The primary hyperparathyroidism is a complex endocrine disease caused by neoplasm or diffuse hyperplasia of parathyroid gland in which excessive paratyroid hormon is secreted. This results in chemical abnormalities of serum, and exerts major influences on the bone, kidney and gastrointestinal tract. The authors report 2 cases of primary hyperparathyroidism with review of the literature.

PURPOSE: We performed dynamic MR imaging using GdDTPA to find characteristic enhancement pattern of hepatic hemangioma distinguishing from hepatocellular carcinoma. METHODS AND MATERIALS: 28 hepatic hemangiomas and 10 hepatocellular carcinomas were evaluated. Serial dynamic scans after Gd-DTPA(0.1mmol/kg) intravenous injection were obtained by using 0.5T machine and analyzed contrast-to-noise ratio(CNR) of the lesion and enhancement pattern on each scan. RESULTS: Hepatic hemangiomas had positive CNR from 1-2 minute images, and revealed typical "fill-in phenomenon" on early phase with prolonged enhancement in 26 cases(92.8%), and early homogeneous enhancement with isointensity on delayed phase in 2 cases(7.2%) of small hemangiome~ Hepatocellular carcinomas revealed inhomogeneous enhancement with hypointensity on delayed phase in 10 cases(100%) and 3 cases (30%) of capsular enhancement. CONCLUSION: Hepatic hemangioma can be easily distinguished from hepatocellular carcinoma by using Gd DTPA enhanced dynamic MR imaging according to its typical enhancement pattern of "fill-in phenomenon" and prolonged enhancement.

BACKGROUND: Serratia marcescens proliferates well in a humid environment or soil and is recently considered as an important pathogen for the severe nosocomial infections. this organism is spreads easily by hand-to-hand transmission, and contaminates medical equipment used for invasive procedures, working environment, medications, and soap. METHODS: We investigated the source of an outbreak of bloodstream infections by S. marcescens isolated that occurred during the period from July to December, 2004, at a university hospital in Gyeonggi Province and attempted to intervene in the outbreak and control it. RESULTS: From July to December, 2004, S. marcescens grew from 296 blood culture from 283 patients. The medical charts of the patients were reviewed, and surveillance cultures were taken to identify the outbreak of nosocomial infections and risk factors. Only four cases of infection were identified and all remaining positive blood cultures were due to contamination. Nine isolates randomly selected from the 296 S. marcescens showed an identical pulsed-field gel electrophoresis pattern. To identify the source of infection, environmental culture and hand cultures of the related medical workers were carried out, but S. marcescens was not isolated. CONCLUSION: As the result of aggressive infection control activities, such as re-education on environmental management methods, hand washing techniques, and blood culture sampling techniques, no more S. marcescens had been grown in blood culture since January, 2005.

The purpose of this study was to identify the influence of diet related factors, such as diet behaviors, food intake, and nutrient intakes, on self-rated health (SRH). Also, in order to determine fitness of classification for SRH reflecting diet related factors, this study surveyed older adults in Gyeongnam province. A total of 101 responses were collected using the interview survey method. The self- rated health of rural older adults was poor as reported by 49.5%. The level of self-rated health was found to be related to the frequencies of coffee and snack, use of sugar and vegetable in diet, the amount of total food intake, and cholesterol intake. The result of discriminant analysis, which was conducted to assess the adequacy of SRH classification and to determine the class of observation, showed frequency of coffee and use of vegetable in diet among 47 variables as predictive variables for explaining SRH. The fitness of self-rated health function was high to 47.7%. Therefore, diet-related factors were ascertained to be important variables to predict SRH.

