1.Comparative Study of the Roentgenographic Methods for the Measurement of the Femoral Anteversion

The angle of the femoral anteverison has been recognized as an important consideration in the orthopedic Geld. The angle of the femoral anteversion can be measured roentgenographically by 3 methods, such as fluoroscopic, biplanar and axial methods. It is the purpose of this study to investigate the comparative accuracy of the roentgenographic methods determining the angle of the femoral anteversion. The measurements by the fluoroscopic(Rogers), biplanar (Magilligan) and axial (Dunn) methods were compared with the direct measurement of 42 adult dried femora and the measured values were statistically analyzed. The results were as follows: l. Among fluoroscopic, biplanar and axial methods, the biplanar method using Magilligan technique was the statistically reliable method to determine the angle of the femoral anteversion. 2. The axial method showed tendency to underestimate the angle of the femoral anteversion. 3. Though the fluoroscopic method was not so accurate as the biplanar method statistically, the 2 methods showed no difference in the ratio of measurement error within ±5°and ±10° range.

3.A Comparative Study of Arrhythmogenic Doses of Epinephrine during Sevoflurane or Halothane Anesthesia in the Dogs.

BACKGROUND: Epinephrine used in surgery to provide hemostasis may elicit ventricular arrhythmias. A desirable anesthetic would not sensitize the myocardium to exogenously administered epinephrine. So the effect of sevoflurane, which was introduced to clinical anesthesia recently, on cardiac arrhythmias induced by the infusion of epinephrine was compared with those of halothane which was already known to epinephrine-induced arrhythmia in the 14 mongrel dogs. METHODS: The authors compared the arrhythmogenicity (three or more premature ventricular contractions, PVCs)of intravenously administered epinephrine in 14 mongrel dogs who were randomly assigned to receive sevoflurane (1.7 vol%) or halothane (0.75 vol%) anesthesia equipotently. The arrhythmogenic doses of epinephrine determined in this comparative study were expressed by both infusion rates of epinephrine during sevoflurane and halothane anesthesia. RESULTS: The mean values of the arrythmogenic infusion rates of epinephrine were 27.1 7.6 g/kg for sevoflurane and 2.7 0.8 g/kg for halothane. CONCLUSIONS: We concluded that the arrythmogenic doses of epinephrine during sevoflurane were significantly higher than those during halothane anesthesia.

5.Clinical significance of urinary growth hormone measurement in patients with growth hormone deficiency.

Urinary growth hormone (GH) excretion was quantitated in 12-hr overnight urine collections obtained from 13 children with complete growth hormone deficiency (CGHD). 6 children with partial growth hormone deficiency (PGHD), 5 children with short stature and normal GH provocation tests (NSC), and 5 normal control children (NC) to investigate whether the measurement of urinary GH can clearly separate the PGHD and CGHD groups from the NSC and NC groups. In addition, the urinary excretion of GH was measured in CGHD after sc injections of 0.1 IU/kg GH three times a week and daily in an attempt to determine the optimal replacement dose. The results were as follows. 1) The 12-hr urinary GH excretion cleary separated the CGHD (1.2+/-1.1 ng/12 hr; range, 0.3-3.1, n=13) and PGHD (3.7+/-2.4 ng/12 hr; range, 0.9-6.8 n=6) groups from the NSC (14.0+/-3.6 ng/12 hr; range, 8.5-18.2 n=5) and NC (12.7+/-5.8 ng/12 hr; range, 6.9-20.8, n=5) groups without any overlap. 2) There were significant difference in 12-hr urinary GH excretion between CGHD and PGHD, but the higher values in CGHD groups overlapped some of the lower values in PGHD group. 3) There were no differencies in 12-hr urinary GH excretion between NC and NSC. 4) A 12-hr urinary GH values less than 6.9 ng/12 hr suggested GH deficiency. 5) The CGHD patients who received o.1 IU/kg GH. sc. daily GH levels within the range of the mean +/-SD in NC. but those who received theree times a week had daily variable urinary GH levels. These results suggest that the measurement of 12-hr nocturnal urinary GH excretion monitering of GH therapy response.

