1.Comparison of Surgical Results Between Bilateral Rectus Muscle Recession and Lateral Rectus Muscle Recession and Medial Rectus Muscle Resection in Exotropia Over 40 Prism Diopters.

It is the purpose of this study to compare the postoperative results of 168 patients with exotropia of basic and simulated divergence excess type over 40 prism diopters; 86 patients had bilateral rectus muscle recessions and 82 patients underwent lateral rectus muscle recession and medial rectus muscle resection. The patients in the former group had averaged 13.7 years of age and 8.2 months of postoperative follow-up period. Those in the latter group averaged 22.9 years and had been followed up for 7.1 months. The postoperative success rates are as follows: 66.7%(57/86) in the bilateral rectus muscle recession group and 68.1%(58/82) in the lateral rectus muscle recession and medial rectus muscle resection group(P>0.05). And the success rates of those older than 16 years without amblyopia are 60.0%(18/30) in the former group and 75.0%(24/32) in the latter group(P>0.05). The lateral rectus muscle recession and medial rectus muscle resection group showed somewhat higher rates of success even though it proved statistically insignificant.

2.Rabbit Corneal Endothelial Cell Damage by Q-switched Nd:YAG laser.

Corneal endothelial damages following Nd: YAG laser application were evaluated rabbits. The first group underwent anterior capsulotomy, while the second group received laser applications at the corneal center and anterior capsule. The rabbits were evaluated with the intraocular pressure, the corneal endothelial cell number, the thickness of the corneal center, and the morphologic change of the cornea, before laser application and at postaplication 1 day, 7 day, and 14 day. But the endothelium was damaged when laser shots were applied at the cornea center and anterior caps ulotomy. In ophthalmologic procedure with Q-switched Nd:YAG laser, the minimal effective poser should be selected, the laser beam should be applicated on extremely fine focus and contact lens should be used to minimize the corneal endothelial injury.

3.The Association between Unexplained Second-Trimester Human Chorionic Gonadotropin Elevations and Pregnancy Outcome.

We conducted this cohort analytic study to determine whether women with unexplained elevations of maternal serum hCG at 15-18 weeks' gestation are at increased risk for pregnancy complications and adverse perinatal outcomes. The inclusion criteria were a singleton gestation, a confirmed gestational age, and an hCG level greater than 2.0 multiples of the median (MoM). The exclusion criteria were fetal anomalies, an abnormal karyotype, molar pregnancy, and an MSAFP level greater than 2.5 multiples of the median (MoM). A group of randomly selected women with hCG levels under 2.0 MoM served as controls. Patients with elevated levels of hCG had a significantly higher risk for PIH (17.9% versus 4.5%; P <.05) and preterm delivery (17.9% versus 3.5%; P<, 05) than control. But no significant differences were observed in the incidence of intrauterine growth restriction and low birth weight and in the newborn weight. We suggested that pregnancies with unexplained elevated hCG levels should be regarded as high-risk pregnancies. And these patients require careful monitoring with adequate obstetric management.

4.Prognostic Factors and Treatment Outcome for Thymoma.

PURPOSE: In this retrospective study, we attempted to evaluate the treatment outcome and the prognostic factors of thymoma treated with surgery, radiotherapy and chemotherapy. METHODS AND MATERIALS: Between 1979 and 1998, 55 patients with thymoma were treated at the Seoul National University Hospital. Of these, 11 patients underwent surgery only, 33 patients received postoperative radiotherapy and 11 patients received radiotherapy only. Twenty-three patients had gross total resection and 21 patients subtotal resection. For postoperative radiotherapy, the radiation dose consisted of 41.4-55.8 Gy. The average follow-up was 64 months, and ranged from 2 to 160 months. The sex ratio was 1:1 and the median age was 48 years (15-74 years). Overall survival and disease-free survival were determined via the Kaplan-Meier method, and the log-rank was employed to evaluate for differences in prognostic factor. RESULTS: The five- and 10-year survival rates were 87% and 65% respectively, and the median survival was 103 months. By univariate analysis, only stage ( p=0.0017) turned out to be significant prognostic factors of overall survival. Also, stage ( p=0.0007) was significantly predictive for overall survival in mutivariated analysis. CONCLUSION: This study showed the stage was found to be important prognostic factors, which influenced survival. Especially, as incomplete resection is related with poor results, complete resection is important to cure the invasive thymoma.

5.Impact of Physician Delay on Postoperative Outcome of Patients with Acute Appendicitis.

BACKGROUND: It is controversial whether the delay of surgery for acute appendicitis by physician results in higher morbidity. Our present study explores this problem of physician delay on the postoperative outcome of patients with acute appendicitis. MATERIAL AND METHODS: Among 432 patients admitted for the presumptive diagnosis of acute appendicitis between Jan., 1995 and Dec., 1997, 358 patients with pathologically proven acute appendicits were analyzed. Physician delay from the hospital admission to the surgery was evaluated in relation to the stage of the disease at operation and postoperative outcome. RESULTS: Postoperative complications occurred signifficantly higher in advanced appendicitis group, than in simple appendicitis group. Postoperative oral consumption started significantly later and hospital days are significantly prolonged in those group. Both patient and physician delays significantly affect the stage of the disease. The finding that physician delay correlated with the stage, however, was denied by multivariate analysis. CONCLUSION: On the contrary to the patient delay, physician delay does not affect the stage of the acute appendicitis nor lead to increased incidence of postoperative complication. Physician delay to clarify the diagnosis is a reasonable strategy and it does not affect the outcome adversely.

