1.A Case of Idiopathic Gingival Fibromatosis and Hypertrichosis.

Idiopathic gingival fibromatosis is associated with other abnorma ities of which the most striking is hypertrichosis of variable degree. Gorlin and Pindborg described this condition as a distinct syndrome termed idiopathic gingival fibromatosis and hypertrichosis. We report a case of idiopathic gingival fibromatosis and hypertrichosis occuring in 34-year-old woman with enlarged gingival tissue, hypertrichosis, and enlargenment of the breasts, ears and nose. Laboratory tests including hormonal and chromosomal studies were all within normal limits. Histologically, the gingival hyperplastic tissue showed thick bundle of collagen fibers with few fibroblasts or capillaries and little or no inflammatory infiltrate.

3.Hounsfield Units of Urinary Calculi as a Predictor of the Therapeutic Effect of Extracorporeal Shockwave Lithotripsy.

PURPOSE: Non-enhanced spiral computed tomography (NESCT) has become the preferred method for imaging urinary calculi. We evaluated the Hounsfield units (HU) of urinary calculi on NESCT as a predictor of the therapeutic effect of extracorporeal shockwave lithotripsy (ESWL). MATERIALS AND METHODS: We evaluated 72 patients with urinary calculi who underwent ESWL. The HUs of calculi were measured on the pretreatment NESCT and at post-treatment via radiographic assessment. The patients were divided into 2 groups, the response group (n=39): the patients with remnant calculi less than 3mm or they were without remnant calculi, and the non-response group (n=33): the patients with remnant calculi greater than 3mm. The HUs of the response and non-response groups were then compared. RESULTS: The HUs of the response and non-response groups were 529.7+/-168.2 and 814+/-116.6, respectively, and the HUs of the non-response group was significantly higher than that of the non-response group. CONCLUSIONS: The HU of urinary calculi, as seen on NESCT, can help to predict the therapeutic effect of ESWL and also select the most effective treatment modalities for the patients suffering with urinary calculi.

8.A Case of Arteriovenous Malformation of the Uterus.

Arteiovenous malformation of the uterus is an extremely rare and becomes one of the causes of severe uterine bleeding. Because uterine bleeding can be aggravated with diagnostic dilatation and curettage, an accurate diagnosis should be achieved before the intervention. This malformation may be congenital or acquired. In the past, the diagnosis is usually made retrospectively after hysterectomy, however, recently it may be made before management by ultrasound, doppler ultrasound, pelvic angiography, magnetic resonance imaging, etc.. We have experienced one case of arteriovenous malformation of the uterus, which is presented with a brief review of the literatures.

The expectoration of blood is always a fearful experience for the patient and a matter of grave concern to the attending physician, because it may be the warning sign of serious diseases. When such bleeding occurs, the possibility of its being due to pulmonary tuberculosis, bronchiectasis or carcinoma of the lung is promptly suggested. Nowadays, with the advance of modern diagnostic studies, differential diagnosis has become possible for the hemoptysis patients to have appropriate treatment. This evaluation is based on the review of 75 cases of patients whom we performed emergency open thoracotomies for 6 years from March 1992 to February 1997 in the Department of Thoracic and Cardiovascular Surgery, Seoul Adventist Hospital. The mean age of the patients was 36.6 years old with a range from 19 to 68, and most prevalent age group was thirties. The most common underlying lung disease causing hemoptysis was tuberculosis(44%). Most accurate diagnostic study was bronchoscopy during hemoptysis(95%) and right middle and lower lobe(17.3%) was the most common site of lesion. Lobectomy(50%) was the most frequent operative method and recurrent hemoptysis(31.6%) was the most common postoperative complication. But most of the patients(82.6%) were completely recovered by surgical treatment. Now we concluded that the proper management of hemoptysis was completed by surgical approach with definite diagnosis and supportive medication.

10.Spontaneous Closing Time of PDA in Term and Preterm Infants Without Respiratory Distress Syndrome.

PURPOSE: To investigate the spontaneous closing time of PDA in term and preterm infants without respiratory distress syndrome. METHOD: For twenty four term and sixteen preterm infants without RDS, prespective echocardiography was performed within 24hr of birth, which was repeated everyday till spontaneous closure of ductus. At the same time, LA/Ao ratio in M-mode echocardiography and blood pressure were checked. RESULT: In term infants, spontaneous ductal closure rate was 15% in the first day of life, 57.7% in the second day, 88.5% in the third day, and 100% in the fourth day. For the preterm infants without RDS, the spontaneous ductal closure rate was 25%, 73.3%, 86.7%, 93.3% in each day. One preterm infant dided of meningitis at second day of life was excluded. One preterm infant (11,23 who had patent ductus arteiosus at fourth day of llife showed persistent ductus at 6 months of life. CONCLUSION: Spontaneous closure of patent ductus arteriosus will not occur if there is persistent shunt after 4th day of life in term and preterm infants without RDS.
