4.Comparison Curative Operation with Internal Drainage Procedure of Periampullary Ca.

BACKGROUND: The optimal management of periampullary Ca. stage III remains controversial. METHODS: A total of 103 patients treated in a 5-ear period at Kwangju Christian Hospital, 54 patients with a curative operation and 49 patients with internal drainage, were retrospectively studied. RESULTS: There was no significant difference in the 30-ay mortality rate between patients with a curative operation and those with internal drainage, 4% versus 6% respectively. There was a difference between a curative operation and the internal drainage procedure in the early morbidity rate (16.7% versus 8%) and the late morbidity rate (9.3% versus 32.7%). The mean survival was more favorable for those with a curative operation than for those with the internal drainage procedure for periampullary Ca. stage III (pancreatic Ca.: 11.6 months versus 4.7 months, CBD Ca.: 12.6 month versus 5.4 months, and Ampulla of Vater Ca.: 16.5 months versus 5.6 months). The 6-onth and the 12-onth survival rates of the curative-operation group were 80% and 55% for pancreatic Ca., 100% and 80% for CBD Ca., and 90% and 80% for Ampulla of Vater Ca.. The 3-onth and the 6-onth survival rates of the internal drainage group were 70% and 30% for pancreatic Ca., 85% and 40% for CBD Ca., and 75% and 45% for Ampulla of Vater Ca. CONCLUSION: The mean survival duration was more favorable for those with a curative operation than for those with an internal drainage procedure for periampullary Ca. stage III. Therefore, if there is no evidence of distant metastasis, a radial resection is the treatment of choice for a patient with a carcinoma of the periampullary Ca.

5.Arthroscopic Treatment of the Discoid Meniscus

Thirty-seven discoid lateral menisci of thirty-six patients were treated by arthroscopic means between January, 1987 and June, 1989. A discoid lateral meniscus was present in 15.3% of all patients who have underwent arthroscopy of the knee in this period. It was present in 25.7% of all meniscal lesions and in 39.4% of all lateral meniscal lesions. The average age was 22.7 years and the average length of follow up was thirteen months. Clinically, click sound was the most frequent symptom comprising 86% except knee pain, which was present in all. On physical examination, joint tenderness was present most frequently in 75%. On arthroscopic findings, twenty-eight discoid menisci had the tear and one had severe degenerative change. A tear was most frequently found in posterior horn and the most frequent pattern of tear was longitudinal including bucket-handle and peripheral type. Partial meniscectomy was performed on twenty-nine knees and subtotal meniscectomy on eight knees. At follow up, twenty-nine knees (78.4%) were rated excellent or good and eight knees (21.6%) were rated as fair. There were no poor results. We suggest that arthroscopic partial resection with preservation of normal meniscal shape as much as possible is more excellent than total meniscectomy for the symptomatic discoid meniscus.

The bony spur formation without degenerative change in the lumbar spines in the young age group is quite interesting and is not uncommon. This bony spur is different from the degenerative spur in the old age group in shape and mechanism of formation. The authors analyzed 25 patients below 29 years old with such posterior lumbar bony spur. We concluded that posterioly located Schmorl's node is main cause of the bony pour in youn age group at lumbar region.

The vaginal stone associated with vesico-vaginal fistula is quite rare and its cause, frequency, composition and others are poorly understood. We report a case of huge vaginal stone associated with vesico-vaginal fistula in 57 years old woman. considering of the possible causes of stone formation in the vagina and its composition, prevention, treatment complication and others. A stone, 6 x 7 x 5cm in size, was removed by litholapaxy. The stone was composed of uric acid, magnesium ammonium phosphate and carbonate appetite.

10.A Clinical Study on resistance and Antibiotics of Gram Negative Bone and Joint Infection

Although mortality and incidence rates of pyogenic osteomyelitis and arthritis have been reduced significantly due to development of treatment methods and antibiotics, the resistance of bacterias to antibiotics, especially in gram negative infections which increase rapidly in recent years, posess many difficult problems in the treatment of bone and joint infections. We have analysed 124 patients of gram negative bone and joint infection admitted in Seoul National University Hospital from Jan. 1976 to Dec. 1985. The following results and conclusion were drawn; 1. The ratio of gram negative infections to total pyogenic infections was 39.2% and has been increased annually. 1. The incidence of gram negative infections under 20 years of age was 54.0%, and male was 1.7 times as much affected as female. 3. Involved sites of pyogenic osteomyelitis were femur(32.6%), tibia(25.6%), humerus(6.2%), and those of pyogenic arthritis were hip(12.4%), knee(7.0%), shoulder(2.3%), and ankle(2.3%). 4. Original diseases of gram negative infection were hematogenous osteomyelitis(41.1%), infected nonunion(33.1%), pyogenic osteomyelitis(16.9%), sepsis(4.8%), and infected total hip replacement arthroplasty(4.0%). 5. Incidence rate of gram negative infection, especially mixed gram negative infection rather than pure gram negative infection, was increased annually. 6. Sensitive antibiotics for gram negative organisms were turned out aminoglycosides such as Amikacin and Tobramycin in general, however, the, resistance to that antibiotics increased annually. 7. In the view that resistance rate of gram negative organisms to antibiotics increased annually, it cannot be too much emphasized that the most sensitive antibiotics to be used must be chosen according to the results of antibiotics sensitivity test, and that further advanced effective antibiotics should be developed.
