1.Epidermal Glycogen in the Diseases Showing Epidermal Hyperplasia.

In order to assess the epidermal glycogen in the epidermal hyperplastic disease, PAS(Periodic Acid Schiff) stain with or without diastase is used. Epidermal PAS positive reaction is significantly increased in psoriasis (90. 0%), verruca vulgaris (100%) and lichen simplex chronicus (100%) than control group (12, 5%) Epidermal PAS positive reaction is relatively increased in prurigo nodularis(66. 7 %) and chronic contact dermatitis (75%) Strong PAS positive reaction is noticed in 3 of 5 cases (60%) with verruca vulgaris, but none of 22 cases with psoriasis. It is suggested that epidermal glycogen epidermal hyperplasia and that the more glycogen accumulated. than control group (12. 5%) in 3 of 5 cases (60%) with verruca vulaccumulation is accompanied with the hyperplastic the epidermis is, the more glycogen accumulated.

2.A Case of Pityriasis Lichenoides et Varioliformis Acuta Treated with Photochemotherapy.

A lg year-old female patient with pityriasis lichenoides et varioliforrnis acuta (PLEVA) was been treated with PUVA, At first, she was treated with tetracycline for 8 weeks with improvement somewhat. But 2 months later, her skin lesions aggravated slowly. So, we tried to treated her with PUVA and then she was cleared completely. The initial dose of UVA was 5 J/cm The UVA dosage increased by 10g of the previous exposure and increased to the maximum dose, 15 J/cm The patient exposed to UVA R times per week. 12 times after the PUVA therapy, the skin lesions begin to improve and completely improved at 21 times of Pl.JVA therapy. She was followed up every week and there was no side effect except hyperpigrnentation till 24 months. The total dose during treatment is 210 J/cm.

3.Multiple Primary Malignant Neoplasm Combined with Musculoskeletal Malignancy

From Jan. of 1990 to Dec. of 1992, we have experienced three cases of multiple primary malignant neoplasms originated from different histogenesis, in the Department of Orthopedic Surgery of Korea Cancer Center Hospital. Liposarcomas, malignant fibrous histiocytoma were combined with renal cell carcinoma, mucinous cystadenocarcinoma of ovary or prostatic cancer. With Moertel's classification, synchronous tumors were found in 2 cases and metachronous in 1 case. Synchronous tumors were 2 liposarcomas-renal cell carcinoma or mucinous cystadenocarcinoma of ovary. Metachronous tumors were malignant fibrous histiocytoma-prostate cancer. We couldn t find any family history among them. The causes of these multiple malignancies were considered as the individual predilrection to tumors.

4.A Clinical Study on the Hypotensive Effect of Nilvadipine in Patients with Essential Hypertension.

BACKGROUND: As an antihypertensive drug, Nifedipine, a calcium channel blocker was introduced recently, which also has antianginal effect. But due to the relatively short duration of action, another antihypertensive agents having longer duration of action and stronger hypertensive effect were under investigation. Nilvadipine, a new calcium channel blocker, was introduced to have more prologned duration of action and to act more specifically on vascular smooth muscle. So the efficacy and safety of oral Nilvadipine on essestial hypertension was investigated and represented by our institute. METHODS: In order to investigate the efficacy and safety of oral Nilvadipine, daily doses of 4mg twice a day were administered in 30 hypertensive patients whose states were compatible to the criteria : 1) severity of hypertension rated in Stage I and Stage II according to the classification by WHO, 2) ages ranging from 30 to 74 years regardless of sex, 3) blood pressure with 95mmHg or higher but less than 115mmHg in diastolic pressure which was the mean in a sitting position at the last two out of not less three consultations in the 2 week observation period, 4) outpatients with informed consent for 6 weeks. Blood pressure and heart rate were measured every 2 weeks. The complete with blood count with platelet, uronalysis and the electrocardiography were performed at the beginning period and the 6th weeks of therapy. And kinds of side effects were questioned by examining physicians. RESULTS: The following results were obtained : 1) Blood pressure fell significantly in 6 weeks of treatment with Nilvadipine(Mean pressure+/-S.D., 6.00mmHg vs 108.90+/-9.68mmHg p<0.05), 2) There was no significant change in EKG in 6 weeks of treatment with Nilvadipine, 3) Pulse rate was decreased in 6 weeks of treatment with Nilvadipine(80.14+/-11.90/min vs 75.39+/-6.47/min, p<0.05). 4) No significant chsange in body weight was observed(64.50+/-8.7kg vs 63.50+/-10.25kg, p<0.05). 5) There were no significant changes in blood chemistry including blood sugar, cholesterol, electrolytes, serum creatinine and alkaline phosphatase values, 6) Hematologic findings and urinalysis findings reamained unchanged, 7) Total 10 patients(33.30%) had various side effects;facial flushing 30.00%, palpitation 23.33%, headache 20.00%, nausea 10.00%, drowsiness 3.33%, heaviness 3.33% and indigestion 3.33%. But there was no serious side effect that requires to discontinue the medication of the test drug. And there was no need to reduce the dosage due to the side effect, 8) The antihypertensive effect was judged to decrease markedly in 76.70%, decrease 20.00%, unchange 3.30% and increase 0.00%, 9) The utility which was assessed with the data from the overall safety and antihypertensive effect, the drug was judged to be very useful in 60.00%, useful 33.30%, useless 6.67% and inhibited 0.00%. CONCLUSION: From the above results, Nilvadipine in doses of 4mg twice a day was effective and useful in most cases without severe side effects in essential hypertensive patients with diastolic blood pressure of 95 to 115mmHg.

