1.Oral allergy syndrome in pollen - sensitized patients.

BACKGROUND: Oral allergy syndrome(OAS) is composed of it,ching sense and edema in oral cavity, lips, throat, pharynx, and larynx following eating some fresh fruits or vegetables. It has been known that most of patients with OAS are allergic to pollens. Common epitopes were found among pollens, fruits and vegetables. Although OAS is a common farm of food allergy in adults, this is the first epidemiologic study of OAS in Korea. MATERIAL AND METHOD: One hundred and fifty one patients who showed positive skin reaction to pollens were telephone-interviewed. Investigation of the prevalence and clinical manifestations of OAS was possible in 81 patients. RESULT: The prevalence of OAS among these patients was 34.6% (28/81). OAS was found in 24(48%) out of 50 patients sensitized to tree pollens, whereas 4(13%) of 31 grass or weed pollen-sensitized paients had OAS. Most common causative food was apple and all of 17 apple- OAS patients were sensitized to tree pollens. Peach was the second common food and 14 of 15 peach-OAS patients were sensitized to tree pollens. Besides oral symptoms, rhinitis, asthma, diarrhea, nausea, vomiting or generalized urticaria were accompanied in half of the OAS patients (14/28). Some patients showed OAS to some unique Korean foods such as dropwort, taro and Aster. CONCLUSION: OAS was very common in pollen-sensitized patients. Larger epidemiologic studies are needed to find unique Korean foods and their antigensm causing OAS.

2.Symptomatic Lateral Patellar Plica : A Case Report.

The plica is a remnant of the synovial folds during the coalescence of three compartments of the knee at 8 weeks of fetal development. The plica are classified according to their corresponding anatomic sites of the knee, such as suprapatellar, medial patellar, infrapatellar and lateral patellar plica. The medial patellar plica is considered to be the one most likely to cause problems and has been well documented in the literature. However, there are very few reports of problems being caused by lateral plica. We encountered one case of pathologic lateral plica that was excised by arthroscopy. We report this case with a review of the relevant literature.

3.The Intraneural Ganglion of the Posterior Tibial Nerve and Medial Plantar Nerve.

Intraneural ganglion of the tibial nerve is very rare. We have experienced a case of the intraneural ganglion cyst in the tibial nerve and medial plantar nerve, which was located at the level of the ankle and foot. A 29-year-old male suffered from lumps causing numbness around the medial aspect of the sole and great toe. But there was no weakness and change of the motor function. We excised the cystic mass successfully and the neurologic symptoms disappeared.

4.Anesthetic Experience for a Klippel-Feil Syndrome Patient: A case report.

We present a 12-year-old female Klippel-Feil syndrome patient who underwent a surgical correction of thoracolumbar scoliosis. The patient had congenital fusion of the cervical vertebrae (C2,3,4,5,6) and showed the classic triad consisting of a low posterior hairline, short neck, and neck motion limitation. Because these patients may have a risk of neurological damage during airway management and subsequent positioning, anesthesiologists need to take care regarding the cervical spine pathology and degree of instability. In this case, fiberoptic bronchoscope guided intubation was performed. We discuss the anesthetic management of Klippel-Feil syndrome patients with a review of the relevant literature.

5.Acute Renal Failure and Electrolyte Imbalance in Patients with Exertional Rhabdomyolysis.

BACKGROUND: Exertional rhabdomyolysis, although uncommon, is a severe critical illness due to acute renal failure(ARF) and other complications. This study evaluated the clinical, laboratory characteristics of exertional rhabdomyolysis. METHODS: A retrospective study was conducted which examined 26 patients with exertional rhabdomyolysis from Apr. 2000 to Oct. 2001. RESULTS: Fourteen patients(54%) were diagnosed with heat stroke which mostly occurred as a result of a forced march and 12 patients(46%) were diagnosed with exercise-induced rhabdomyolysis which mainly occurred during basic or guerrilla training. All the patients were severely dehydrated and 8 patients(31%) presented with shock. The patients diagnosed with heat stroke were more at risk to shock, seizures, and disseminated intravascular coagulation, than patients who were not diagnosed with heat stroke. Sixteen patients(62%) were diagnosed with ARF(oliguric ARF 9, non-oliguric ARF 7). Seven patients with oliguric ARF received renal replacement therapy. Most patients presented with normal or hypokalemic state in spite of frequent renal failure and metabolic acidosis. Eight patients(31%), mainly diagnosed with heat stoke, were in hypokalemic state and 4 patients(15%) were in hypophosphatemic state. Three patients(20%) died. Of those who survived, three patients suffered from persisting CNS dysfunction. All of them were victims of the heat stroke. CONCLUSION: Acute renal failure was a common complication of exertional rhabdomyolysis. Severe dehydration, shock, and hypokalemia were common during the early course of exertional rhabdomyolysis, especially in patients with heat stroke which was a main, poor prognostic factor of exertional rhabdomyolysis.