PURPOSE: To evaluate potential usefulness of dynamic Gd-DTPA enhanced MRI in the diagnosis of HgCI2 induced acute tubular necrosis of rabbits. MATERIALS AND METHODS: Sixteen rabbits were used as control group, and 14 rabbits and 12 rabbits were used as acute tubular necrosis groups of 24 hours and 48 hours after HgCI2 injection, respectively. Sequential dynamic MR imagings were acquired using Gd-DTPA(0.25mmol/kg), and time-signal intensity curves were obtained from cortex, outer medulla and inner medulla. RESULTS: In control group, a dark band, which reflects concentrated Gd-DTPA, migrated from cortex to inner medulla of the kidney, and the ratio of the signal intensity of post Gd-DTPA injection to the signal intensity of pre Gd-DTPA injection(RSI) decreased below 1 at 13sec and 26sec (mean:17 +/- 6.2sec) in cortex, at 52sec (mean :52sec) in outer medulla, and after 117sec(mean :112 +/- 33.9sec) in inner medulla of the kidney. In acute tubular necrosis group of 24 hours after HgCI2 injection, the dark band did not appear and signal intensity in cortex and medulla increased diffusely, and RSI increased above 1 in all locations. In acute tubular necrosis group of 48 hours after HgCI2 injection, the dark band appeared only in the cortex and no sign of migration was observed, and RSI is little changed except in cortex at 13sec(0.76 +/- 0.05) and 26sec(0.86 +/- 0.06). There were statistically significant differences in the time-RSI curves among cortex, outer medulla, and inner medulla in study groups, respectively (p<0.0001). CONCLUSION: Dynamic Gd-DTPA enhanced MRI of the kidney could be utilized to evaluate both renal structure and functional changes.

BACKGROUND: The influenza is a contagious respiratory illness caused by influenza viruses. The primary target groups recommended for annual vaccination are healthcare workers and households which have frequent contact with persons at high risk and can transmit influenza to those persons at high risk. Members of these groups should be vaccinated against the flu so that they can avoid getting infected with continuously mutating influenza viruses. We assessed healthcare workers' knowledge and attitudes regarding influenza vaccination in order to help promote the vaccination rate. METHODS: This survey was carried out in two hospitals affiliated with the Catholic University School of Medicine, from December 2004 to January 2005. Of the 3,023 questionnaires distributed, 2,023 could be evaluated. RESULTS: The most frequently cited reason for receiving influenza vaccine was self-protection against influenza (55.4%). The most common reasons for not receiving influenza vaccine are personal health problems such as concurrent flu, pregnancy or breast-feeding (29.2%). There is no significant difference in the frequency of side effect between two groups receiving and not receiving vaccine. The most frequent side effect of influenza vaccination is flu-like syndrome; People receiving vaccine have more significant knowledge than those people not receiving vaccine about efficacy of flu vaccination, risk of influenza infection of healthcare workers and their need of flu vaccination. CONCLUSION: In order to promote the vaccination rate, education targeting people at high risk need to keep continuous and facilitate access to vaccination.

Various methods of taking of optic foramen view and avalubilities have been studied by many foreigninvistigators, but those in Korea have not been reported yet. The authors have measured and examined the size andshapes of optic foramina of optic foramen views in 111 Korean checked at Inje Medical College Hospital duringabout 3 years form June 1979 to March 1982. The cases were divided into gwo groups, A and B, for the convenienceof study. Group A consisted of 50 normal Korean 5 to 55 years of age with no visual symptoms and signs. Group Bincluded 69 patients 3 to 75 yrs of age with visual symptoms and signs. The results were as follows; 1. Mean valueof actual diameter of optic foramen meassured by Goalwin's method in Group A; Right; A diameter; 4.21mm, Bdiameter; 4.32mm, Left; A diameter; 4.29mm, B diameter; 4.30mm. 2. Absolute symmetry of the greatest diameters ofboth optic foramina in Group A; 36%, difference of 0.5 or lessl 28%, from 0.5 to 1.0mm; 18%, from 1.0 to 1.5mm;8%, uncomparable cases; 10%. The greatest difference was 1.4mm. 3. The shapes of optic foramen in Group A; ovalshape; 65%, circular ; 29%, keyhole or double foramen; 6%. 4. In Goup B, the peak age group was second decade(31%), the most comon clinical cause of examiniation of optic foramen view was optic nerve atrophy(30%), and thepathologic findings were only 4 cases(7%). 5. The technique of optic foramen view by modified Pfeiffer's methoddelineated the outline of optic foramen more clearly than ordinary Rhese's method in our experience.
