Ossification of the ligamentum flavum (OLF) has been recognised as a definite clinical entity as an ossification of the posterior longitudinal ligament. It has been known that the incidence of OLF is high in Japan and OLF usually occurs in the thoracic and lumbar region. Recently OLF has received considerable attention as a cause of myelopathy. OLF is quite distinct from “hypertrophy” of the ligamentum flvum. We experienced 2 cases of OLF with neurological symptom, which were treated by decompressive laminectomy and removal of the ossified ligamentum flavum. So remarkable symptomatic improvement was obtained.

4.Calcifying mucinous adenocarcinoma of the stomach: report of two cases

There is a great variety of abdominal calcifictions of varying etiology. Among them, calcification in gastriccarcinoma is rare. To the best of our knowledge, there has been no previous report of calcifying mucinousadenocarcinoma of the stomach in Korea. In the present communication, we report two cases of this rare tumor. Thefirst case was 27 years old female who had anorexia and palpable mass in the epigastrium. By palpation, a firmnon-tender large mass ws felt in the epigastrium. A plain abdominal film showed numerous pnuctate calcificationsin the left upper quadrant. Film from an upper G-I series demonstrated findings of advanced gastric carcinoma withmultiple punctate calcifications involving the antrum and body. Gastroscopic biopsy proved the lesion to bemucinous adenocarcioma(signet ring cell type). The second case was 38 years old female who compained of nausea,vomiting and weight loss. On physical examination, she appeared normal. Routine laboratory tests were withinnormal limits. A plain abdominal film revealed stippled calcifications in the left upper quadrant medial to thesplenic shadow. The film from an upper G-I series showed a mass in the fundus and upper body of stomach withmultiple stippled calcifications along the lesser curvature. Subtotal gastrectomy was perfomred and the pathologicfindings was calcifying mucinous adenocarcinoma(signet ring cell type).

6.Congenital Agenesis of Odonteid Process: A Case Report

Anomalous development of the odonteid precess is an uncommon clinical occurence that can seriously impair the integrity af the atlantoaxial articulation. Absence of the odontoid process may be either congenital or acquired, but clinically the etiology is of little importance as the clinical signs and symptoms and the treatment are identical in both. We experienced a case of congenital absence of the odontoid process and good result was obtained from the posterior atlantoaxial fusion with hale-cast.

7.Epithelioid Leiomyosarcoma of Skin: A case report.

This report concerns a rare malignant smooth muscle neoplasm of the skin and the subcutaneous tissue, which was examined immunohistochemically and ultrastructurally. It occured in a 48-year-old female patient who had suffered from painful nodules on the left shoulder. The nodules were removed and diagnosed as a benign fibrohistiocytic tumor. One year after local excision, multiple nodules were detected by physical examination at the previous operation site. Grossly, thirteen well-defined small nodules had spread in the dermis and the subcutaneous tissue, of which nine were located from lower dermis to the subcutaneous tissue and four were in the subcutaneous tissue. Histologically, each nodule consisted mainly of epithelioid tumor cells having eosinophilic or clear plump cytoplasm and round to oval nuclei with small nucleoli, and some peripheral spindle tumor cells. The tumor cells were negative for PAS reaction with and without diastase digestion or alcian blue. Immunohistochemically, tumor cells were positive for smooth muscle actin and vimentin. Ultrastructurally, the tumor cells showed actin filaments and dense bodies in cytoplasm. This case was diagnosed as an epithelioid leiomyosarcoma of the skin and the subcutaneous tissue with local metastasis one year after local excision.

8.The Usefulness of Endo-rectal Coil MRI in the Staging of Clinically Localized Prostate Cancer.

PURPOSE: We evaluated the ability of endo-rectal coil MRI (ER-MRI) to predict the local pathological stage of prostate cancer prior to radical prostatectomy and compared the results with those of transrectal ultrasonography (TRUS). MATERIALS AND METHODS: ER-MRI using high field magnets (1.5 Tesla) were performed in 22 patients (mean age 62.8 years, range 51-73) with clinically localized prostate cancer before radical prostatectomy. Of the 22 patients, 17 patients were also assessed by TRUS. The results of the imaging techniques were compared with the post-operative histopathological findings. As one patient with pelvic lymph node metastasis, which was detected on frozen-section examination during surgery, was spared radical prostatectomy, the final evaluation included 21 patients. RESULTS: DSeven of the 21 patients (33%) were found to have extraprostatic extension (EPE), and 5 had seminal vesicle invasion (SVI). The sensitivity and specificity for diagnosing EPE using ER-MRI were 62.5% and 84.6%, respectively, and 16.7% and 100% with TRUS. The sensitivity and specificity for diagnosing SVI were 80.0% and 93.8%, respectively with ER-MRI, and 0% and 92.3% with TRUS. The accuracy of predicting SVI was 90.5% with ER-MRI compared to 70.6% with TRUS. CONCLUSIONS: ER-MRI was significantly better than TRUS for determining the local extent of prostatic cancer and for prediction of SVI in the preoperative staging of clinically localized prostate cancer.