6.A Case of Spontaneous Extracapsular Hemorrhage of Intrathyroidal Parathyroid Adenoma.

Intrathyroidal parathyroid adenoma is an infrequent lesion which can be explained by abnormalities during embryonic migration of the parathyroid glands. Extracapsular hemorrhage from a intrathyroidal parathyroid adenoma is extremely rare, however, due to its location near upper airway, it might cause a life-threatening situation. Therefore, if patients presented with anterior neck painful swelling, skin ecchymosis associated with dysphagia and/or hoarseness, extracapsular hemorrhage from parathyroid adenoma should be considered as a possible diagnosis. Estimation of serum calcium level is essential as the presence of hypercalcemia can help to make the diagnosis pre-operatively. As rupture of a parathyroid adenoma and cyst may lead to local dissemination of tumor cells and an increased risk of recurrent hyperparathyroidism, careful longterm follow-up is required. We report an unusual case of spontaneous rupture of intrathyroidal parathyroid adenoma with a review of literature.

7.A Case of Portal Vein Gas Embolism from Hydrogen Peroxide Ingestion.

We present a case of a 19-year-old male with portal vein gas embolism resulting from accidental ingestion of 35% hydrogen peroxide. Hydrogen peroxide is a commonly used germicidal cleansing agent. When it is applied to tissues, catalase causes its rapid molecular decomposition with the release of oxygen bubbles. The patient recovered without sequelae. Hyperbaric oxygenation may be of benefit in patients with respiratory compromise or central nervous system symptoms from gas embolism in hydrogen peroxide ingestion.

9.Analysis of trinucleotide repetitive sequences for Korean patients with spinocerebellar ataxia types 8, 12, and 17.

PURPOSE: Spinocerebellar ataxias (SCAs) are progressive neurodegenerative disorders with diverse modes of inheritance. There are several subtypes of SCAs. SCA 8, SCA 12, and SCA 17 are the less common forms of SCAs with limited information available on their epidemiological profiles in Korea. The purpose of this study was to investigate the prevalence of SCA8, SCA12, and SCA17 in Korea. MATERIALS AND METHODS: Ninety-six unrelated Korean patients were enrolled and showed normal trinucleotide repeats through polymerase-chain reaction (PCR) for the genes ATXN1, ATXN2, ATXN3, CACNA1A , and ATXN7, which correspond to SCA1, SCA2, SCA3, SCA6, and SCA7, respectively. PCR products from patients were further analyzed by capillary electrophoresis using fluorescence labeled primers for the genes ATXN8OS, PPP2R2B, and TBP, which correspond to SCA8, SCA12, and SCA17. RESULTS: Three patients had 104, 97, and 75 abnormal expanded repeats in the ATXN8OS gene, the causative gene for SCA8. None of the patients exhibited abnormal repeats in SCA12 and SCA17. Normal trinucleotide repeat ranges of the cohort in this study were estimated to be 17-34 copies (average, 24+/-4 copies) for SCA8, 7-18 copies (average, 13+/-3 copies) for SCA12, and 26-43 copies (average, 35+/-2 copies) for SCA17. CONCLUSION: This study demonstrated that SCA8, SCA12, and SCA17 are rare in Korean patients with SCA, and further genetic studies are warranted to enhance the mutation detection rate in the Korean SCA population.

Takotsubo cardiomyopathy, also called stress-induced cardiomyopathy, usually occurs in patients with severe emotional or physiologic stress. The prognosis is favorable, and the wall motion abnormlities normalize within weeks. However, stress-induced cardiomyopathy is rarely assosicated with left ventricular thrombus and thromboembolic complications. Here, we report a case of stress-induced cardiomyopathy with left ventricular thrombus that embolized to cause cerebral infarction.