6.Lectins Binding in Squamous Cell Carcinoma of the Uterine Cervix as a Diagnostic and Prognostic Marker.

OBJECTIVE: The aim of the study was to evaluate the corelation between an expression of lectins and survival of patients with squamous cell carcinoma of uterine cervix. METHODS: The cell surface carbohydrate profile of formalin-fixed paraffin embedded tissue section of squamous cell carcinoma of the uterine cervix was evaluated using ulex europaeus agglutinin(UEA-1), peanut agglutinin(PNA), dolichos biflorus agglutinin(DBA), soybean agglutinin(SBA) and lotus tetragonobus lectin(LTL) by the avidin-biotin complex method. Fifty-one cases of squamous cell carcinoma and 10 cases of normal squamous epithelium of the uterine cervix were selected from the file which were treated during Jan. 89 to 31 Dec. 1992 in Department of Obstetrics and Gynecology, Korea University Hospital. RESULTS: UEA-1 and LTL were negative in normal cervix while positive in 76.5%, 47.1% of squamous cell carcinoma respectively, and useful markers for differential diagnosis between normal and squamous cell carcinoma. SBA and LTL were useful for differential diagnosis of keratinizing and non-keratinizing squamous cell carcinoma. UEA-1 may play an important role in lymphovascular invasion of squamous cell carcinoma of the uterine cervix. CONCLUSIONS: There was no correlation among clinical staging, patient's survival and lectins binding in squamous cell carcinoma.

7.Percutaneous Balloon Mitral Valvuloplasty Guided by Transesophageal Echocardiography.

BACKGROUND: Balloon mitral valvuloplasty is a favorable procedure as a therapy for mitral stenosis because it minimizes morbidity and shorten hospital stay compared with surgical mitral commissurotomy or mitral valve replacement. Recent reports about concomitant transesophageal echocardiography guide in addition to fluoroscopy suggest that transesophageal echocardiograpy can provide additional benefits during balloon mitral valvuloplasty especially in transseptal puncture, balloon positioning, evaluation of immediate result, and early detection of complications. We performed this study to identify the potential benefits of on-line transesophageal echocardiography guide during balloon mitral valvuloplasty. METHOD: We performed balloon mitral valvuloplasty under on-line transesophageal echocardiography guide in addition to fluoroscopy in 70 patients(male:14, female:56, mean age:44+/-13) with rheumatic mitral stenosis from May 1995 to May 1996. Thirty-two(46%) patients had atrial fibrillation. Included patients were symptomatic with more than NYHA class 2 symptom. Patients with mitral valve score more than 11 and mitral regurgitation more than 2/4 were excluded. Inoue balloons were utilized in all cases. RESULTS: The average mitral valve area increased from 0.9+/-0.2cm2 before valvuloplasty to 1.8+/-0.4cm2 after valvuloplasty(p<0.0001). The averagetransmitral pressure gradient measured by continuous wave Doppler decreased from 14+/-6mmHg before valvuloplasty to 5+/-2mmHg after valvuloplasty(p<0.0001), and the average left atrial pressure measured by catheterization decreased form 22+/-8 mmHg before valvuloplasty to 11+/-5mmHg after valvuloplasty(p<0.0001). The average procedure time was 64+/-22 minutes(ranged from 13 to 150 minutes) and the average fluoroscopy time was 19+/-15 minutes(ranged from 1 to 94 minutes). Two patients underwent surgery due to severe mitral regurgitation associated with papillary muscle rupture which developed after valvuloplasty. In one patient, transesophageal echocardiography detected pericaridal tamponade during the procedure and the transducer was quickly switched to transthoracic transducer to guide the pericardial puncture site. The pericardial tamponade was drained with pigtail catheter and the patient underwent balloon mitral valvuloplasty successfully a week later. Four patients were pregnant at the time of the valvuloplasty procedure and the valvuloplasty was successfully performed with minimal fluoroscopy time(1-3 minutes) without complications in all four patients. Five patients had thrombus in left atrial appendage, but the transesophageal echocardiography was useful in monitoring the ballon position during the procedure and the valvuloplasty was successfully performed without embolic complications in all five patients. CONCLUSION: The transesophageal echocardiography is a very useful guiding adjunct during balloon mitral valvuloplasty in transseptal puncture, balloon positioning, evaluation of immediate result, early detection of complications, and shortening fluoroscopy time especially in pregnant women.

