3.A Case of Imipramine(Tofranil(R)) Poisoning with Cardiac Arrhythmias.

Imipramine(Tofranil(R)) is one of the tricyclic antidepressants commonly used in depressive symptoms or enuresis. An accidental or nonaccidental poisoning of imipramine is common today. A 16 years old girl was admitted to Seoul National university Hospital because of unocnsciousness and frequent attacks of seizure after the impulsive ingestion of 1.4gm of imipramine. She showed variable arrhythmias such as complete RBBB, secod degree AV block and ventricular tachycardia and recovered from poisoning without residual myocardial damage. We present a case of imipramine poisoning with cardiac arrhythmias with review of literatures.

4.A Clinical Study on Antihypertensive Effects of Aldactazide (Spironolactone+Hydrochlorothiazide).

The antihypertensive effects fo aldactazide and serum electrolytes changes before and after treatment with aldactazide were observed. The followings were obtained: 1. Results of antihypertensive therapy with aldactazide in 23 hypertensive patients revealed good effects in 52.17%, fair in 26.09%, poor in 8.7%, and failure in 13.04% of cases. In 78.26% of cases, good or fair control of blood pressure was obtained. 2. Daily doses of aldactazide ranged from 25mg to 50mg depending upon the level of blood pressure. The antihypertensive effects were appeared after average 11 days of administration. 3. The side effects during treatment with aldactazide were dizziness, weakness, indigestion, headache, and restlessness in 26.28% of cases (6 of 23 cases). 4. No significant changes in serum Na and K values were observed before and after treatment with aldactazide for average 23 days.

5.Retraction: Thoracic Intramedullary Clear Cell Meningioma.

6.The relationship of maturation value of vaginal epithelium and bone mineral density in postmenopausal women.

OBJECTIVE: To assess the relationship between vaginal cytology and bone mineral density in postmenopausal women. METHODS: In 93 postmenopausal women, vaginal cytology smears were taken for maturation index, serum estradiol level and bone mineral density were also taken. the percentage of each cell type found By vaginal cytology was multiplied to its specific value:superficial cells, 1.0; intermediate cell, 0.6; parabasal cell, 0.2 and modified to maturation value. Bone mineral density was measured at lumbar spine using DEXA. RESULTS: The mean bone mineral density was 0.88+/-0.14g/cm2. Mean maturation value was 50.53+/-20.74 and it was related with age and bone mineral density. CONCLUSION: Maturation value of vaginal epithelial cell represents the influence of estrogen on bone mineral density.

7.The Location of Molar Furcation in Korean.

The objective of the present study was to investigate the vertical and horizontal location of the molar furcations in korean. The samples used in this study included 132 maxillary molars and 120 mandibular molars. Of them, 47 maxillary molars and 34 mandibular molars had the fused roots. So, 85 maxillary molars(54 1st and 31 2nd molars) and 86 mandibular molars(46 1st and 40 2nd molars) were measured. The vertical and horizontal location of molars were measured with divider and digimatic micrometer and their means and standard deviation calculated. The results were summarized as follows: 1. The ratio of fused roots found in this study was the highest in the maxillary second molars with 59%, followed by mandibular second molars(46%) and maxillary first molars(7%) and none were discovered in the mandibular first molars. 2. In the study of the vertical location of molar furcation, the results were as follows : In the maxillary first molars, the length in descending order were distal(5.06mm), mesial(4.52mm) and buccal(4.01mm) and in the maxillary second molar, distal(4.04mm), mesial(4.02mm) and buccal(3.87mm). In the mandibular first molar, the length was 3.69mm on the lingual side and 2.81mm on the buccal side, and in the mandibular second molar, 3.87mm on the lingual and 3.61mm on the buccal side. 3. The location of the mesial and distal furcations in horizontal dimension measured showed following results : buccal and mesial furcations of the maxillary molars and buccal and lingual furcations of the mandibular molars generally found at the center, but the mesial furcation of the maxillary molars were found approximately two thirds toward the palatal aspect.
8.Thoracic Intramedullay Clear Cell Meningioma.

Intramedullary clear cell meningioma(CCM), which is more aggressive than other variants of meningioma, is extremely rare. To date, only one case has been documented in spinal tumors. We report the first case of an intramedullary CCM originating in the spinal cord at the thoracic region.

OBJECTIVES: A retrospective study was performed to clarify the change of lumbar bone mineral density(BMD) according to the duration of hormone replacement therapy(HRT) and the other factors such as age, years since menopause(YSM) and initial BMD in postmenopausal women. METHODS: From January, 1995 to December, 1998 we measured lumbar bone mineral density in 100 postmenopausal women in the department of obstetrics and gynecology, Inha University Hospital. These women had been followed for 2 years after taking HRT. We investigated whether there were any relation between the duration of HRT, age, YSM, initial BMD and change of BMD. RESULTS: Lumbar BMD was increased 2.06% after one year of HRT(p=0.0001) but there was no change of BMD at the 2nd year of HRT(p=0.847). The response to HRT was greatest in those who were oldest(r=0.209 ; p=0.039) and furthest YSM(r=0.209; p=0.039), and consequently among those who had the lowest BMD(r=0.590 ; p=0.0001). CONCLUSION: The bone mass was increased upto 2.06% at the 1st year of HRT but no more progressive increase was occurred. It suggest that intensive HRT is needed at 1st year of therapy. The risk of fracture is not decreased to that of the healthy population inspite of HRT, so the earlier therapy is necessary to prevent osteoporotic fracture despite of intervention.

10.Clinical Observation on Corrected Transposition of the Great Arteries(CTGA).

A clinical obsevation was made on 8 patients with Corrected Transposition of the Great Arteries who visited seoul National University Hospital during the period of Feb., 1979-Sep., 1982. 1. Sex distribution was 5 male and 3 female patients. 2. Age distribution was from 16 to 39 years and the mean age was 24 years. 3. Symptoms were exertional dyspnea in all cases, cyanosis in 3 cases. Other symptoms were palpitation, chest pain, growth retardation and chest deformity. Duration of illness was from 6 to 16 years and N.Y.H.A. functional class was between II and III. 4. On physical examination, cardiac mumurs were heard in all cases. Cyanosis on the lips and nail beds were noticed in 3 cases and clubbing of fingers was seen in 1 case. 5. Situs inversus was noticed in 1 case, mesocardia in 3 cases, dextrocardia in 1 cases and right sided aortic arch in 1 case by simple Chest PA. 6. Electrocardiographic findings were Biventricular hypertrophy in 3 cases, Left ventricular hypertrophy in 2 cases, Right ventricular hypertrophy in 2 cases and Incomplete right bundle branch block in 1 case. No arrhythmias were observed. Septal Qwave was not observed on the left precordial leads in all cases. 7. CTGA were suspected or diagnosed by Echocardiography in 7 cases. 8. The diagnosis of CTGA and its associated anomalies were confirmed in all cases by cardiac catheterization and angiocardiography. There were one case of I.D.D. type CTGA and seven cases of S.L.L. type CTGA's. Associated anomalies were ventricular septal defect in 5 cases, pulmonary stenosis in 5 cases, patent foramen ovale in 3 cases, atrial septal defect in 2 cases, patent ductus arteriosus in 1 case, dextrocardia in 1 case, left sided A.V. valve regurtation in 2 cases, and right sided aortic arch in 1 case. 9. Operation was done in the case associated with patent ductus arteriosus, and corrective surgery was done in the another case associated with ASD, VSD, PS and TR with C-TGA.