5.A Case of Septicemia Caused by Moraxella lacunata.

Moraxella lacunata is part of the normal flora of the mucosal surface and skin. It is considered to have low pathogenic potential and has been known to cause eye infection but rarely systemic infection. We experienced a case of septicemia due to M. lacunata in a 28-year-old man who was admitted to our hospital due to high fever. He had been well untill 3 weeks earlier when he experienced fever. Physical examination revealed no audible heart murmur and echocardiogram revealed no remarkable abnormalities. Culture of blood revealed M. lacunata. The patient expired due to subdural hemorrhage, 12th days after admission. We report the case with a brief review of related literature.

6.Inhibitory effects of CTLA4-Ig fusion protein on the proliferation of T cell and the antibody production of B cell.

BACKGROUND: Atopic asthma is characterized by activation of Th2-type T cells in the bronchial mucosa. Several reports have suggested an important role for costimulation through the CD28/CTLA4 (cytotoxic T lymphocyte-associated antigen 4)-B7 (CD80/CD86) pathway in allergen activation of T cells in animal models of allergen-induced asthma, because B7-CD28/ CTLA4 interaction can promote the differentiation and development of the Th2 lymphocyte subset. OBJECTIVE: In the present study, we intended to investigate a potential role of humanized CTLA4-Ig on the inhibition of T and B cell activation by blocking B7/CD28 interactions. METHOD: For this purpose we produced humanized CTLA4-Ig fusion protein by transfection to CHO cell and examined its inhibitory effects for activated T and B cell responses. We evaluated the inhibitory effect of MLR (mixed lymphocyte reaction) and con A-stimulated T cell proliferation. And we assayed wheather B cell was inhibited by stimulation of costimulatory signal in LPS-induced B cell response and PFC assay. RESULT: In vitro assay, humanized CTLA4-Ig fusion protein inhibited T cell-specific immune response in dose-dependent manner: CTLA4-Ig inhibited allogeneic stimulation in murine MLR, and the proliferation of T cell by the stimulation of Con A. But CTLA4-Ig did not inhibit directly the proliferative response of B cell by the stimulation of LPS. In addition, in vivo assay, CTLA4-Ig inhibited the production of antibody from B cell, which was presented by plaque-forming cell (PFC) assay. CONCLUSION: These findings suggest that humanized CTLA4-Ig is effective to inhibit the proliferation of activated T cell directly by blocking B7/CD28 costimulation. And humanized CTLA4-Ig influences antibody-producing capacity of B cell indirectly by regulating T cell.

7.Classification and Prognostic Significance of Immunophenotypic Subgroups in Non-T-ALL(I).

