4.Intramedullary Nailing for Pathological Fractures of the Proximal Humerus.

BACKGROUND: Endoprosthetic reconstruction is widely applied for pathological fractures of the proximal humerus; however, functional impairment is usually unsatisfactory. The aims of the current study are to evaluate the reliability of interlocking intramedullary (IM) nailing with cement augmentation as a fixation method in proximal humeral lesions and to assess functional outcomes. METHODS: We reviewed 32 patients with pathological fractures of the proximal humerus who underwent interlocking IM nailing and cement augmentation. Functional scores and pain relief were assessed as outcomes. RESULTS: The mean follow-up period was 14.2 months. The mean Musculoskeletal Tumor Society functional score and Karnofsky performance status scale score were 27.7 and 75.6, respectively. Improvement of pain assessed using the visual analogue scale was 6.2 on average. Thirty-one patients (97%) experienced no pain after surgery. The mean ranges of forward flexion and abduction were 115° and 112.6°, respectively. All patients achieved stability and had no local recurrence without failure of fixation until the last follow-up. CONCLUSIONS: Proximal interlocking IM nailing with cement augmentation appears to be a reliable treatment option for pathological or impending fractures of the proximal humerus in selected patients with metastatic tumors, even with extensive bone destruction.

5.The Effect of Revision Endoscopic Dacryorhinostomy (EDCR).

PURPOSE: The purpose of this study was to evaluate the effects of revision endoscopic endonasal dacryocystorhinostomy (EDCR) and the causes of postoperative failure after primary endoscopic DCR in patients with nasolacrimal duct obstruction. METHODS: This prospective study enrolled 32 patients (32 eyes) who had undergone revision of endoscopic DCR with the diagnosis of obstruction of nasolacrimal system after primary EDCR between October 1997 and February 2003. RESULTS: The most common cause of revision operation after primary EDCR was granuloma, followed by membranous obstruction and common canalicular obstruction. The success rate of the cases with granuloma was 69.2%, and that with membranous obstruction was 100%. CONCLUSIONS: Revision EDCR success rate can be improved by reducing the frequency of granuloma as this was the most common factor for revision EDCR. The effect of antimetabolites in revision EDCR success rate is to be further evaluated.

6.Torsional Characteristics between Single and Double Distal Screws in the Interlocking Intramedullary Nailing of Humeral Shaft Fracture.

The use of interlocking intramedullary nail is accepted one of treatment choices in the comminuted fractures of humeral shaft. The insertion of distal interlocking screws remains technically problematic. The use of intrageon's hands during the procedure. In order to reduce technical difficulty and radiation exposure, it is necessary to compare the rigidity of intramedullary nail according to the number of distal interlocking screws. The purpose of study is to compare the stability of interlocking intramedullary nail according to the number of distal screws by means of torsional compliance measurements in the simulated humeral shaft fractures. Simulated fractures were made in 20 humora from 10 cadavera at the mid-junction of humeral shaft. All humora were fixated with titaium humeral nail system. Interlocking screws were placed at proximal and distal screw holes by standard procedure. Group I consisted of 10 humora fixated with one distal interlocking screw and group II consisted of 10 humora fixated with two distal intterlocking screws. Torsional compliance was measured with single-end of 10 humora fixated with two distal interlocking screws. Torsional compliance was measured with single-end double arm torquing machine. The torsional compliance analog was 0.0294+/-0.0033 mm/N mm for one screw and 0.0241+/-0.0045 mm/N mm for two distal screws. The torsional compliance analog between two groups was found to be statistically insignificant(p=0.23). In conclusion, One distal interlocking screw was not inferior to two interlocking screws in terms of biomechanical characteristics, especially torsional compliance analog.

7.Clinical Analysis of Patients Who Refused a Blood Transfusion.

