4.The Usefulness of Endo-rectal Coil MRI in the Staging of Clinically Localized Prostate Cancer.

PURPOSE: We evaluated the ability of endo-rectal coil MRI (ER-MRI) to predict the local pathological stage of prostate cancer prior to radical prostatectomy and compared the results with those of transrectal ultrasonography (TRUS). MATERIALS AND METHODS: ER-MRI using high field magnets (1.5 Tesla) were performed in 22 patients (mean age 62.8 years, range 51-73) with clinically localized prostate cancer before radical prostatectomy. Of the 22 patients, 17 patients were also assessed by TRUS. The results of the imaging techniques were compared with the post-operative histopathological findings. As one patient with pelvic lymph node metastasis, which was detected on frozen-section examination during surgery, was spared radical prostatectomy, the final evaluation included 21 patients. RESULTS: DSeven of the 21 patients (33%) were found to have extraprostatic extension (EPE), and 5 had seminal vesicle invasion (SVI). The sensitivity and specificity for diagnosing EPE using ER-MRI were 62.5% and 84.6%, respectively, and 16.7% and 100% with TRUS. The sensitivity and specificity for diagnosing SVI were 80.0% and 93.8%, respectively with ER-MRI, and 0% and 92.3% with TRUS. The accuracy of predicting SVI was 90.5% with ER-MRI compared to 70.6% with TRUS. CONCLUSIONS: ER-MRI was significantly better than TRUS for determining the local extent of prostatic cancer and for prediction of SVI in the preoperative staging of clinically localized prostate cancer.

5.A Study on the Morphological Analysis of Sperm.

In male reproducible health, fertility and IVF (in-vitro fertilization), semen analysis has been most important. Semen analysis can be divided into concentration, motional and morphological analysis of sperm. The existing method which was developed earlier to analyze semen concentrated on the sperm motility analysis. To provide more useful and precise solutions for clinical problems such as infertility, semen analysis must include sperm morphological analysis. But the traditional tools for semen analysis are subjective, imprecise, inaccurate, difficult to standardize, and difficult to reproduce. Therefore, with the help of development of microcomputers and image processing techniques, we developed a new sperm morphology analyzer to overcome these problems. In this study the agreement on percent normal morphology was studied between different observers and a computerized sperm morphology analyzer on a slide-by-slide basis using strict criteria. Slides from 30 different patients from the SNUH andrology laboratory were selected randomly. Microscopic fields and sperm cells were chosen randomly and percent normal morphology was recorded. The ability of sperm morphology analyzer to repeat the same reading for normal and abnormal cells was studied. The results showed that there was no significant bias between two experienced observers. The limits of agreement were 4.1%~-3.8%. The Pearson correlation coefficient between readers was 0.79. Between the manual and sperm morphology analyzer, the same findings were reported. In this experiments the slides were stained by two different methods, PAP and Diff-Quik staining methods. The limits of agreement were 7.2%~-5.7% and 6.0%~-6.3%, respectively. The Pearson correlation coefficients ware 0.76 and 0.91, respectively. The limits of agreement was tighter below 20% normal forms. In the experiments of repeatability, 52 cells stained by PAP and Diff-Quik staining methods were analyzed three times in succession. Estimating pairwise agreement, the kappa statistic for the pairs were 0.76, 0.81, 0.86, and 0.75, 0.88, 0.88 respectively. In this study it was shown that there was good agreement between manual and computerized assessment of normal and abnormal cells. The repeatability and agreement per slide of computerized sperm morphology analyzer was excellent. The computer's ability to classify normal morphology per slide is promising. Based on results obtained, this system can be of clinical value both in andrology laboratories and IVF units.

6.Surgical Treatment of Chondrosarcoma

Chondrosarocoma is a malignant, relatively slow-growing cartilaginous tumor that generally has a long natural history. From Feb. 1986 to Jan. 1993, 29 cases of chondrosarcoma were registered in the Department of Orthopedic Suergery of Korea Cancer Center Hospital. We reviewed 23 cases of chondrosarcoma which were treated surgically, aimed at curative procedure. Fourteen patients were male and 9 female. The average age at the time of diagnosis was 31.9(17-59) years. The pelvis was the most frequent site(7 patients) followed by femur(6), scapula(4), tibia(2), and each one case of spine, rib, humerus, and finger. The Enneking s surgical stage was II B (18 cases), II A (1), I B (3) or I A (1). The histological grades were as follows : 4 cases of grade 1; 3 grade 2 and 16 grade 3. Local resection with or without reconstruction were done for 20 cases and amputation or disarticulation 3. Chemotherapy was done in four cases and was not effective. The average follow-up period was 34.2(9-81) months. As complications, local recurrence occurred in 4(17.3%) cases, infection 2(8.7%), fracture 1(4.3%), lymphedema 1 and rediation necrosis of soft tissue 1, Distant metastasis occurred in 7(30.0%) cases: 6 cases to lung and 1 to brain. At final follow up 15 cases were continuous disease free, 3 alive with disease, 4 died of disease and 1 no evidence of disease. The Kaplan-Meier's estimated 5-year actuarial survival rate of total 23 cases was 66. 9%. Seven cases with histopathologic grade less than 2 showed 100% of 5 year actuarial survival rate, even one case is alive with disease at final follow up of 62 months. Fifteen cases of surgical stage II B with histopathologic grade 3 had 55.4% of 5 year actuarial survival rate: ten cases with wide surgical margin 87.5%o five cases with intralesional or marginal surgical margin 25%. Early surgical excision of chondrosarcoma with enough wide surgical margin can prevent local recurrence and metastasis.

7.Multiple Primary Malignant Neoplasm Combined with Musculoskeletal Malignancy

From Jan. of 1990 to Dec. of 1992, we have experienced three cases of multiple primary malignant neoplasms originated from different histogenesis, in the Department of Orthopedic Surgery of Korea Cancer Center Hospital. Liposarcomas, malignant fibrous histiocytoma were combined with renal cell carcinoma, mucinous cystadenocarcinoma of ovary or prostatic cancer. With Moertel's classification, synchronous tumors were found in 2 cases and metachronous in 1 case. Synchronous tumors were 2 liposarcomas-renal cell carcinoma or mucinous cystadenocarcinoma of ovary. Metachronous tumors were malignant fibrous histiocytoma-prostate cancer. We couldn t find any family history among them. The causes of these multiple malignancies were considered as the individual predilrection to tumors.

8.Colitis Cystica Profunda Causing Intussusception: A Case Report.

Colitis cystica profunda is a benign disorder in which a mucous cyst is located in the submucosal layer of, primarily, the pelvic colon and rectum. Radiologic reports of the condition are rare. We report the radiological findings of a case of colitis cystica profunda arising from the proximal ascending colon near the ileocecal valve, and causing intussusception. 9.A Case of Idiopathic Gingival Fibromatosis and Hypertrichosis.

Idiopathic gingival fibromatosis is associated with other abnorma ities of which the most striking is hypertrichosis of variable degree. Gorlin and Pindborg described this condition as a distinct syndrome termed idiopathic gingival fibromatosis and hypertrichosis. We report a case of idiopathic gingival fibromatosis and hypertrichosis occuring in 34-year-old woman with enlarged gingival tissue, hypertrichosis, and enlargenment of the breasts, ears and nose. Laboratory tests including hormonal and chromosomal studies were all within normal limits. Histologically, the gingival hyperplastic tissue showed thick bundle of collagen fibers with few fibroblasts or capillaries and little or no inflammatory infiltrate.
