1.Intralesional Recombinant Alpha-2a Interferon for the Treatment of Patients With Verruca.

BACKGROUND: Interferon alpha-2a has already been shown to be effective in clinical use of virus-originated diseases such as hairy cell leukemia, condyloma acuminatum, and AIDS-related Kaposi's sarcoma. The use of recombinant alpha-interferon may allow common warts to be treated relatively atraumatically and with less incidence of recurrence. OBJECTIVE: We tried to determine the safety and effectiveness of intralesional injections of recombinant alpha-2a interferon in the treatment of patients with common warts. METHODS: A single wart on each patient was weekly injected with 0.75 to 1.5×10(5) IU/25mm2 of interferon for 8 weeks, and the response to treatment was followed up-to 6 months. RESULTS: Clearing of the treated wart at the end of treatment occurred in 5(71%) out of 7 patients and the rest showed no improvement. With evaluation for relapses up-to 6 months after treatment, warts relapsed in 2(40%) out of 5 patients. Therefore, 3(43%) out of 7 patients were completely free of warts 6 months after treatment. CONCLUSION: Intralesional recombinant interferon alpha-2a has a limited therapeutic effect, but may be considered as a therapeutic modality of recalcitrant verruca or when it can be anticipated that destructive techniques or blistering agents will not be tolerated.

2.Studies on the VP4 and VP7 Genes of Bovine Rotaviruses from Field Samples Using RT-PCR and RFLP Analysis.

Characterizations of the VP4 (P type) and VP7 (G type) genes of Korean isolates of bovine rotavirus were performed using RT-PCR/RFLP and nucleotide sequencing analysis. After RT-PCR amplification of partial length (1094bp) of the VP4 and full length (1062bp) of the VP7 genes, amplified PCR products were digested with restriction endonucleases and digestion patterns were compared with those of reference rotaviruses. With the VP4 genes, four RFLP (AD) profiles were observed; three (A, B and C) were the same as those of bovine rotavirus NCDV (P[1]), IND (P[5]) and B223 (P[11]), respectively, Profile D was the same as that of porcine rotavirus OSU (p[7]). With the VP7 genes, five RFLP profiles (I-V) were observed; three of them (1, II and III) were the same as those of bovine rotavirus NCDV (G6), Cody I-801 (G8), and B223 (G10), respectively, Profile IV and V were atypical to those of reference bovine rotaviruses used in this study. These two profiles were identified as G6 and G5, respectively, after analyzing and comparing the nucleotide sequences. The G typing analysis revealed that 61.9% (26/42) were G6, which included G6 subtype; 28.6% (12/42) were G5; 7.1% (3/42) were G10; 2.4% (1/42) were G8. The P typing analysis revealed that 54.8% (23/42) were P(5); 28.6% (12/42) were P(7); 11.8% (5/42) were P(11); 4.8% (2/42) were P(1). Our results showed that G6/P(5) were the most prevalent rotaviruses in diarrheic calves in Korea. Also, this is the first report that G5P(7) rotaviruses were identified from cattle with diarrhea.

3.The Influence of Taekwon

The bone mineral densities of 60 middle school boys were measured with dual photon absorptiometry to define the influence of Taekwon-Do. They were divided into Taekwon-Do and control group, each comprising 30 boys. The bone mineral densities of the head, trunk, pelvis, both upper and lower extremities, and body total were measured. The results were as follows. 1. The BMD of head was 1.668 ± 0.153 in Taekwon-Do and 1.554 ± 0.153 0.167 in control. It was significantly increased in aekwon-Do(p <0.05). 2. The BMD of trunk was 0.378 ± 0.153 0.044 in Taekwon-Do and 0.353 ± 0.153 0.053 in control. There was no significant difference(p >0.05). 3. The BMD of pelvis was 1.648 ± 0.153 0.112 in Taekwon-Do and 1.444 ± 0.153 0.215 in control. It was significantly increased in Taekwon-Do(p <0.005). 4. The BMD of right and left legs were 1.597 ± 0.153 0.118 and 1.579 ± 0.153 0.125 in Taekwon-Do and 1.425 ± 0.153 0.115 and 1.421 ± 0.153 0.113 in control group respectively. They were significantly increased in Taekwon-Do(p <0.005). 5. The BMD of right and left arms were 1.184 ± 0.153 0.090 and 1.178 ± 0.153 0.115 in Taekwon-Do and 1.056 ± 0.153 0.095 and 1.066 ± 0.153 0.097 in control group respectively. They were significantly increased in Taekwon-Do(p.<0.005). 6. The total body BMD was 0.932 ± 0.153 0.068 in Taekwon-Do and 0.846 ± 0.065 in control. It is significantly increased in Taekwon-Do(p<0.005). In conclusion, Taekwon-Do increases the total bone mineral density and the mineral densities of the head, pelvis and extremities but not that of trunk. It is thought that continued practice of Taekwon-Do throughout adult life might prevent the senile osteoporosis

