5.The Accuracy of Pulse Oximeter in Predicting the Arterial Oxygen Saturation.

We studied 21 neonates who required mechanical ventilation during study period in NICU OF Il Sin Christian Hospital with diagosis of prematurity, IRDS, pneumothorax and diaphragmatic hernia to evaluate the accuracy of pulse oximeter in predicting the arterial oxygen saturtion, hypoxia and hyperoxemia. We also studied whether the changes of birth weight, hematocrit, blood pressure and body temperature affect the accuracy of pulse oximeter. The results are 1) Linear regression analysis revealed a close correlation between in vivo pulse oximeter readings and in vitro SaO2 measurements in patients (Y=0.008X+12.34, r=0.946) 2) The changes of birth weight, hematocrit, blood pressure and body temperature did not affect the accuracy of pulse oximeter. 3) SpO2-PaO2 relationship was similar to the SsO2-PaO2 relationship. 4) When Nellcor N-100 pulse oximeter was setted at alarm limit 96% to avoid hyperoxemia, it identified 16 of 22 hyperoxemic instances (sensitivity 73%) and alarmed falsely in 48 of 92 nonhyperoxemic instances (specificity 59%).

7.Dynamic MR Imaging of Hepatic Hemangioma and Hepatocellular: Findings and Differential Diagnosis.

PURPOSE: We performed dynamic MR imaging using GdDTPA to find characteristic enhancement pattern of hepatic hemangioma distinguishing from hepatocellular carcinoma. METHODS AND MATERIALS: 28 hepatic hemangiomas and 10 hepatocellular carcinomas were evaluated. Serial dynamic scans after Gd-DTPA(0.1mmol/kg) intravenous injection were obtained by using 0.5T machine and analyzed contrast-to-noise ratio(CNR) of the lesion and enhancement pattern on each scan. RESULTS: Hepatic hemangiomas had positive CNR from 1-2 minute images, and revealed typical "fill-in phenomenon" on early phase with prolonged enhancement in 26 cases(92.8%), and early homogeneous enhancement with isointensity on delayed phase in 2 cases(7.2%) of small hemangiome~ Hepatocellular carcinomas revealed inhomogeneous enhancement with hypointensity on delayed phase in 10 cases(100%) and 3 cases (30%) of capsular enhancement. CONCLUSION: Hepatic hemangioma can be easily distinguished from hepatocellular carcinoma by using Gd DTPA enhanced dynamic MR imaging according to its typical enhancement pattern of "fill-in phenomenon" and prolonged enhancement.

Methods:
Van Manen’s hermeneutic phenomenological method was used to analyze qualitative data. The partici- pants were 8 mental health nurses who worked in hospitals, mental health welfare centers, or psychiatric reha- bilitation facilities. Data were collected through in-depth interviews from July 24, 2017, to June 20, 2018.
Results:
Eight essential themes emerged: “A body that must endure severe symptoms”, “A hand that cares for the wound”, “Ambivalence in a closed space”, “Making them adapt to the open world”, “Being together and getting close to the patients”, “Going forward, leaning on each other”, “Time to protect even a small light”, and “Becoming a shining star by myself”.
Conclusion
Strategies are needed to cope with patient violence and prevent psychological trauma. Mental health nurses should be trained for empathetic competency to understand symptoms and build trusting relationships. Caring requires respect and love based on humanity. Mental health nurses learn from patients and experience mutual care that grows through self-reflection. Policy support including financial and manpower security is essential to improving the quality of care and preventing exhaustion.

10.A Case of Systemic Lupus Erythematosus with Myelofibrosis.

A 42-year-old man presented with severe pancytopenia and uncontrolled epistaxis. The diagnosis of SLE was made and the pancytopenia was found to be due to myelofibrosis. The pulse therapy with methylprednisolone and maintenance therapy with prednisolone reversed both pancytopenia and myelofibrosis. Although myelofibrosis has been described in SLE, this coexistence must be very rare since there has been only 19 cases showing this combination. We report a case of SLE with myelofibrosis which was reversed by the treatment with glucocorticoid.