PURPOSE: Non-blood transfusion is of increasing interest as more patients are refusing a blood transfusion because of religious belief, infection, or fear of a blood-transmitted disease such as AIDS. This study analyzed clinical findings to help the treatment and management of patients who want a non-blood transfusion. METHODS: Of the 83 patients who visited the non-blood transfusion center in Chosun University Hospital from June 2001 to December 2003, 59 patients had a sufficient clinical record, and these were reviewed retrospectively. We investigated sex, age, the reason for the visit, the reason for refusing a blood transfusion, the degree of anemia, the necessity of the blood transfusion, the histories of transfusion and attempted non-blood transfusion, and the histories of visits to emergency departments and the outcomes. RESULTS: The male-to-female ratio was 1:2.93, and the most prevalent ages of the patients were in the forties (26 patients) and thirties (10 patients). The reasons for refusing a blood transfusion were religion (48 patients, 81.4%), risk of infection (1 patients, 1.7%), and other (10 patients, 16.9%). Among the specialty departments, the proportions of patients from the departments of obstetrics and gynecology, internal medicine, general surgery and orthopedics were 40.7% (24 patients), 25.4%, 11.9%, and 6.8%, respectively. Of the patients, 28 patients (47.5%) were admitted directly to the emergency department, and 39 patients (66.1%) were operated on. A blood transfusion was considered to be necessary for 16 patients (27.1%), but only a 2 patients (3.4%) actually received a blood transfusion. Of those 14 patients, 3patients (21.4%) died. After admission, 17 patients (29.8%) received a non-blood transfusion. CONCLUSIONS: Most of the patients visited the non-blood transfusion center for religious reasons, and the majority of the patients were admitted from the Department of Obstetrics and Gynecology. Many of the patients medically should have received blood transfusion, but only a few actually did.

8.Mandibulotomy, A Surgical approach for Oral cancer: Its Complications and contributing factors.

We reviewed 56 patients who received mandibulotomy at Yonsei medical center between 1989 and 1999. We also analysed the complications associated with mandibulotomy and its contributing factors. The complications occur in 16 patients(28.6%) and are classified into two categories; intraoperative and postperative complications. Nonunion was observed in 5 patients and osteoradionecrosis in 5. The patients who received preoperative radiation therapy were more tend to develop nonunion and osteoradionecrosis. This study suggests the benefits of mandibulotomy as a surgical approach to oral cancer: 1. Paramedian osteotomy was recommended for preservation of neurovascular bundle and ease of surgical access. 2. By using thin saw blade, reapproximation was improved with minimal bone loss. 3. osteotomy on anterior mandible which lies outside the usual portals of radiation therapy decreases the incidence of osteoradionecrosis.

9.A Study on the N-methylformamide Excretion Rate of Workers at Synthetic Leather Factories in Korea.

This study was conducted to examine the excretion rate of dimethylformamide (DMF) from the workers exposed to DMF. The study was done at two synthetic leather factories located in Kyeonggi-do from the period of May 2 to 30, 1996. N-methyl- formamide (NMF) concentrations in urine were measured and compared by the three exposure level of DMF in air. The mean concentration of the Low (dry and winder part). Moderate (rinsing part) and High (mixing and coating part) exposure group were 3.99+/-3.54. 10.19 +/-5.69 and 32.10+/-7.87 mg/m3 during workshift of 8 hours, respectively. The mean concentration of urinary N-methylforinamide (NMF) were 2.13+/-2.58, 11.16+/-4.98 and 26.24 +/-7.35 mg/g creatinine, respectively. The concentration of NMIF in urine could reach to maximum in 3 hours and was reduced nearly to zero in about 18 hours after exposure to DMF.

10.Studies on Activities of Xenobiotic Metabolic Enzymes in Rats treated with Trichloroethylene.

Trichloroethylene (TRI) is one of the most widely used organic solvent in many industries. It is used a degreasing agent, extract and dillution solvents. TRI is metabolized by liver microsomal cytochrome P4SO to reactive intermediates, which are considered responsible for its hepatotoxicity. Cytochrome P450 is a key enzyme in the first step of TRI metabolism resulting in chloral hydrate (CH) formation, which is a rate-limiting step in TRI metabolism. This study was perfomed to find out the change of hepatic cytochrome P450 levels and of electron transport system for P-450 dependent reaction. Intraperitoneal injections of TRI were done to the male Sprague Dawley rats(mean body wights,170+/-10g) in corn oil at the dosage of 150,300,600 mg/kg for 2 days. The results of experiments are following : 1. The contents of microsomal protein, cytochrome P450 and b(5) are tended to be decreased with increasing amount of TRI, but not significantly (p>0.05) . 2. The activity of NADPH-cytochrome P450 reductase is slowly decreased by increasing amount of TRI, but not slgnificantly (p>0.05) . On the other hand, the activity of NADH-b(5) reductase is increased with increasing amount of TRI (p<0.05) 3. The activity of 7-ethoxyresolufin 0-deethylase did not show any critical changes (P>0.05) . This result suggests that the inductive pattern of TRI may not be related to P 450IAl and P450IIB.
