The osteoporosis is frequently observed in the patients with the inflammatory arthritis and painful rheumatism. The treatment of the osteoporosis for them is different from that for the patients without the arthritis or rheumatism. The recently developed biologic agents blocking tumor necrosis factor, interleukin(IL)-1, IL-6, or receptor activator of nuclear factor-kappaB ligand (RANKL) which are designed to treat the inflammatory arthritis are also expected to heal the osteoporosis in the inflammatory arthritis. The early use of the bisphosphonate is useful to prevent the glucocorticoid induced bone loss and to treat the spondyloarthropathy including the SAPHO (synovitis, acne, pustulosis, hyperostosis, osteitis) syndrome. The clacitonin is useful for the painful rheumatism and osteoporotic fracture. The estrogen replacement is disputed because the stroke is known to occur more commonly in chronic inflammatory rheumatism than in general population. Moreover the pathogenesis of the most rheumatism may be partly related to the hormone. The parathyroid hormone therapy needs caution in the patients with calcium deposition disease and the hyperuricemia. We are reviewing the recent trend and development in the management of the primary, secondary and inflammatory osteoporosis in the patients with arthritis.

Acromelanosis progressiva is a peculiar progressive pigmentary disorder characterized by intensely dark and sharply demarcated hyperpigmentation over the dorsa of fingers and toes, and the pigmentation rapidly spread proximally. A 19-year-old female had sharply demarcated symmetric dark aroarn macules on the dorsa of her hands and feet, forearms, lower legs, face and neck. These pigmentations at first appeared at the age of 5 on the dorsa of her fingers and toes, thereafter these pigmertatons spread progressively to the proximal portion of her body. Histologically, a proliferation of mela biocyte at the epidermal-dermal junction was seen, associated with mild hyperkeratosis. She was othervise normal on physical examination and had no family historv of consanguinity.

BACKGROUND: The regional fat distribution is known to be related with some cardiovasular risk factors independently to total body fat amount. Regional fat distribution pattern can be assessed by waist to-hip ratio(WHR). WHR distribution patterns depend on age, sex, race and show regional difference. But, our own unique data are inadequate, so we tried to investigate distribution patterns, cut off points and related factors of WHR in middle aged men and women among Korean. METHODS: The study population were middle aged men and women above 40 years old who visited our hospital for routine check up from April to July, 1996. We measured height, weight, blood pressure, waist circumference, hip circumference, fasting blood sugar and total cholesterol in 1,000 persons. We gathered data about diet habits, exercise, smoking and drinking status by standard questionnaire. We analized above data by using SAS for Window 6.10. RESULTS: The study population numbers of men and women were 494 and 499, and average ages of men and women were 49.3 and 50.5 years respectively. For men, the average WHRs as a whole were 0.895, and 0.893, 0.893 and 0.911 in their forties, fifties and above sixties respectively. For women, the average WHRs as a whole were 0.827, and 0.816, 0.837 and 0.850 in their forties, fifties and above sixties respectively. The percentage above WHR 1.0, 0.95, and 0.90 in men were 2.4%, 11.7% and 46% and the percentage above WHR 0.90, 0.85, and 0.80 in women were 8.2%, 32.9% and 68.7%. Adjusting by age, body mass index, WHR was significantly correlated with weight, systolic blood pressure, diastolic blood pressure in men only. For men, WHR was associated with alcohol drinking positively and with exercise negatively. For women, WHR was associated with alcohol drinking and smoking status positively. CONCLUSIONS: The WHR had a tendency to increase according to age. The 90th percentile points in men and women were 0.957 and 0.894 respectively. WHR was related with several cardiovascular risk factors and several life styles.

Malposition of the extracorporeal membrane oxygenation (ECMO) venous cannula in the azygos vein is not frequently reported. We hereby present such a case, which occurred in a neonate with right-sided congenital diaphragmatic hernia. Despite ECMO application, neither adequate flow nor sufficient oxygenation was achieved. On the cross-table lateral chest radiograph, the cannula tip was identified posterior to the heart silhouette, which implied malposition of the cannula in the azygos vein. After repositioning the cannula, the target flow and oxygenation were successfully achieved. When sufficient venous flow is not achieved, as in our case, clinicians should be alerted so they can identify the cannula tip location on lateral chest radiograph and confirm whether malposition in the azygos vein is the cause of the ineffective ECMO.

Meigs' syndrome is defined as a hydrothorax with ascites and a pelvic tumor, both of which resolve on removal of the tumor. Pseudo-Meigs' syndrome is a variant not possessing the original tumor cell types described by Meigs. Both these syndromes should be considered in otherwise healthy women who present with either new or recurrent hydrothorax and ascites. Pseudo-Meigs' syndrome occurs with the clinical triad of (1)ascites, (2)pleural effusion and (3)Brenner tumors, struma ovarii, benign thecomas, extreme ovarian edema, uterine leiomyomas or other benign pelvic tumors. A case of Pseudo-Meigs' syndrome associated with Brenner tumor is presented with a brief review of literatures.

We report a case of Undifferentiated carcinoma of the prostate in a 66 years old male resulted in severe chronic anemia (Hb value: 2.9gm/100dl, Hct:9%). The patient was treated with multiple blood transfusions, diethylstilbestrol and cystostomy with good improvement of general condition.