BACKGROUND: Recently, the importance of immunologic subclassification based on surface marker expression of acute lymphoblastic leukemia (ALL) has been emphasized. The importance of immunophenotypic analysis of ALL has been documented by numerous studies, but study for survival rate of subgroups in the non-T-ALL was rarely documented in Korea. The aim of this study was to classify the non-T-ALL immunologically, and to compare the survival rates among the subgroups. MATERIALS AND METHODS: From April, 1987 to October, 1997, 142 patients diagnosed as non-T- ALL by immunophenotyping were classified into 6 subgroups, group I to group VI. Review of medical records were undertaken in 130 patients and analysed for survival rates. For the cases in whom CD20 not tested, classification was also undertaken into 4 groups. The survival analysis of immunophenotypic subgroups was carried out by Kaplan-Meier method and differences in survival rates were tested by the log-rank test. RESULTS: The 142 patients, which included patients in whom CD20 was not tested, were assigned into 4 groups as follows: Group I (2 cases, 1%), Group II (24 cases, 17%), Group III-V (110 cases, 78%), Group VI (6 cases, 4%). On the other hand, 113 patients in whom CD20 was tested, were assigned into 6 groups as follows: Group I (0 cases, 0%), Group II (22 cases, 20%), Group III (59 cases, 52%), Group IV (23 cases, 21%), Group V (3 cases, 3%), Group VI (5 cases, 4%). The result showed that median survival time was higher in Group III and Group III-V, and lower in Group VI. However, statistic difference was not seen for 6 subgroups, while in the study for 4 subgroups, statistic difference was seen (p=0.04). And in children, study for 6 subgroups and 4 subgroups showed statistic differences in survival rate. CONCLUSIONS: We conclude that immunophenotypic subclassification of Non-T-ALL is a valuable prognostic marker in children.

8.An Effect of Fibular Fracture in Healing of Tibial Shaft Fracture

Three hundred and fourty two patients with a fracture of tibial shaft were treated and managed in the Department of Orthopaedic Surgery, Soon Chun Hyang University Hospital from January 1981 to December 1985. In 147 cases, 62 cases had only tibial shaft fracture and 85 tibial and fibular shaft fracture. All of these were treated conservatively by manipulation and cast immibilization. We analyzed the initial fracture characteristics, the treatment employed, and the subsequent complications in fracture healing in a series of patients who had sustained a tibial shaft fracture with and without a fibular fracture. The results were as follows; 1. Duration of average bone healing was more slowly in the patients more than twenty years old who were treated for a tibial shaft fracture with and without a concomitant fibular fracture than in the patients less than twenty years old. 2. Duration of average bone healing was more slowly by one or two weeks in the the group of tibial shaft fracture without fibular fracture than in than in those with fibular fracture. 3. The frequency of delayed union, nonunion, varus malunion and pain in the ipsilateral ankle joint were more increased in the group of the tibial shaft fractures without fibular fracture in than those with fibular fracture. 4. The frequency of valgus malunion was more increased in the group of the tibial shaft fractures with fibular fracture than in those without fibular fracture.

9.The Effects of the Several Hormones on the MC3T3 Osteoblast Cells: In Vitro Study

The cause of osteoporosis are multifactorial; these include aging, immobilization, genetic fsctor, initial bone mass, nulliparity, postmenopause, cigarette, etc. Among them the hormonal factors are very important. It is worthwhile to study the effects of various hormones on bne cells. Authors evaluated the effects of TGB-B, 17-B estradiol, insulin, and human growth hormone as a stimulatory factors, and r-interferon as a inhibitory factor on the MC3T3 osteoblast cells with measurement of cell numbers, osteocalcin and 3[H]-thymidine incorporation. 1. TGF-B was a potent stimulator on ostoblast with increased change in cell morphology (number, size, shape), osteoclacin level and 3[H]-thymidine incorporation in dose depen- dant fashion. 2. 17-B estradiol was also a potent stimulator on osteoblast activity as well as TGF-B except osteoclacin level which w#as not shown in dose dependant fashion. 3. There were little changes on osteoblast with insulin, growth hormone, and r-interferon. Through this study it is confirmed that TGF-B and 17-B estradiol showed marked stimulatory effect on osteoblast cell in vitro.

Alveolar soft part sarcoma is a clinically and morphologically distinct soft tissue tumor that was first defined and named by Christopherson et al in 1952. Since 1953, alveolar soft part sarcoma invading bone have been reported sporadically. We experienced a case of alveolar soft part sarcoma with metastasis to femoral shaft, which was treated by wide resection & vascularized fibular strut graft.