8.Percutaneous Mitral Balloon Valvuloplasty in Patients with Left Atrial Appendage Thrombi.

BACKGROUND: Percutaneous mitral balloon valvuloplasty(PMV) is a good treatment modality for patient with mitral stenosis(MS). But it is considered relatively contraindicated in patients with left artrial thrombi because of high risk of embolism. Limitted studies have suggested the feasibility of PMV in patients with left atrial appendage(LAA) thrombi. This study was performed to evaluate the feasibility and safty of PMV in patients with LAA thrombi using Inoue balloon under the transesophageal echocardiographic (TEE) monitoring. METHOD: PMV was performed in 5 patients diagnosed as MS with LAA thrombi from October, 1995 to July, 1996. Four cases were female, and one case was male. Their mean age was 525(46-58years old). Two of them had history of cerebrovascular accident(CVA). The duration of anticoagulant treatment was 6-49 moths. All patients underwent PMV using Inoue balloon catheter under the TEE monitoring. RESULTS: EKG finding of all 5 patients were atrial fibrillation(Af). Their mitral valve score were 5-10(Mean score was 82). Transmitral mean pressure gradient was decreased from 14.62.1 to 5.82.0mmHg, and mitral valve increased from 0.840.43 to 1.720.19 after PMV. There was no procedure related complication. In 3 cases of them LAA thrombi diappeared in the follow up TEE. In two patients, the LAA thrombi were calcified and remained unresolved at the time of follow up TEE( 6month-and 12 month-F/U, eath). CONCLUSION: Although the reported number of PMV in patients with LAA thrombi is small in this study, we believe that, with special precaution and TEE monitoring, LAA thrombi is no longer an absolute contraindication to PMV.

9.Congenital Agenesis of Odonteid Process: A Case Report

Anomalous development of the odonteid precess is an uncommon clinical occurence that can seriously impair the integrity af the atlantoaxial articulation. Absence of the odontoid process may be either congenital or acquired, but clinically the etiology is of little importance as the clinical signs and symptoms and the treatment are identical in both. We experienced a case of congenital absence of the odontoid process and good result was obtained from the posterior atlantoaxial fusion with hale-cast.

10.Surface Replacement Arthroplasty of the Hip

The concept of surface replacement arthroplasty of the hip is to replace the diseased joint surface with artificial plastic and metal cup in the acetabular and femoral head respectively, and to restore the normal anatomy and gain biomechanical function. Ten cases of Wagner type's surface replacement arthroplasty performed in Department of Orthopedic Surgery, Seoul National University Hospital since July 1979 are reviewed, and their minimum follow up is two months. In terms of the Pain control and improvement of range of motion in the hip, surface replacement arthroplasty showed about the same result compared with conventional total hip. In our cases there was no one complicatian, even though it is too short to evaluate the result of this operation. Wagner recommends surface replacement arthroplasty to perform through anterior approach, but 2 cases of our ten patients that have relatively good range of motion of the hip could be operated on this surgery by anterolateral approach (oblique skin incision) without trochanteric osteotomy, because of high incedence of myositis ossification with anterior approach. It is our conclusion that surface replacement arthroplasty of the hip may be used as an interim procedure to gain time in young patient who needs conventional total hip replacement which reveals increasing complication with the lapse of time.
