms and fatigue symptoms. 3. The research workers who excercise regularly complained significantly lower stress symptoms and fatigue symptoms. 4. The research workers who are somewhat satisfied by Job and income complained lower stress symptoms and fatigue symptoms. 5. The high risk stress group was 4.5 % of the research workers. The latent stress group was 93.9 % and healthy group was 1.6 %. The research workers belonged to high risk stress group complained significantly higher fatigue symptoms.

2.A Case Of Lymphangioma Circumscriptum: Magnetic Resonance Imaging (MRI) in Diagnostic Evalutaion.

A 21-year-old male visited our clinic with an erythematous plaque studded with papulovesicules on the right thigh and calf which had been noticed at 3 months after birth. Moving the limb caused intermittent pain. He was diagnosed with lymphangioma circumscriptum which involved muscle layers after checking the skin biopsy and magnetic resonance imaging (MRI). Partial removal of mass resulted in relieving pain on motion.

4.A Case of Diffuse Plane Xanthoma.

Diffuse plane xanthoma (DPX) was first described by Altman and Winkelmann in 1962. Patients with DPX usually exhibit yellow, flat or slightly elevated plaques that gradually develop on the eyelids, neck, upper trunk, and flexures. Histopathologically, histiocytic foamy cells diffusely spread in the upper dermis with an admixture of lymphocytes and histiocytes. This disease may occur with either hyperlipoproteinemic or normolipoproteinemic states. DPX with hyperlipoproteinemia is associated with familial hyperlipidemia or liver cirrhosis. DPX with normolipoproteinemia is often associated with hematological problems such as multiple myeloma, lymphoma, chronic lymphatic leukemia, chronic myeloid leukemia, and chronic myelomonocytic leukemia. We report a case of diffuse plane xanthoma with hyperlipoproteinemia.

5.A Case of Angiokeratoma Treated with a 532 nm KTP (Potassium-titanyl-phosphate) Laser.

Angiokeratomas are cutaneous vascular lesions that are characterized by dilated thin-walled blood vessels that lay in the upper part of the dermis, and this condition is mostly associated with epidermal reactions such as acanthosis, hyperkeratosis and elongation of rete ridges. Four variants have been described (angiokeratoma of Fordyce, angiokeratoma of Mibelli, angiokeratoma corporis diffusum, and solitary or multiple angiokeratomas). Among these variants, angiokeratoma of Fordyce develops mainly in elderly Caucasian men and it is common on the genitalia. Angiokeratomas are blue to purple, smooth, 2- to 5-mm papules on the scrotum, penile shaft or glans. They generally appear to multiply during life, but they occasionally present as single lesions. They may bleed after trauma and they may be mistaken for a nevus, melanoma or Kaposi sarcoma. Electrocautery or laser ablation can be offered for treatment. Various studies have recently reported successful treatment with argon laser, copper vapor laser, variable pulse width 532 nm neodymium:yttrium-aluminium-garnet (Nd:YAG) laser, 532 nm KTP (potassium-titanyl-phosphate) laser and flashlamp-pumped pulsed dye laser (PDL). We report here on a case of angiokeratoma that was treated with a 532nm KTP laser.

6.A Case of Traumatic Panniculitis Following Fractional Laser Therapy.

Traumatic panniculitis can be induced through the injection of various substances into the subcutaneous fat, especially in patients with psychiatric problems. This condition can also be caused through the subcutaneous implantation of different materials for cosmetic or therapeutic reasons or through mechanical or physical means. The diagnosis is always worth considering in those patients with bizarre clinical lesions and inflammatory changes in the subcutaneous fat that defy ready classification. There have been reported cases of traumatic panniculitis induced by acupuncture, but no reported cases associated with laser therapy. Fractional photothermolysis is the newest approach to laser therapy, with multiple micro-laser beams that target water-containing tissues to cause photocoagulation of narrow columns of skin. The penetration depth of the multiple micro-laser beams is up to 900microgram, generally to the reticular dermis level. A 19-year-old female patient presented with a tender 4x7 cm sized erythematous to brownish, irregularly shaped, atrophic plaque on the right shin. She was treated for a leg scar using a 1,550 nm erbium fiber laser. Histopathologic examination showed moderate inflammatory infiltration and lipomembranous fat necrosis in the subcutaneous septa with mild inflammation in the overlying dermis. The tissue proved to be infiltrated by lymphohistiocytic cells and a few eosinophils. The patient was treated with systemic and topical corticosteroids; she has experienced no recurrence. We report an interesting case of traumatic panniculitis following fractional laser therapy.

8.A Case of Valproic Acid Induced DRESS Syndrome.

DRESS syndrome is an uncommon but potentially life-threatening subset of severe drug eruption characterized by fever, rash, leukocytosis with eosinophilia or atypical lymphocytosis, lymphadenopathy, and liver or renal dysfunction. The aromatic anticonvulsants (phenytoin, phenobarbital, carbamazepine) and sulfonamides are the most common cause of DRESS syndrome. Valproic acid, which is a widely used non-aromatic anticonvulsant, rarely causes DRESS syndrome. We report a case of DRESS syndrome in 47-year-old man caused by the use of valproic acid.

9.A Case of Acral Syringomas Mimicking Chronic Eczema.

Syringomas are common benign adnexal tumors, probably of the eccrine origin. They are more common in women and usually found around the eyelids. An exclusively acral site of the lesions is very rare. We present a case of acral syringomas in a 44 year-old woman, involving only the dorsum of the fingers of both hands. It is important to include syringomas in the clinical differential diagnosis of papular lesions of the hands.

10.Pustular Pyoderma Gangrenosum Associated with Ulcerative Colitis.

Pyoderma gangrenosum (PG) is a rare and painful skin disease, frequently associated with underlying systemic conditions, such as inflammatory bowel disease or hematological malignancy. PG is generally classified into 4 types: classic (ulcerative), bullous, pustular, and vegetative. Pustular PG is an uncommon variant of PG that the pustules do not progress to form ulcers. We report a case of pustular PG, which was successfully treated with infliximab and topical tacrolimus, in a 28 year-old Korean man with ulcerative colitis.
