1.Two Case of Testicular Teratoma.

Testicular teratomas are germ cell neoplasms that consist of tissue derived from ectoderm, mesoderm and endoderm in various stages of maturation. The teratomas are subclassified into mature teratoma, immature teratoma, and malignant teratoma. depending on the appearance and maturity of the various components. Prognosis is directly related to maturity of the tissue. Recently we experienced two cases of testicular teratoma, so we report these two cases of testicular teratoma with review of literature.

2.Therapeutic Results of Concurrent Chemoradiation in Locally Advanced Uterine Cervical Cancer.

PURPOSE: Despite a development fo therapeutic machines and advance in modern radiation therapy techniques, locally advanced cervical carcinoma has shown high rate of local failure and poor survival rate. Combination of chemotherapy and radiotherapy demonstrated benefit in improving local control and possibly the overall survival. Our study was performed to evaluate effect of concurrent chemoradiation on locally advanced uterine cervical cancer. METHODS AND MATERIALS: Twenty six patients with locally advanced stage(FIGO stage IIB with > or = 5 cm in diameter, III, IVA) were treated with combination of radiation therapy and concurrent cisplatinum between May of 1988 and Septermber of 1993 at our hopital. Radiation therapy consisted of external irradiation and 1-2 sessions of intracavitary irradiation, Cisplatinum was administered in bolus injection of 25mg/m(2) at weekly intervals during the course of external radiation therapy. RESULTS: Of the 26 patients, twenty-five patients were evaluable for estimation of response. Median follow-up period was 25 months with ranges from 3 to 73 months. Stage IIB, III, and IVA were 16, 5,4 patients, respectively. Twenty patients were squamous cell carcinoma. Response was noted in all 25 patients: complete response(CR) in 17/25(68%), partial response(PR) in 8/25(32%). Of the 24 patients except one who died of sepsis at 3 months follow-up, seventeen patients(70.8%) maintained local control in the pelvis: 16/17(94.1%) in CR, 1/17(14.3%) in PR. Fourteen of the 17 patients with CR are alive disease free on the completion of follow-up. Median survival is 28 months for CR and 15 months for PR. Analysis of 5-year survival by stage shows 11/16(59.8) in IIB, 3/5(60.6%) in III, and 1/4(25.0%) in IVA. Overall 5-year survival rate was 55.2%. Ten Patients recurred: 4 at locoegional,3 in distant metastasis and 3 with locoregional and distant site. Toxicity by addition of cisplatinum was not excessive. CONCLUSION: Although the result of this study was obtained from small number of patients, it is rather encouraging in view of markedly improved response rate compared with the results of historical group.

Published reports of these rare lesions and possible mechanisms of their origin are reviewed. Magnetic resonance imaging is better than myelography and computed tomography at delineating the intramedullary extent of the tumor. We belive that complete resection is the treatment of choice.

6.The Appearance and the Axial Alignment of the Lower Extremity.

PURPOSE: To investigate the relationship between the varus appearance and the axial alignment of the lower extremity. MATERIALS AND METHODS: The axial alignments of the lower extremities of 46 normal male adults (volunteers) were measured from the radiographs and compared with the reports for Caucasians, and between the groups divided according to their subjective appearance. RESULTS: The femorotibial mechanical angle was 177.4+/-2.4degrees and the medial obliquity of the knee was 94.3+/-2.0degrees, which were the varus compared with the reported reference points. In the group containing those with the subjective varus appearance of the lower extremities, the femorotibial mechanical angle and the medial obliquity of the knee and ankle joint were the varus, and the metaphyseal-diaphyseal angle was greater than in the group containing those with the subjective straight appearance of the lower extremity (p<0.05). CONCLUSION: The axial alignment of the lower extremities of young male adult Korean is the varus, and the subjective varus appearance of the lower extremity is related to the varus axial alignment.

Behcet's disease is a chronie inflammatory symptom complex consisting of recurrent oral ulcer, genital ulcer, ocular lesion or skin lesion. Intestinal involvement in Behcet's disease most commonly affects the ileocecal region in the patient with gestrointestinal symptoms, But esophageal involvement in Behcet's disease is very uncommon, most commonly affects the midesophagus with localized ulcerative lesion. A 38-Year-old woman with refractory and recurrent oral and genital ulcer was admitted because of aggravation of pharyngodynia and dysphagia. Gastrofiberscopic examination showed multiple, small sized ulcers on oral cavity and mid-to-distal esophagus. She was treated with steroid.

8.The Role of Hepatobiliary Scintigraphy and Oral Cholecystography in Predicting the Performance of Laparoscopic Cholecystectomy.