6.A Case of CHAP Syndrome.

CHAP syndorme consists of choreoathetosis, orofacial dyskinesia, affective change, hypotonia, and pseudobulbar signs. CHAP syndrome was firstly des- cribed with onset 1 to 7 days after induced profound hypothermia and complete circulatory arrest for con- genital heart lesion repair. Since then, only a few cases have been reported until now. Case We report a patient who demonstrated all of the elements of the CHAP syndrome in severe cachexic state. Brain SPECT(99mTC-ECD) shows decreased perfusion in left fronto-parieto-temporal lobe. Comments The pathogenesis of CHAP syndrome is obscure. However our case led us to suggest that more essential component pre- disposing to the CHAP syndrome is hypoxia rather than hypothermia. Furthermore, it seems to be related with circulatory insufficiency in extrapyramidal nervous systems.

7.Changes of mRNA Expression of Dopamine Receptor in the Visually Deprivated Rat Striatum.

PURPOSE: To investigate the changes of the dopaminergic system in rat striatum after visual deprivation during a critical period of postnatal development. METHODS: The changes of dopamine D1 and D2 receptor (Rc) mRNA were investigated by using Northern blot analysis, in situ hybridization in the rat striatum. The right eyelid of visually deprivated rat was sutured at 10 postnatal days. After visual deprivation for 4 weeks, rats were sacrificed and striatum tissues were removed for analysis. RESULTS: By Northern blot analysis and in situ hybridization, decreased expression of D1 and D2 Rc mRNA was noted in the ipsilateral striatum to the deprivated eye. CONCLUSIONS: These results have shown that visual deprivation during a critical period of postnatal development influences the dopaminergic system in the striatum.

The importance of tick-borne diseases is increasing because of climate change, with a lack of long-term studies on tick-borne pathogens in South Korea. To understand the epidemiological characteristics of tick-borne diseases, the monthly distribution of field ticks throughout the year was studied in South Korea between May 2014 and April 2018 in a cross sectional study. The presence of various tick-borne pathogens ( During the study period there were 11,717 ticks collected and 4 species identified. In conclusion, due attention should be paid to preventing tick-borne infections in humans whilst engaged in outdoor activities in Spring and Autumn, particularly in places where there is a high prevalence of ticks.
9.Neuroprotection Effect of Ascorbic Acid in Model of Ischemic Retinal Injury.

PURPOSE: An attempt to establish the neuroprotection effect of ascorbic acid in ischemic retinal injury. METHODS: A rat anterior chamber was cannulated with a 30-gauge infusion needle connected to a container of balanced salt solution (BSS). The container was raised to a height of 3m during 60mins, thereby inducing retina ischemia by high intraocular pressure (HIOP). The effect of ascorbic acid was demonstrated by hematoxylin-eosin (H and E) stain and neuronal nitric oxide synthase (nNOS) immunohistochemical stain. The effect was classified into three HIOP-induced groups: without ascorbic acid treatment, ; ascorbic acid-treated at 24 hr and just before experiment; and ascorbic acid-treated at 24hr, just before experiment and every 24hr after experiment. RESULTS: After retinal ischemia, in the ascorbic acid-treatment groups (Ed- confirm the plural here; from above there are two ascorbic acid-treated groups), the thickness of each retinal layer was preserved more than that in the non-ascorbic acid treated group. There was little difference according to ascorbic acid administration time and period. By nNOS immunohistochemical stain, in the non-ascorbic acid administration group, nNOS immune reactive cells were increased remarkably, in the Inner Nuclear layer (INL) and Ganglion Cell Layer (GCL). In the ascorbic acid-treated groups (Ed-agin confirm), nNOS immune reactive cells were stained in a similar pattern to that of the control group. CONCLUSIONS: The neuroprotection effect of ascorbic acid in ischemic retinal injury was demonstrated, as was the suppression of nNOS expression in the ischemic retina tissue by ascorbic acid administration.

10.Cerebral Arterial Embolism Treated by Intra-Arterial Infusion of Urokinase Which Was Occurred during Percutaneous Balloon Mitral Valvuloplasty.

Cerebral arterial embolism which resulted in total occlusion of right proximal middle cerebral artery was occured in 33 years old female patients with severe mitral stenosis and atrial fibrillation during percutaneous mitral balloon valvuloplasty(PMV). The occluded artery could be detected by transfemoral carotid angiography and recanalized immediately by intra-arterial infusion of urokinase. There were no cerebral hemorrhage by CT scan after intra-arterial urokinase infusion and the patients discharged without any neurologic sequale 7 days after thrombolysis. We report a case of successful recanalization with intraarterial infusion of urokinase in patient with cerebral arterial embolism occurred during PMV.
