Crohn's disease is a rare inflammatory bowel disease in Korea which requires a differential diagnosis with intestinal tuberculosis. In the early stages of Crohn's diseaae, the combiation of bowel wall edema and spasm produces intermittent obstructive manifestations, so the usual subjective symptoms were appeared long standing diarrhea, low grade fever and abdominal pain and distension. We recently experienced a case of Crohn's colitis with repeated bowel obstruction in female adult who underwent surgical resection.

3.The short-term effect of a comprehensive smoke-free policy on hospital employees.

BACKGROUND: Because passive smoking causes lung cancer, ischemic heart disease, and stroke in nonsmokers, implementation of policy to prevent workplace environmental tobacco smoke (ETS) are deadly needed. Most of our hospital employees desire more strict smoke-free policy as shown in a questionnaire survey conducted in January 1999. This study was to investigate the short-term effect of a comprehensive smoke-free policy on hospital employees which prohibited any smoking within the hospital building since March 15, 1999. METHODS: The first survey was carried out on January 1999 including all the employees of Sanggye Paik Hospital as subjects. After two months a second round of survey was done to monitor the effects of the new regulation. Questions included in the survey were subjects' demographic characteristics, smoking history, perception and observance of no-smoking areas, and change in perception of ETS. Smokers were questioned about changes in smoking rate after the new policy. Statistical analysis was done using and paired t-test to detect if there was any statistical significance after enforcing the new smoke-free policy. RESULTS: Among total 1,026 employees, 621 subjects (men 146, women 475) in the first survey and 552 subjects (men 146, women 406) in the second survey were responded. There was no significant difference in demographic characteristics and occupation mix between the two surveys. Although the perception and observance of no-smoking areas were increased after the new policy, half of nonsmokers replied that the new policy was not well observed. 76.9% of subjects replied the exposure to cigarette smoke during work decreased. The question item asking the smokers of the amount of cigarettes smoked after the new policy adopted revealed a decrease in 2.83( 0.48) cigarettes during the working hours(p=0.0001), a decrease in 1.18( 0.29) cigarettes during the non-working hours(p=0.0002). However, 84.0% of nonsmokers agreed on total ban within the premises of hospital or more stringent regulation for violation of the policy. CONCLUSION: Despite several limitations, this study suggests that a comprehensive smoke-free policy in hospital has a positive impact on the reduction of smoker's cigarettes consumption as well as reduction of ETS exposure on the nonsmokers.

5.Two cases of conservative management of 10 and 8 week cervical pregnancies.

Cervical pregnancy is a rare and dangerous form of ectopic pregnancy in which the blastocyst implants within the cervical canal below the internal os of the uterine cervix. The characteristic clinical sign is a severe asymptomatic uterine bleeding in early pregnancy or during curettage. Most cervical pregnancies result in early spontaneous abortion, complicated by severe hemorrhage from the eroded blood vessels within the cervical tissue. Because of uncontrolled profuse vaginal bleeding, total hysterectomy has been the mostly necessitated to control life-threatening bleeding. Transvaginal sonography allows early diagnosis, and conservative treatment (two-time treatment : first treatement with systemic or intraamnionic methotrexate, secondly with curettage or ligature of the uterine arteries) improves the patient's fertility. Successful treatment is defined as elimination of the cervical pregnancy with preservation of the uterus. We report two cases of cervical pregnancy managed, successfully with methotrexate, leucovorin and curettage.

6.A Case of Idiopathic Long QT Syndrome(LQTS).

The idiopathic long-QT syndrome is an infrequent inherited disorder, characterized by prolonged QT interval and by the occurrence of life-threatening tachyarrhythmia, particularly in association with emotional or physical stree. In its characteristic presentation, with obvious QT prolongation and stree induced syncope with torsades de points, the diagnosis is straightforward for physicians aware of the disease. But sometimes in cases of borderline QT prolongation and vague symptom, a correct diagnosis is delayed and frequently misdiagnosed as a seizure disorder. The mortality of untreated symptomatic patients with LQTS exceeds 20% in the year after their first syncopal episode and aapproaches 50% within 10 years. But this high mortality rate has been significantly reduced by the use of pharmacological or surgical antiadrenergic therapy or both. So early detection of the disease and antiadrenergic treatment sympathetic innervation to the heart. Second, there was intrinsic gene abnormality in the mechanisms responsible for cardiac repolarization. We report one case of LQTS in a 24 years old female patient who had been suffering from stree related syncope. Her initial EKG had prologed QT interval(588msec) and increased QT intrval dispersion(200msec). In the Holter monitor, long and short cycle sequence induced (pause-dependent) torsades de pointes was detected. She has been treated by beta-blocker and has maintained her condition without recurrence of the symptoms.

7.Clinical Features of Stenotrophomonas Maltaphilia Infection.

OBJECTIVE: Stenotrophomonas maltophilia has been emerging as an important nosocomial pathogen in recent years in patients with impaired host- defense mechanism or who has been exposed to large amount of inocula. This organism is usually resistant to multiple (commonly used) antimicrobial agents, particularly to those of the beta-lactam class. To evaluate the clinical feature of Stenotrophomonas maltophilia infection and in vitro anti- microbial susceptibility, we performed a retrospective study. METHODS: We analyzed the result of in vitro antimicrobial susceptibility test for 200 isolates of S. maltophilia and the annual isolation rate during the period between January 1990 and December 1994 in our institution, and performed a retrospective study for the available records of 165 cases among them. The data were obtained with only the first isolation of the organism for each patients. RESULTS: Total of 165 initial isolates, the isolates were from wounds in 50(30.3%), urine in 47(28.5%), the respiratory tract in 37(22.4%), blood in 9(5.5%), bile in 6(3.6%), and miscellaneous sources in 16(9.7%). The 84.2% of isolates were hospital-acquired isolate and 58.3% of these patients had received antecedent antibiotic therapy: polymicrobial growth was demonstrated in 61.9% of the cases. In vitro antimicrobial susceptibiiity test, ofloxacin was active against the isolates in 89.2%, moxalactam in 85.9%, ciprofloxacin in 83.9%, TMP-SMX(trimethoprim-sulfamethoxazole) in 64.2%, As expected, S. maltophilia isolates were, in general, not susceptible to cephalosporins, penicillins. The annual isolation rate at Kyung Hee University hospital was not increased significantly from 1990 to 1994, 19.53 per 10,000 patients dismissals in 1990, 13.56 in 1994. The major underlying diseases of patients were malignancy(17.6%), cerebrovascular disorder(17%), diabetic mellitus(13.3%). Mortality rate is 10.3%. CONCLUSION: S. maltophilia has been emerging as an important nosocomial pathogen in immunocompromised patients, especially those receiving broad-spectrum antimicrobial therapy. And this organism is resistant to multiple antimicrobial agents, particularly to those of the beta-lactam class. When antimicrobial treatment is necessary, the clinician should be guided by results of in vitro susceptibility testing because of the notable in vitro resistance of S. maltophilia to commonly used antibiotics. And when S. maltophilia has been recovered from a patient, wound and contact isolation is warranted.

Congenital cutis laxa is a rare inherited disorder of connective tissue manifested by loose and hanging skin, resulting in a prematurely-aged appearance. Cutis laxa may be inherited in an autosomal dominant, autosomal recessive and X-linked recessive pattern or an acquired form. In this report we describe a patient with congenital cutis laxa and growth retardation. The skin showed loose folds, wrinkling, and sagging of the face, abdomen, and both thighs. His weight and height were below the 3rd percentile. He had no family history for this skin disease.
