1.Consecutive Esotropia Following Surgery for Intermittent Exotropia.

Authors reviewed 459 cases which had been followed up over 3 months after surgery to evaluate the incidence of consecutive esotropia and its associated factors. For every case, we investigated the age at the surgery, the methods of surgery, stereopsis, cycloplegic refractive power, the deviation difference between far and near fixation, the deviation before surgery and the results at the postoperative 1 day, 1 week, 1 month, 4 months, 6 months, 1 year and 3 years. The incidence of consecutive esotropia tended to decrease as time passed, and occurred in 21 cases[4.6%] at the last follow-up[averaging 13.1 months]. When the deviation difference between at the distance and the near fixation was above 10 delta and the results at the postoperative 1 day was overcorrection above 10 delta, with significantly higher[p<0.05] incidence of consecutive esotropia. Other factors were not statistically significant. In conclusion, the preoperative factor was the far and near deviation difference above 10 delta and the postoperative factor was the first day postoperative overcorrection above 10 delta. It is considered that the nonsurgical treatment can be desirable for the former cases, and overcorrection above 10 delta should be avoided for the latter.

2.Hair Follicles of Anterior Parietal Region are More Vulnerable than Occipital Region in Classic Acute Telogen Effluvium.

Background: The well known clinical feature of the classic acute telogen effluvium is diffuse hair loss from all over the scalp and the diagnosis is confirmed by a trichogram showing an increased telogen count. While the telogen hair shedding continues, newly cycled back young anagen hairs develop in the involved scalp. Objective: To see if there is a regional difference in the numbers of the involved hair follicles in that seemingly diffuse hair loss. Methods: In 7 cases of the telogen effluvium, the telogen count was done at two sites, anterior parietal and occipital. During the count, the short tapered anagen hairs(<1cm) which developed during the course of the telogen effluvium were counted together and compared according to the regions. Results: The sum of the short anagen hairs and the telogen hairs was 50.3% in anterior parietal area, whereas it was 31.6% in occipital area(p<0.0l). Conclusion: Hair follicles in anterior scalp appear to be more vulnerable than occipital scalp in the acute telogen effluvium.

Transsexuals are those who have appropriate chromosomal, hormonal and anatomical characteristics corresponding to their sexual phenotype but feel strongly with respect to their sexual identity, that they belong to the opposite sex. For this reason, they wish to be reassigned to their psychological sex by means of endocrinological and surgical treatment. Although its etiology is unknown, the majority of modern psychiatrists and behavioral scientists agree that the transsexualism is a disease entity belonging to gender identity disorder or gender dysphoria but not to be confused with the transversite, honosexualism or psychiatric problem. Three kinds of hermaphroditism have been defined according to the histology of the gonad : female pseudohermaphroditism, male pseudohermaphroditism and true hermaphroditism. It is very difficult, if not possible to assign sex especially in neonate and young children. Once sex assignment has been decided and accepted by the patient, surgical intervention should be undertaken as soon as possible. For adults there are many kinds of operative techniques. We have operated 28 cases of transexualism patients by using the penile and scrotal skin flap. This technique is superb for improving sexual appearance and function, and finally for a good opportunity for social activity. We report the results of surgery.
