1.Tinea Incognito Caused by Application of 0.03% Tacrolimus (Protopic(R)) Ointment in Atopic Dermatitis Patient.

We report a case of tinea incognito in a 29-year-old man after applying the 0.03% tacrolimus ointment. He was known atopic dermatitis patient and has been treated with intermittent application of tacrolimus. For his facial pruritus 0.03% tacrolimus ointment was prescribed, and after one week he developed annularly grouped erythematous plaques and patches around the ointment-applied area. The KOH smear revealed multiple fungal hyphae. Dermatologists should be aware of the possibility of topical tacrolimus to be the causative agent for tinea incognito.

2.Pseudomonas Folliculitis Suggestively Caused by Water from Bathing Tub: A Case Report.

Pseudomonas infections can occur from inadequate chemical treatment of water, prolonged exposure to the water source and excessive numbers of bathers in the pool or hot tub. Therefore, pseudomonas folliculitis is also called 'hot tub folliculitis'. We report a 3-year-old boy with Pseudomonas aeruginosa folliculitis suggestively caused by water from the bathing tub of a Hotel. According to the sanitary code of the United States of America, pool staff should regularly check both chlorine and pH levels to prevent swimmers from recreational water infection. However, in Korea, there is no definite regulation regarding chlorine or pH level of recreational water. Thus, we report this issue with a review of the literature.

5.Diagnosis and Preoperative Evaluation of Subungual Glomus Tumors Using Ultrasound.

Subungual glomus tumors are benign neoplasms that arise from the neuromyoarterial apparatus. They present with various clinical symptoms such as localized pain, tenderness, and sensitivity to temperature change and can be palpated as a nodule or mass on physical examination. Pressure erosion can be seen at adjacent bones on radiography. However, in most cases, the tumors are too small to be inspected and located under the nail plate, which makes correct diagnosis and complete excision very difficult. Incomplete removal can cause recurrence or deformity to the residual nail, whereas extensive removal can cause trauma or deformity to the nail plate. Thus, correct differential diagnosis and preoperative evaluation of the location and extent of the tumor are very important. Here, we report 2 cases that were diagnosed and preoperatively evaluated using ultrasound.

Intense pulsed light (IPL) is a high-intensity polychromatic incoherent light of a determined wavelength spectrum, fluence, and pulse duration. Clinicians can combine these variable factors of IPL for the purpose of selective destruction of target chromophores. Due to these properties, IPL has been widely used for numerous indications including hair removal and treatment of vascular or pigmented lesions. For skilled and experienced clinicians, IPL is a great treatment modality; however it can be a source of serious adverse effects when performed by untrained clinicians. Thus, to promote the proper and safe use of IPL, we summarized the origin, history, basic principles, and clinical application of IPL and discussed qualification factors needed for clinicians who use IPL.