131I-6beta-iodomethyl-19-norcholesterol(NP-59) has an advantage to assess adrenal dysfunction caused by adrenal cortical disorders. The aim of this study is to evaluate the clinical usefulness of NP-59 scintigraphy in each adrenral disease. Ten patients who did eleven NP-59 adrenal scintigraphies at Dong-A University Hospital from March 1990 to December 1996 were selected as the subject. Among the subject there were 5 cases of Cushing's syndrome, 2 cases of incidentaloma, 1 case of metastatic adrenal tumor, liver cirrhosis with hirsutism and hypertension respectively. Among 5 case of Cushing's syndrome, there were 2 cases of Cushing's disease, 2 cases of adrenal adenoma and 1 case of adrenal carcinoma. There are no disagreement between clinical diagnosis and scan finding in Cushing's syndrome. In 2 incidentaloma cases, even though one is interpretated as a functioning tumor, both of 2 cases could avoid unnecessary biopsy according to scintigraphy result. One case of hirsutism, clinically adrenal originated, revealed the normal scintigraphic hirsutism was extra-adrenal origin. One case of hypertension took the study to exclude the possibility of primary aldosteronism. Normal suppression scan finding revealed that primary aldosteronism did not exist in this case. In conclusion, NP-59 scintigraphy was very useful in diagnosis and differential diagnosis of Cushing's syndrome and it could avoid unnecessary biopsy in the incidental adrenal tumor.

Multiple symmetric lipomatosis (MSL) is a rare disease characterized by the presence of diffuse, non-tender, symmetric deposits of adipose tissue on the neck, back and upper part of the trunk. It has been reported that this disease could be associated with various metabolic abnormalities such as impaired glucose tolerance, hyperuricemia, alcohol consumption, type 4 hyperlipidemia, renal tubular acidosis and thyroid disorder. We present a case of type 2 MSL with pulmonary thromboembolism. He suffered sudden dyspnea and decreased urine output. The causes of dyspnea and acute renal failure (ARF) in this case were dehydration and hemodynamic disturbance due to pulmonary thromboembolism. After thrombolytic and fluid therapy, his condition was improved. He was discharged on warfarin medication and has remained asymptomatic for 12 months.

10.Chest X-ray Findings and Acid-base Disturbances in the Early Phase of Leptospirosis.

BACKGROUND: Clinical syndrome of leptospirosis in Korea which accompanied by hemoptysis, pulmonary hemorrhage, respiratory failure was quite different from that of typical Weil's disease. The death rate in the early stage of disease has relationship with chest X-ray findings and acid-base disturbances resulted from hypoxia and asphyxia. We've come to consider chest X-ray findings and arterial blood gas findings as factors influencing the prognosis of the disease. METHODS: Forty one cases of leptospirosis diagnosed by acute febrille illness symptoms and serologic tests were graded for chest X-ray findings and grouped according to acid-base abnormalities. Retrospectively, we attempted to correlate the acid-base disturbances and chest X-ray findings with morbidity and mortality of the disease. RESULTS: The result were as follows. 1) Abnormal chest X-ray findings were observed in 30 cases(73%). Mortalities according to the chest X-ray findings were 0(0%), 2(17%), 3(0%), 4(0%), 5(33%), 6(50%) and chest X-ray findings influenced the mortality with statistical significance(p<0.05). 2) Acid-base abnormalities were observed in 39 cases(95%) and the types were divided to six groups . Respiratory alkalosis was the most common acid-base disturbance(44%). Five cases were died. Two of five were respiratory alkalosis, two cases were mixed respiratory metabolic acidosis and another case was mixed respiratory-metabolic alkalosis. As shown, acid-base abnormality influenced the mortality and mixed acid-base disturbances were showed high mortality with statistical significance (p<0.05). 3) Total mortality was 12%(five of forty-one). The chest X-ray score, morbidity, pH, pCO2, pO2, and HCO3 were 5.0 +/- 0.8, 3.4 +/- 1,3, 7.16 +/- 0.17, 32.5 +/- 1.1, 43.3 +/- 13.4 and 17.6 +/- 3.1 in died group, 2.4 +/- 0.4, 12.7 +/- 1.0, 7.46 +/- 0.01, 48.8 +/- 13.4, 65.4 +/- 3.7 and 23.4 +/- 0.9 in recovery group, respectively. There was difference between two groups with statistical significance. CONCLUSION: In this study, we concluded that chest X-ray findings and acid-base abnormalities influenced the mortality of leptospirosis. The factors above mentioned make us consider chest X-ray and blood gas analysis are essential to the patient hospitalized for the suspicion of leptospirosis. Especially, these factors pointed out the patient having chest X-ray abnormalities and mixed acid-base disturbances needed intensive care in the early stage of leptospirosis.