4.A case of fetal atrial flutter with hydrops fetalis.

Fetal hydrops is often serious and associated with a high perinatal motality rate. Cardiac causes of fetal hydrops include congenital heart diseases and rhythm disturbances. An irregular fetal heart rate may indicate atrial fibrillation and atrial flutter with variable AV conduction. Fetal atrial flutter is characterized by the pressence of flutter waves which are regular sawtooth undulations in the baseline that are larger than p waves. Authors experienced a case of fetal atrial flutter with hydrops fetalis at 30 week's gestation which was confirmed by fetal M-mode echocardiogram and electrocardiography. A new born infant had shown to have atrial flutter in utero and after delivery was successfully converted to normal sinus rhythm with digoxin and quinidene.

5.Dose the Self-Assessment(in training examination) for the Residents of Anesthesiology have any Significance?.

The Korean Society of Anesthesiologists began to take in training examination(Self Assessment) for the residents in April 1989. Two hundreds thirty four participants took the first examination and after that 343 in 1990, 399 in 1991, 455 in 1992 took the examination obviously the actual number is increasing each year. This study was done to see if there is any correlationship between the results of self assessment and board examination in the same population group. Ninety two persons who took self assessment in April 1990 and board examination in Jan 1991 and 108 persons who took self assessment in Sept. 1991 and board examination in Jan 1992 were investigated. In both groups we can see the close positive correlationships between the results of self assessment and written board examination.

8.Postoperative Idiopathic Generalized Convulsion Conversed to Generalized Status Epilepticus Following Spinal Anesthesia.

Generalized tonic-clonic convulsion was noted immediately after arriving at recovery room in eighteen year-old female patient who underwent an uneventful knee joint arthroscopy with spinal anesthesia. The first attack was successfully controlled with immediate treatment such as airway insertion, oxygenation and ventilation with mask, and infusion of 100 mg thiopental and 5 mg diazepam. The seizure activity changed to generalized status epilepticus after several recurrences of generalized convulsion, which was hard to control. The seizure activity was controlled by loading dose and continuous infusion of thiopental (total 1750 mg) and dilantin (total 1500 mg). A thorough diagnostic evaluation including a detailed medical investigation, neurologic physical examination, laboratory investigation with particular reference to blood chemistry test, computed tomogram, and magnetic resonance image except past history and electroencephalography (EEG) showed no abnormal findings. The patient has the past history of several attacks of idiopathic generalized convulsion in five or six year-old age and she showed generalized slow wave in EEG. No definite precipitating factor could be found in this case. But it may be vaguely suspected that the emotional stress of awakening state during operation under spinal anesthesia could have precipitated asymtomatic underlying seizure activity especially in the adolescent period even though the patient had clinically insignificant history of seizure attack.

9.A Case of Mycosis Fungoides Treated with Recombinant Interferon - alpha and PUVA.

We report a case of mycosis fungoides in a 28 year-old female patient who had multiple, bizzare, kidney or annular shaped, erythematous to brownishaly patches and plaques on the trunk, buttock, upper and lower extremities. Histopathologic finclingm showed epidermotropism and Pautrir microabscess in the epidermis and sssatypical lymphocytes infiltration in the dermis. T cell marker in leukocyte common antigen were positively stained in an immunohistochemical study. The skin lesions were reproved markedly after treatment it a total dose of 45 x 10 IU recombinant interferonaiv combination with a total dose of 385 iules/art PUVA.
